Notre étude vise à étudier le rôle des liens d'amitié dans cette association. Sur un échantillon de 11 015 adolescents âgés de 14 à 17 ans, répartis dans 50 écoles de six villes européennes (enquête SILNE, 2013), nous avons utilisé des modèles à plusieurs niveaux pour analyser l'effet de médiation de la composition des liens d'amitié et des types d'école sur l'association entre les performances scolaires. et fumer. Les résultats montrent que le tabagisme était plus répandu chez les adolescents ayant un rendement scolaire inférieur que chez ceux ayant un rendement supérieur. Cette association était plus forte dans les écoles non professionnelles que dans les écoles professionnelles. Les adolescents avaient tendance à avoir des liens d'amitié avec une personne partageant le même statut de fumeur et le même rendement scolaire. Finalement, les réseaux d’amitié sont modelés à la fois sur le tabagisme et les résultats scolaires. Many carbonyls are produced from the combustion of tobacco products and many of these carbonyls are harmful or potentially harmful constituents of mainstream cigarette smoke.
One carbonyl of particular interest is acrolein, which is formed from the incomplete combustion of organic matter and the most significant contributor to non-cancer respiratory effects from cigarette smoke. Sheet-wrapped cigars, also known as “little cigars,” are a type of tobacco products that have not been extensively investigated in literature.
Methods:
This study uses standard cigarette testing protocols to determine the acrolein yields from sheet-wrapped cigars. Sheet-wrapped cigar and cigarette products were tested by derivatizing the mainstream smoke with 2,4-dinitrophenylhydrazine (DNPH) solution and then quantifying the derivatives using conventional analytical systems.
Results:
The results demonstrate that sheet-wrapped cigars can be tested for acrolein yields in mainstream smoke using the same methods used for the evaluation of cigarettes. The variability in the sheet-wrapped cigars and cigarettes under the International Organization for Standardization smoking regimen is statistically similar at the 95% confidence interval; however, increased variability is observed for sheet-wrapped cigar products under the Health Canada Intense (CI) smoking regimen.
Conclusion: The amount of acrolein released by smoking sheet-wrapped cigars can be measured using standard smoking regimen currently used for cigarettes. The sheet-wrapped cigars were determined to yield similar quantity of acrolein from commercial cigarette products using two standard smoking regimens.
Implications:
This article reports on the measured quantity of acrolein from 15 commercial sheetwrapped cigars using a validated standard smoking test method that derivatizes acrolein in the
mainstream smoke with DNPH solution, and uses Liquid Chromatography/Ultra-Violet Detection (LC/UV) for separation and detection. These acrolein yields were similar to the levels found in the smoke from 35 commercial cigarette products measured in the same manner. Although sheetwrapped cigar data were slightly more variable than those found for the cigarette data, this article reports that the production of acrolein is similar to cigarettes. Acupuncture and related techniques are promoted as a treatment for smoking cessation in the belief that they may reduce nicotine withdrawal symptoms.
Objectives
The objectives of this review are to determine the eBectiveness of acupuncture and the related interventions of acupressure, laser therapy and electrostimulation in smoking cessation, in comparison with no intervention, sham treatment, or other interventions.
Search methods
We searched the Cochrane Tobacco Addiction Group Specialized Register (which includes trials of smoking cessation interventions identified from the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE, EMBASE, and PsycINFO) and AMED in October 2013. We also searched four Chinese databases in September 2013: Sino-Med, China National Knowledge Infrastructure, Wanfang Data and VIP.
Selection criteria
Randomized trials comparing a form of acupuncture, acupressure, laser therapy or electrostimulation with either no intervention, sham treatment or another intervention for smoking cessation.
Data collection and analysis
We extracted data in duplicate on the type of smokers recruited, the nature of the intervention and control procedures, the outcome measures, method of randomization, and completeness of follow-up. We assessed abstinence from smoking at the earliest time-point (before six weeks) and at the last measurement point between six months and one year. We used the most rigorous definition of abstinence for each trial, and biochemically validated rates if available. Those lost
to follow-up were counted as continuing smokers. Where appropriate, we performed meta-analysis pooling risk ratios using a fixed-eBect model.
Main results
We included 38 studies. Based on three studies, acupuncture was not shown to be more eBective than a waiting list control for longterm abstinence, with wide confidence intervals and evidence of heterogeneity (n = 393, risk ratio [RR] 1.79, 95% confidence interval [CI] 0.98 to 3.28, IO = 57%). Compared with sham acupuncture, the RR for the short-term eBect of acupuncture was 1.22 (95% CI 1.08 to 1.38),and for the long-term eBect was 1.10 (95% CI 0.86 to 1.40). The studies were not judged to be free from bias, and there was evidence of funnel plot asymmetry with larger studies showing smaller eBects. The heterogeneity between studies was not explained by the technique used. Acupuncture was less eBective than nicotine replacement therapy (NRT). There was no evidence that acupuncture is superior to
psychological interventions in the short- or long-term. There is limited evidence that acupressure is superior to sham acupressure for shortterm outcomes (3 trials, n = 325, RR 2.54, 95% CI 1.27 to 5.08), but no trials reported long-term eBects, The pooled estimate for studies testing an intervention that included continuous auricular stimulation suggested a short-term benefit compared to sham stimulation (14 trials, n = 1155, RR 1.69, 95% CI 1.32 to 2.16); subgroup analysis showed an eBect for continuous acupressure (7 studies, n = 496, RR 2.73, 95% CI 1.78 to 4.18) but not acupuncture with indwelling needles (6 studies, n = 659, RR 1.24, 95% CI 0.91 to 1.69). At longer follow-up the CIs did not exclude no eBect (5 trials, n = 570, RR 1.47, 95% CI 0.79 to 2.74). The evidence from two trials using laser stimulation was inconsistent and could not be combined. The combined evidence on electrostimulation suggests it is not superior to sham electrostimulation (shortterm
abstinence: 6 trials, n = 634, RR 1.13, 95% CI 0.87 to 1.46; long-term abstinence: 2 trials, n = 405, RR 0.87, 95% CI 0.61 to 1.23).
