In preparation for the 2021 revision of the European Union Tobacco Products Directive, the Scientific Committee on Health, Environmental and Emerging Risks (SCHEER) has posted its Preliminary Opinion on Electronic Cigarettes. They concluded that e-cigarettes only achieve a sub-optimal level of protection of human health. In this
paper, we provide evidence that the Opinion’s conclusions are not adequately backed up by scientific evidence and did not discuss the potential health benefits of using alternative combustion-free nicotine-containing products as substitute for tobacco cigarettes.
Methods:
Searches for articles were conducted in PubMed and by citation chasing in Google Scholar. Articles were also retrieved with a review of references in major publications. Primary data from World Health Organization surveys, the conclusions of reviews, and peer-reviewed non-industry studies were cited to address errors and omissions identified in the Opinion.
Results:
The Opinion omitted reporting on the individual and population health benefits of the substitution of e-cigarettes (ENDS) for cigarette smoking. Alternative hypotheses to the gateway theory were not evaluated. Its assessment of cardiovascular risk is contradicted by numerous reviews. It cites ever-use data that do not represent current patterns of use. It did not report non-nicotine use. It presented erroneous statements on trends in ENDS prevalence. It over-emphasized the role of flavours in youth ENDS initiation. It did not discuss cessation in sufficient length.
Conclusions:
For the delivery of a robust and comprehensive final report, the members of the Working Group of the Scientific Committee on Health, Environmental and Emerging Risks will need to consider (1) the potential health benefits of ENDS substitution for cigarette smoking, (2) alternative hypotheses and contradictory studies on the gateway effect, (3) its assessment of cardiovascular risk, (4) the measurements of frequency of use, (5) non-nicotine use, (6) the role of flavours, and (7) a fulsome discussion of cessation.Permalink : https://biblio.fares.be/opac_css/index.php?lvl=notice_display&id=9869 Aucun avis, veuillez vous identifier pour ajouter le vôtre !

Electronic cigarettes (e-cigarettes) are often promoted to assist with cigarette smoking cessation. In 2016–2017, the relationship between e-cigarette use and having stopped smoking among ever (current and former) smokers was assessed in the European Union and Great Britain by itself.
Methods:
Cross-sectional logistic regression of the association between being a former smoker and e-cigarette use was applied to the 2014 Eurobarometer survey of 28 European Union countries controlling for demographics.
Results:
Among all ever smokers, any regular ever use of nicotine e-cigarettes was associated with
lower odds of being a former smoker (unadjusted OR¼0.34, 95% CI¼0.26, 0.43, AOR¼0.43, 95% CI¼0.32, 0.58) compared with smokers who had never used e-cigarettes. In unadjusted models, daily use (OR¼0.42, 95% CI¼0.31, 0.56); occasional use (OR¼0.25, 95% CI¼0.18, 0.35); and experimentation (OR¼0.24, 95% CI¼0.19, 0.30) of nicotine e-cigarettes were associated with lower odds of being a former smoker compared with having never used nicotine-containing e-cigarettes. Comparable results were found in adjusted models. Results were similar in Great Britain alone. Among current smokers, daily cigarette consumption was 15.6 cigarettes/day (95% CI¼14.5, 16.7) among those who also used e-cigarettes versus 14.4 cigarettes/day (95% CI¼13.4, 15.4) for those who did not use them (po0.05).
Conclusions:
These results that e-cigarettes are associated with inhibiting rather than assisting in smoking cessation. On the population level, the net effect of the entry of e-cigarettes into the European Union (and Great Britain) is associated with depressed smoking cessation of conventional cigarettes.

[TABAC] préventionIndex. décimale : TA 0.3 Modes d'action des professionnels Résumé : The European Network for Smoking Prevention was created in 1997 with the mission of developing a co-ordinated approach among non-governmental organisations active in tobacco control in Europe. One of the objectives is to initiate and co-ordinate joint framework grant applications to the European Commission in the field of tobacco control. The aim being to promote high quality, European projects with a wide geographical coverage, and by doing do adding value to single, stand-alone projects. From past experience there was a clear need for a change in the application procedure. Overlapping between projects was frequent and projects generally showed very little European added value. In response to this ENSP proposed to the European Commission that it changed its procedure from passive project collection to pro-active, co-ordinated project building. Instead on introducing a multitude of small applications, ENSP suggested that interested parties should be invited to submit project proposals to ENSP several months before the submission deadline of the Commission. For this purpose an innovative project building procedure was developed which would become an accepted, ongoing process. Each year ENSP develops action priorities to promote a co-ordinated approach to European tobacco control policy. Based on these priorities combined with the funding conditions of the European Commission, a call for expression of interest to the tobacco control community is launched several months in advance of the official
Commission call for proposals. Those project proposals that can be developed into large scale European grant applications are grouped according to priority sectors. The project building procedure starts with a workshop in Brussels where EU funding conditions are explained and project partners have the opportunity to meet, share views and start developing the co-ordinated project grant application. Once individual project grant applications have been developed, they are regrouped into an ENSP framework grant application. The aim of this survey was to collect information about healthcare policies, particularly policies related to prevention, early diagnosis, support measures and therapies such as pulmonary rehabilitation for COPD patients. The results will help to understand the decision- making processes related to prevention, early diagnosis and rehabilitation, and also to identify the decision makers in the countries surveyed.
The ultimate goal is to use this information to tailor advocacy campaigns, at national level, targeted to national decision makers in order to tackle the gaps in prevention and care in each country, and thus bring about a concrete improvement of COPD policies in Europe as well as harmonizing healthcare for COPD patients throughout the continent. Methods The data were collected during the summer 2014 via telephone interviews (or via email) with national healthcare professionals who are expert in COPD using a structured questionnaire during the summer 2014 with open reply options (Appendix 1).
The questions were aimed at obtaining qualitative data. A selection of possible reply options was provided to assist respondents. Missing or complementary data were obtained from the European Observatory on Health Systems and Policies reports and from other official sources. These sources are indicated at the beginning of the section dedicated to each country. EFA members were invited to add patient focused information. Results We collected information on 19 countries: telephone interviews were conducted with experts in most countries, whereas the Czech Republic and Portugal provided written replies.