Authors' conclusions
Although pooled estimates suggest possible short-term eBects there is no consistent, bias-free evidence that acupuncture, acupressure, or laser therapy have a sustained benefit on smoking cessation for six months or more. However, lack of evidence and methodological problems mean that no firm conclusions can be drawn. Electrostimulation is not eBective for smoking cessation. Well-designed research into acupuncture, acupressure and laser stimulation is justified since these are popular interventions and safe when correctly applied, though these interventions alone are likely to be less eBective than evidence-based interventions.

Addiction to nicotine gum has never been described in never smokers or in never users of tobacco.
Methods
Internet questionnaire in 2004–2006 in a self-selected sample of 434 daily users of nicotine gum. To assess dependence on nicotine gum, we used modified versions of the Nicotine Dependence Syndrome Scale (NDSS), the Fagerström Test for Nicotine Dependence and the Cigarette Dependence Scale.
Results
Five never smokers used the nicotine gum daily. They had been using the nicotine gum for longer than the 429 ever smokers (median = 6 years vs 0.8 years, p = 0.004), and they had higher NDSS-gum Tolerance scores (median = 0.73 vs = -1.0, p = 0.03), a difference of 1.5 standard deviation units. Two never smokers had never used smokeless tobacco, both answered "extremely true" to: "I use nicotine gums because I am addicted to them", both "fully agreed" with: "after a few hours without chewing a nicotine gum, I feel an irresistible urge to chew one" and: "I am a prisoner of nicotine gum".
Conclusion
This is to our knowledge the first report of addiction to nicotine gum in never users of tobacco. PURPOSE:
To examine the effect of plain packaging on adolescents' perceptions of cigarette packs, attributes of smokers, and expectations of cigarette taste, and to identify the effect of increasing the size of pictorial health warnings on appraisal of plain packs.
METHODS:
We used a 5 (degree of plain packaging and graphic health warning)x 3 (brand type) between-subjects experimental design, using a Web-based methodology to expose adolescents to one randomly selected cigarette pack, during which respondents completed ratings.
RESULTS:
When brand elements such as color, branded fonts, and imagery were progressively removed from cigarette packs, adolescents perceived packs to be less appealing, rated attributes of a typical smoker of the pack less positively, and had more negative expectations of cigarette taste. Pack appeal was reduced even further when the size of the pictorial health warning on the most plain pack was increased from 30% to 80% of the pack face, with this effect apparent among susceptible nonsmokers, experimenters, and established smokers.
CONCLUSIONS:
Removing as much brand information from cigarette packs as possible is likely to reduce positive cigarette brand image associations among adolescents. in vitro approach to assess the biological impact of differing e-cigarette activation mechanism (button vs puff-activated) and heating technology (cotton vs ceramic wick). The e-liquids selected for each device contained the same nicotine concentration and flavourings. We tested both e-liquid and aqueous extract of e-liquid aerosol using a high throughput
cytotoxicity and genotoxicity screen. We also conducted whole aerosol assessment both in a reconstituted human airway lung tissue (MucilAir) with associated endpoint assessment
(cytotoxicity, TEER, cilia beat frequency and active area) and an Ames whole aerosol assay with up to 900 consecutive undiluted puffs. Following this testing it is shown that the
biological impact of these devices is similar, taking into consideration the limitations and capturing efficiencies of the different testing matrices. We have contextualised these
responses against previous published reference cigarette data to establish the comparative reduction in response consistent with reduced risk potential of the e-cigarette products

Il est décrit dans la littérature que la consommation de tabac augmente le seuil de perception du sel et que les fumeurs ont une alimentation plus riche en sel ce qui favorise le risque de maladies cardio-vasculaires. Il est également décrit que cet impact négatif du tabac sur la perception du sel est réversible lors de son arrêt. L’objectif de ce travail était de vérifier si lors de l’arrêt du tabac, après récupération du goût et de l’odorat, les personnes se dirigent spontanément vers une alimentation qualitativement moins salée ou non. Le travail consistait en la réalisation d’une étude qualitative de l’évolution de la consommation en aliments riches en sel et en l’utilisation de ce dernier, lors de l’arrêt du tabac. Les sujets ont été sélectionnés parmi les patients suivis au centre d’aide aux fumeurs au sein des Cliniques Universitaires Saint-Luc. Ils ont reçu deux questionnaires de fréquence de consommation d’aliments sources et riches en sel : un avant et un après servage. D’après l’étude réalisée, il y a peu de différence entre les deux échantillons, fumeurs et exfumeurs. Il y a 4 catégories de produits pour lesquelles il existe une différence significative. La fréquence de consommation de charcuterie fumée et/ou salée, de fromage bleu, de lards et de lardons est plus faible chez les ex-fumeurs que chez les fumeurs. Ces résultats sont discutables car le nombre de patients ayant participé à l’étude jusqu’au bout s’élève à 8. Cet échantillon est donc peu représentatif de l’ensemble de la population. Current levels of exposure to tobacco smoke may also be responsible for the development of chronic cough in both children and adults. The present study analyses the effects of tobacco smoke exposure as potential causes of chronic cough. A panel of PubMed-based searches was performed relating the symptom of cough to various forms of tobacco smoke exposure. It was found that especially prenatal and postnatal exposures to ETS have an important influence on children's respiratory health including the symptom of cough. These effects may be prevented if children and pregnant women are protected from exposure to ETS. Whereas the total number of studies adressing the relationship between cough and ETS exposure is relatively small, the present study demonstrated that there is a critical amout of data pointing to a causative role of environmental ETS exposure for the respiratory symptom of cough. Le tabagisme est responsable d’une lourde morbimortalité. Cependant, une optimisation de sa prise en charge contribue à la réduction des pathologies qui en écoulent.
Objectif :
L’objectif de cette étude était de développer, organiser et évaluer une formation basée sur l’approche par compétences (APC) auprès des résidents au Liban leur permettant de savoir agir devant un patient fumeur.
Méthodes :
La formation a été planifiée en débutant par la détermination des tâches à accomplir et des compétences visées. Une étude descriptive transversale auprès de 16 résidents a relevé leurs besoins éducatifs permettant ainsi la formulation des objectifs pédagogiques. Le moyen d’apprentissage choisi était l’atelier. Une analyse pré/postintervention a permis d’évaluer les progrès.
Résultats :
L’analyse des besoins a montré un niveau de maîtrise insuffisant dans l’approche du patient fumeur ainsi que l’existence d’obstacles (score « Connaissances générales» : 7,1/10 ; « Pratiques » : 6,5/10 ; « Compétences » : 3,8/10 ; « Obstacles » : 5,6/10). L’analyse pré/postintervention a montré une amélioration
significative de tous les scores ainsi qu’une diminution significative des obstacles (score « Connaissances générales » : 9,4/10 ; « Pratiques » : 9,2/10 ; « Compétences » : 8,3/10 ; « Obstacles » : 2,1/10). L’évaluation de l’atelier était nettement favorable. Conclusion : Un atelier basé sur l’APC est efficace en termes
d’acquisition des compétences professionnelles nécessaires à l’optimisation de l’aide au sevrage tabagique chez les résidents. Il devrait à ce titre être intégré dans le cursus médical et délivré à tout médecin, a fortiori dans un pays présentant un taux de tabagisme des plus élevés et des politiques antitabac presque

The objective of the present study was to determine whether single nucleotide polymorphisms (SNPs) of CSF2 and CSF3 are associated with lung function in smoking-induced chronic obstructive pulmonary disease. In total, five SNPs of CSF2 and CSF3 were studied in 587 non-Hispanic white subjects with the fastest (n = 281) or the slowest (n = 306) decline of lung function selected from among continuous smokers in the National Heart, Lung, and Blood Institute Lung Health Study (LHS). These SNPs were also studied in 1,074 non-Hispanic white subjects with the lowest (n = 536) or the highest (n = 538) baseline lung function at the beginning of the LHS. An increase in the number of CSF3 -1719T alleles was significantly associated with protection against low lung function (odds ratio 0.73, 95% confidence interval 0.56-0.95), and was still significant after adjustment for multiple comparisons. There was also a significant association of a CSF3 haplotype with baseline levels of forced expiratory volume in one second. No association was found for CSF2 SNPs and lung function, nor was there evidence of epistasis. In conclusion, genetic variation in colony-stimulating factor 3 is associated with cross-sectionally measured lung function in smokers. Cet article décrit la variété des profils de consultants et la réponse qui leur est offerte, en retraçant les parcours individuels au sein du dispositif. Le recueil des données a été effectué par questionnaire anonyme auprès des professionnels accueillants entre le 20 mars et le 20 avril 2007, avec un suivi des patients jusqu’au 30 juin 2007. L’analyse descriptive a été présentée par sexe et par âge celle des facteurs prédictifs du décrochage et des tentatives réussies de réduction de l’usage de cannabis a été réalisée par analyse multivariée. Résultats : Sur 3 788 consultants reçus dans 214 des 274 CJC, 2 938 sont des consommateurs, pour 81 % masculins, généralement majeurs (83 %). Près de 40 % sont usagers quotidiens de cannabis. La moitié des consultants sont orientés par la justice, contre 22 % de demandes spontanées et 30 % d’orientations familiales, éducatives ou médico-sociales. Plus de la moitié des usagers (53 %) sont diagnostiqués en situation d’abus ou de dépendance. L’analyse décrit les facteurs influant sur la « réussite » des tentatives de réduction de l’usage : la fréquence d’usage du cannabis, le motif d’usage, l’intégration socioprofessionnelle, le nombre de visites, le sexe et l’âge. Elle caractérise aussi le public exposé au risque de « décrocher » en cours de suivi (30 % en moyenne), en soulignant le poids du sexe, du rang de consultation, des attentes à l’entrée dans le dispositif, des motifs d’usage du cannabis. PURPOSE:
Smoking is the leading controllable risk factor for heart disease. Only about 69% of U.S. indoor workers are currently covered by a smoke-free workplace policy. This analysis projects the cardiovascular health and economic effects of making all U.S. workplaces smoke free after 1 year and at steady state.
METHODS:
We estimated the number of U.S. indoor workers not covered by smoke-free workplace policies, and the effects of making all workplaces smoke free on smoking behavior and on the relative risks of acute myocardial infarctions and strokes. One-year and steady-state results were calculated using an exponential decline model. A Monte Carlo simulation was performed for a sensitivity analysis.
RESULTS:
The first-year effect of making all workplaces smoke free would produce about 1.3 million new quitters and prevent over 950 million cigarette packs from being smoked annually, worth about 2.3 billion dollars in pretax sales to the tobacco industry. In 1 year, making all workplaces smoke free would prevent about 1500 myocardial infarctions and 350 strokes, and result in nearly $60 [corrected] in savings in direct medical costs. At steady state, 6250 myocardial infarctions and 1270 strokes would be prevented, and $279 million [corrected] would be saved in direct medical costs annually. Reductions in passive smoking would account for 60% of effects among acute myocardial infarctions.
CONCLUSION:
Making all U.S. workplaces smoke free would result in considerable health and economic benefits within 1 year. Reductions in passive smoking would account for a majority of these savings. Fine particulate matter exposure from both ambient air pollution and secondhand cigarette smoke has been associated with larger risks of cardiovascular mortality than would be expected on the basis of linear extrapolations of the relative risks from active smoking. This study directly assessed the shape of the exposure-response relationship between cardiovascular mortality and fine particulates from cigarette smoke and ambient air pollution.
METHODS AND RESULTS:
Prospective cohort data for >1 million adults were collected by the American Cancer Society as part of the Cancer Prevention Study II in 1982. Cox proportional hazards regression models that included variables for increments of cigarette smoking and variables to control for education, marital status, body mass, alcohol consumption, occupational exposures, and diet were used to describe the mortality experience of the cohort. Adjusted relative risks of mortality were plotted against estimated average daily dose of fine particulate matter from cigarette smoke along with comparison estimates for secondhand cigarette smoke and air pollution. There were substantially increased cardiovascular mortality risks at very low levels of active cigarette smoking and smaller but significant excess risks even at the much lower exposure levels associated with secondhand cigarette smoke and ambient air pollution.
CONCLUSIONS:
Relatively low levels of fine particulate exposure from either air pollution or secondhand cigarette smoke are sufficient to induce adverse biological responses increasing the risk of cardiovascular disease mortality. Cigarette smoke (CS) is a major risk factor for the development of chronic obstructive pulmonary disease. This study mainly aims to investigate the effect of CS on rat BD-2 (rBD-2) expression in rat airways. Rats were exposed to CS and treated with caffeic acid phenethyl ester (CAPE), a nuclear factor (NF)-κB inhibitor, or astragaloside IV (AS-IV), an active ingredient of Astragalus mongholicus. Besides the analysis of bronchoalveolar lavage fluid (BALF) and histological changes after CS exposure, rBD-2 expression was investigated with immunohistochemistry, reverse transcription PCR and ELISA. Total glutathione and nitric oxide (NO) levels in rat lungs were also detected. CS exposure markedly increased rBD-2 immunoreactivity, as well as rBD-2 mRNA and protein levels in rat airways, which were inhibited by CAPE treatment. Moreover, associated airway inflammation induced by CS was demonstrated by histological changes, increased cell counts and pro-inflammatory cytokines in BALF, and NF-κB activation and high levels of total glutathione and NO, which were all reversed by AS-IV in a dose-dependent fashion. Airway epithelial cells (AEC) are exposed to various environmental insults like cigarette smoke and bacterial infections, but their direct effect on VEGF production in well-differentiated primary human AEC remains unclear. The current authors determined the effect of cigarette smoke extract (CSE) alone and in combination with Mycoplasma pneumoniae (Mp) on VEGF production in well-differentiated primary normal human bronchial epithelial (NHBE) and small airway epithelial cells (SAEC) in air-liquid interface cultures. Secretion and expression of VEGF were determined by ELISA and real-time RT-PCR, respectively. Cell growth, apoptosis, extracellular signal-regulated kinase (ERK)1/2 and protein kinase (PK)C signalling pathways were evaluated to further dissect VEGF regulation under CSE treatment. CSE significantly reduced VEGF secretion in NHBE and SAEC. In SAEC, Mp alone significantly increased the VEGF, while the presence of CSE attenuated Mp-induced VEGF production. While ERK inhibitor reduced VEGF secretion only in NHBE, a PKC inhibitor significantly decreased VEGF secretion in both NHBE and SAEC. In conclusion, direct cigarette smoke extract exposure significantly reduced vascular endothelial growth factor production in well-differentiated primary human airway epithelial cells, in part through modifying extracellular signal-regulated kinase 1/2 and protein kinase C signalling pathways. The characteristic changes in airway morphology, inflammatory cell infiltration and mediator expression in COPD may result from direct effects of cigarette smoke on airway cells. Toll-like receptors (TLRs) are key elements in pathogen recognition by the host immune system. Although TLRs have been intensely studied in innate immunity and infection, their critical role in noninfectious challenges has only recently emerged. Here we investigate whether cigarette smoke induces TLR9 signalling in human neutrophils. Human neutrophils were isolated from buffy coat and exposed to cigarette smoke extract. The production of CXC chemokine ligand (CXCL)8 was measured as a functional readout and the role of TLR9 signalling was investigated. Cigarette smoke extract induced CXCL8 release via TLR9 activation in neutrophils, which was confirmed in TLR9 stably transfected human embryonic kidney 293 cells. Moreover, cigarette smoke extract upregulated the expression of TLR9 and the upregulated expression was suppressed by N-acetylcysteine. TLR9 mediates cigarette smoke-induced release of CXCL8 and this may contribute to the accumulation of neutrophils and inflammation within the airways of smokers. Co-use of tobacco and cannabis is a common and complex behavior. The lack of harmonized measures of co-use yields confusion and inconsistencies in synthesizing evidence about the health effects of co-use. We aimed to classify co-use patterns based on temporal proximity and describe preferred products and motives for each
pattern in order to improve co-use surveillance.
Methods: We conducted semi-structured interviews in a sample of 34 young adults (Mage = 22.8 years, 32.4 % female) during 2017–2019 in California, USA. We employed a qualitative thematic analysis to identify timing, reasons, and contexts for tobacco and cannabis co-use and classify co-use patterns.
Results:
Four emergent patterns of co-use with increasing temporal proximity between tobacco use and cannabis use were: Same-month different-day co-use (Pattern 1); Same-day different-occasion co-use (Pattern 2); Same-occasion sequential co-use (Pattern 3); and Same-occasion simultaneous co-use (Pattern 4). Participants used various product combinations within each pattern. Similar motives for all patterns were socialization, product availability, and coping with stress/anxiety. Unique motive for temporally distant patterns (Patterns 1 and 2) was seeking substancespecific effects (e.g., stimulant effect from nicotine, relaxation effects from cannabis), while unique motives for temporally close patterns (Patterns 3 and 4) were seeking combined effects from both substances (e.g., more intense psychoactive effects, mitigating cannabis adverse effects) and behavioral trigger (e.g., cannabis use triggers tobacco use).
Conclusions:
Our classification of co-use patterns can facilitate consistency for measuring co-use and assessing its health impacts. INTRODUCTION:
A paucity of data exists regarding the safety and effectiveness of combination treatment with varenicline and nicotine replacement therapy (NRT).
METHODS:
We reviewed the clinical experience of two groups of cigarette smokers enrolled in a residential tobacco treatment program: (a) patients receiving combination treatment with varenicline and NRT (N = 104) and (b) usual-care patients receiving treatment before the release of varenicline (N = 135).
RESULTS:
Demographic characteristics were similar between the two groups. Among smokers receiving varenicline and NRT, 71% used the nicotine patch with a mean dose of 32 mg/day (SD = 14) and 73% used at least two types of NRT. Adverse events were experienced by 39% (95% CI = 31%-49%) of patients receiving varenicline and NRT and by 59% (95% CI = 51%-67%) of usual-care patients during the residential program. A total of five patients (5%) discontinued varenicline due to adverse events, compared with one patient in the usual-care group. We did not observe a significant difference in the 30-day point prevalence smoking abstinence rate at 6 months between patients treated with varenicline and NRT (54%; 95% CI = 44%-64%) and usual-care patients (59%; 95% CI = 50%-66%).
DISCUSSION:

It has been suggested that active smoking in adolescence and preexisting airflow obstruction have synergistic deleterious effects.
Objective:
We used a mouse model to investigate whether there is a synergistic effect of exposure to CS in utero and during adolescence on lung function.
Methods:
Female C57Bl/6J mice were exposed to CS or to filtered room air during pregnancy. Exposure to CS began 2 weeks before mating and continued until delivery. After birth, the pups were not exposed to CS until day 21 (D21). Between D21 and D49, corresponding to “adolescence,” litters were randomized for an additional 4 weeks of exposure to CS. Lung morphometry, lung mechanics, and the expression of genes involved in senescence were evaluated in different subsets of mice on D21 and D49.
Results:
In utero exposure to CS induced significant lung function impairment by D21. CS exposure between D21 and D49 induced significant functional impairment only in mice exposed to CS prenatally. On D49, no difference was observed between subgroups in terms of lung p53, p16, p21, and Bax mRNA levels.
Conclusions:
Our findings suggest that prenatal and adolescent CS exposure have a synergistic effect on lung function in mice. The combined effect did not appear to be a consequence of early pulmonary senescence.

Introduction:
Cette étude évalue l’efficacité d’un programme de formation à la communication de seize heures. Ce programme vise à entrainer des étudiants de Master en psychologie à mener un premier entretien ayant pour thème le tabac.
Méthode:
Les étudiants ont été assignés aléatoirement au groupe formé ou au groupe contrôle. Le groupe formé a reçu quatre séances de quatre heures d’exercices pratique et jeux de rôle basés sur la communication. Les évaluations consistent en un entretien simulé qui est enregistré par audio afin de mesurer les compétences communicationnelles des étudiants. Ces enregistrements audio ont été retranscris par un programme d’analyse de contenu : LaComm. La satisfaction des étudiants et de l’actrice jouant le patient a été évaluée à l’aide d’échelles visuelles analogiques (EVA) après chaque entretien simulé.
Résultats:
Cent soixante-trois étudiants ont été inclus. Premièrement, comparé au groupe contrôle, les étudiants formés ont acquis plus de compétences au niveau de l’évaluation (OR = 1.15; p = 0.006) et du soutien (OR = 1.38; p = <.001). Deuxièmement, l’actrice jouant le patient rapporte une plus grande satisfaction au second temps d’évaluation auprès des étudiants formés comparé aux étudiants du groupe contrôle (p = <.001).
Conclusion:

En effet, l'irradiation est d'autant plus bénéfique que l'oxygénation tissulaire est grande. Le monoxyde de Carbone produit par le tabac prend la place de l'oxygène dans le sang et donc dans les tissus sains et tumoraux. Compte tenu du nombre de patients oncologiques poursuivant leur assuétude durant la radiothérapie, nous nous posons diverses questions: connaissent-ils les conséquences qui en découlent? Comment les motiver à arrêter alors qu'ils mènent un combat difficile contre la maladie? Le moment de la prise en charge avant le début du traitement a t-il un impact sur la démarche u patient vers l’arrêt du tabac?
Objectifs: L’objectif est de déterminer si le délai qui existe entre le choix du traitement réalisé lors de la Concertation Oncologique Multidisciplinaire (C.O.M) et la 1ère séance de radiothérapie peut être mis à profit pour aider le patient à entreprendre une démarche vers l'arrêt du tabagisme.
Méthodologie: Il s’agit d’une étude randomisée, réalisée dans le service de radiothérapie du CHU de Liège. Elle comportait 2 groupes de patients. Un groupe comprenait les patients vus lors de la consultation post COM et l’autre lors de la simulation (séance de planification de la radiothérapie). Lors des C.O.M de pneumologie, sénologie et O.R.L, quand le choix du protocole de traitement était finalisé, un tirage au sort était réalisé et déterminait dans quel groupe le patient fumeur était inclus. Lors de ces rencontres, le patient remplissait plusieurs questionnaires et un entretien avait lieu entre l’étudiante et le patient.
Résultats: 30 patients ont participé à l’étude: 16 patients dans le groupe "post COM" et 14 dans le groupe "simulation". L’analyse a montré des groupes homogènes quant à leurs caractéristiques, leur statut tabagique, leur connaissance et les informations reçues quant aux effets du tabac sur leur santé. Dans le groupe post COM, la diminution du tabagisme entre le premier contact et le début du traitement est significative (p=0,002). Dans le groupe simulation, la différence de tabagisme entre le premier contact et le début du traitement est également significative (p=0,0469). La diminution est plus marquée dans le groupe post COM, c’est à dire celui rencontré à plus grande distance du début de la radiothérapie.
Conclusion: L’étude a mis en évidence, dans les deux groupes, une diminution du tabagisme lors du début de la radiothérapie lorsque les patients ont eu une entrevue avec l'étudiante. Cette diminution est plus importante en début de traitement, dans le groupe de patients ayant été vu à plus grande distance de celui-ci c’est à dire en consultation post COM que lors de la simulation. La prise en charge la plus précoce possible est à considérer afin d'accompagner au mieux les patients fumeurs dans leur participation active à l'efficacité de leur traitement.

Trials of antidepressant medications for smoking cessation have had mixed results. We conducted a double-blind, placebo-controlled trial of a sustained-release form of bupropion for smoking cessation. We excluded smokers with current depression, but not those with a history of major depression. The 615 subjects were randomly assigned to receive placebo or bupropion at a dose of 100, 150, or 300 mg per day for seven weeks. The target quitting date (or "target quit date") was one week after the beginning of treatment. Brief counseling was provided at base line, weekly during treatment, and at 8, 12, 26, and 52 weeks. Self-reported abstinence was confirmed by a carbon monoxide concentration in expired air of 10 ppm or less.
RESULTS:
At the end of seven weeks of treatment, the rates of smoking cessation as confirmed by carbon monoxide measurements were 19.0 percent in the placebo group, 28.8 percent in the 100-mg group, 38.6 percent in the 150-mg group, and 44.2 percent in the 300-mg group (P<0.001). At one year the respective rates were 12.4 percent, 19.6 percent, 22.9 percent, and 23.1 percent. The rates for the 150-mg group (P=0.02) and the 300-mg group (P=0.01) -- but not the 100-mg group (P=0.09) -- were significantly better than those for the placebo group. Among the subjects who were continuously abstinent through the end of treatment, the mean absolute weight gain was inversely associated with the dose (a gain of 2.9 kg in the placebo group, 2.3 kg in 100-mg and 150-mg groups, and 1.5 kg in the 300-mg group; P= 0.02). No effects of treatment were observed on depression scores as measured serially by the Beck Depression Inventory. Thirty-seven subjects stopped treatment prematurely because of adverse events; the frequency was similar among all groups.
CONCLUSIONS:
A sustained-release form of bupropion was effective for smoking cessation and was accompanied by reduced weight gain and minimal side effects. This review compared the effect of high-dose nicotine replacement therapy (NRT) and combinations of NRT for increasing smoking abstinence rates compared to standard-dose NRT patch, varenicline, and bupropion on smoking abstinence.
METHODS:
Ten electronic databases were searched (up to January 2012) for randomized controlled trials (RCT) of standard-dose (≤ 22 mg) or high-dose nicotine patch therapy (> 22 mg), combination NRT (e.g. nicotine patch + nicotine inhaler), bupropion, and varenicline. Analysis consisted of random-effects pairwise meta-analysis and a Bayesian multiple treatment comparison (MTC).
RESULTS:
We identified 146 RCTs (65 standard-doses of the nicotine patch (≤ 22 mg); 6 high-dose NRT patch (> 22 mg); 5 high versus standard-dose NRT patch; 5 combination NRT versus inert controls; 6 combination versus single NRT patch; 48 bupropion; and 11 varenicline). The MTC found that all therapies offered treatment benefits at most time points over controls. Combination NRT and higher-dose NRT did not demonstrate consistent effects over other interventions. With the exception of varenicline, the benefits of treatments over standard-dose NRT were not retained in the long term.
CONCLUSIONS:
All pharmacologic treatments were significantly more effective than inert controls. Varenicline was the only treatment demonstrating effects over other options. This study aimed at assessing the impact of smoking on life expectancy (LE) and LE with (LED) and without disability (DFLE). We further estimated the contribution of disability and mortality and their causes to differences in LED and DFLE by smoking.
Methods
Data on disability, chronic conditions, and smoking from 17 148 participants of the 1997, 2001, 2004 Belgian Health Interview Surveys were used to estimate causes of disability using the attribution method. A 10-year mortality follow-up of survey participants was used. The Sullivan method was applied to estimate LED and DFLE. The contribution of disability and mortality and of causes of disability and death to smoking differences in LED and DFLE was assessed using decomposition methods.
Results
Never smokers live longer than daily smokers. DFLE advantage at age 15 of +8.5/+4.3 years (y) in men/women never compared with daily smokers was the result of lower mortality (+6.2y/+3y) and lower disability (2.3y/1.3y). The extra 0.3y/1.6y LED in never smokers was due to lower mortality (+2.6y/+2.9y) and lower disability (-2.3y/-1.3y). Lower mortality from lung/larynx/trachea cancer, chronic respiratory, and ischaemic heart diseases was the main contributor to higher LED and DFLE in never smokers. Lower disability from musculoskeletal conditions in men and chronic respiratory diseases in women increased LED and DFLE in never smokers.
Conclusions

We examined youth e-cigarette prevalence by TAPS regulations in countries with different income levels.
Methods: We analysed data on 165 299 respondents from 48 countries with 2016/2018 WHO FCTC implementation reports and 2016-2019 Global Youth Tobacco Survey. We used multilevel logistic regressions to examine associations between TAPS regulations and current e-cigarette use, stratified by country income.
Results: About 1 in 10 respondents was currently using e-cigarettes. Respondents in countries with TAPS bans on the internet were less likely to use e-cigarettes (adjOR=0.58; 95% CI 0.39 to 0.86) than youth in countries without such bans. In lower middle-income and low-income countries, bans on displaying tobacco products at the point of sale (adjOR=0.55; 95% CI 0.34 to 0.90), bans on product placement (adjOR=0.44; 95% CI 0.28 to 0.69) and strength of additional TAPS measures were associated with lower prevalence of e-cigarette use among students. Being taught about the dangers of the use of tobacco in school was associated with lower odds of e-cigarette use. No differences in the use of e-cigarettes were observed by types of TAPS among respondents in high-income countries.
Conclusions: Strengthening implementation of TAPS policies and assuring they cover new and emerging products, online channels and points of sales are essential, especially in lower income countries. Maintaining tobacco health education is also important to protect youth from e-cigarette use.

Cigarette smoking—highly prevalent during pregnancy—is associated with addiction and withdrawal syndrome in adults. We conducted a prospective, two-group parallel study on 17 consecutive newborns of heavy-smoking mothers and 16 newborns of nonsmoking, unexposed mothers (controls). Neurologic examinations were repeated at days 1, 2, and 5. Finnegan withdrawal score was assessed every 3 h during their first 4 d. Newborns of smoking mothers had significant levels of cotinine in the cord blood (85.8 ± 3.4 ng/mL), whereas none of the controls had detectable levels. Similar findings were observed with urinary cotinine concentrations in the newborns (483.1 ± 2.5 μg/g creatinine versus 43.6 ± 1.5 μg/g creatinine; p = 0.0001). Neurologic scores were significantly lower in newborns of smokers than in control infants at days 1 (22.3 ± 2.3 versus 26.5 ± 1.1; p = 0.0001), 2 (22.4 ± 3.3 versus 26.3 ± 1.6; p = 0.0002), and 5 (24.3 ± 2.1 versus 26.5 ± 1.5; p = 0.002). Neurologic scores improved significantly from day 1 to 5 in newborns of smokers (p = 0.05), reaching values closer to control infants. Withdrawal scores were higher in newborns of smokers than in control infants at days 1 (4.5 ± 1.1 versus 3.2 ± 1.4; p = 0.05), 2 (4.7 ± 1.7 versus 3.1 ± 1.1; p = 0.002), and 4 (4.7 ± 2.1 versus 2.9 ± 1.4; p = 0.007). Significant correlations were observed between markers of nicotine exposure and neurologic-and withdrawal scores. Electronic cigarettes (e-cigarettes) are often promoted to assist with cigarette smoking cessation. In 2016–2017, the relationship between e-cigarette use and having stopped smoking among ever (current and former) smokers was assessed in the European Union and Great Britain by itself.
Methods:
Cross-sectional logistic regression of the association between being a former smoker and e-cigarette use was applied to the 2014 Eurobarometer survey of 28 European Union countries controlling for demographics.
Results:
Among all ever smokers, any regular ever use of nicotine e-cigarettes was associated with
lower odds of being a former smoker (unadjusted OR¼0.34, 95% CI¼0.26, 0.43, AOR¼0.43, 95% CI¼0.32, 0.58) compared with smokers who had never used e-cigarettes. In unadjusted models, daily use (OR¼0.42, 95% CI¼0.31, 0.56); occasional use (OR¼0.25, 95% CI¼0.18, 0.35); and experimentation (OR¼0.24, 95% CI¼0.19, 0.30) of nicotine e-cigarettes were associated with lower odds of being a former smoker compared with having never used nicotine-containing e-cigarettes. Comparable results were found in adjusted models. Results were similar in Great Britain alone. Among current smokers, daily cigarette consumption was 15.6 cigarettes/day (95% CI¼14.5, 16.7) among those who also used e-cigarettes versus 14.4 cigarettes/day (95% CI¼13.4, 15.4) for those who did not use them (po0.05).
Conclusions:
These results that e-cigarettes are associated with inhibiting rather than assisting in smoking cessation. On the population level, the net effect of the entry of e-cigarettes into the European Union (and Great Britain) is associated with depressed smoking cessation of conventional cigarettes.

The present authors hypothesised that treating smokers with asthma with low-dose theophylline added to inhaled corticosteroids would enhance steroid sensitivity and thereby improve lung function and symptoms. In a double-blind, parallel group exploratory trial, 68 asthmatic smokers were randomised to one of three treatments for 4 weeks: inhaled beclometasone (200 microg day(-1)), theophylline (400 mg day(-1)) or both treatments combined. Outcome measures included change in lung function and Asthma Control Questionnaire (ACQ) scores. At 4 weeks, theophylline added to inhaled beclometasone produced an improvement in peak expiratory flow (39.9 L min(-1), 95% confidence intervals (CI) 10.9-68.8) and ACQ score (-0.47, 95% CI -0.91- -0.04) and a borderline improvement in pre-bronchodilator forced expiratory volume in one second (mean difference 165 mL, 95% CI -13-342) relative to inhaled corticosteroid alone. Theophylline alone improved the ACQ score (-0.55, 95% CI -0.99- -0.11), but not lung function. In the present pilot study, the combination of low-dose theophylline and inhaled beclometasone produced improvements in both lung function and symptoms in a group of smokers with asthma. Pictorial warnings on cigarette packs draw attention and increase quit intentions, but their effect on smoking behavior remains uncertain.
OBJECTIVE
To assess the effect of adding pictorial warnings to the front and back of cigarette packs.
DESIGN, SETTING, AND PARTICIPANTS
This 4-week between-participant randomized clinical trial was carried out in California and North Carolina.We recruited a convenience sample of adult cigarette smokers from the general population beginning September 2014 through August 2015. Of 2149 smokers who enrolled, 88%completed the trial. No participants withdrew owing to adverse events.
INTERVENTIONS
We randomly assigned participants to receive on their cigarette packs for 4 weeks either text-only warnings (one of the Surgeon General’s warnings currently in use in the United States on the side of the cigarette packs) or pictorial warnings (one of the Family Smoking Prevention and Tobacco Control Act’s required text warnings and pictures that showed harms of smoking on the top half of the front and back of the cigarette packs).
MAIN OUTCOMES AND MEASURES
The primary trial outcomewas attempting to quit smoking during the study.We hypothesized that smokers randomized to receive pictorial warnings would be more likely to report a quit attempt during the study than smokers randomized to
receive a text-only Surgeon General’s warning.
RESULTS
Of the 2149 participants who began the trial (1039 men, 1060 women, and 34 transgender people; mean [SD] age, 39.7 [13.4] years for text-only warning, 39.8 [13.7] for pictorial warnings), 1901 completed it. In intent-to-treat analyses (n = 2149), smokers whose packs had pictorial warnings were more likely than those whose packs had text-only warnings to attempt to quit smoking during the 4-week trial (40% vs 34%; odds ratio [OR], 1.29; 95% CI, 1.09-1.54). The findings did not differ across any demographic groups. Having quit smoking for at least the 7 days prior to the end of the trial was more common among smokers who received pictorial than those who received text-only warnings (5.7%vs 3.8%; OR, 1.53; 95%CI, 1.02-2.29). Pictorial warnings also increased forgoing a cigarette, intentions to quit
smoking, negative emotional reactions, thinking about the harms of smoking, and conversations about quitting.
CONCLUSIONS AND RELEVANCE
Pictorialwarnings effectively increased intentions to quit, forgoing cigarettes, quit attempts, and successfully quitting smoking over 4 weeks. Our trial
findings suggest that implementing pictorial warnings on cigarette packs in the United States would discourage smoking.

Few studies have been conducted on the effects of smoking bans on cardiac health. We evaluated changes in the frequency of acute coronary events in Rome, Italy, after the introduction of legislation that banned smoking in all indoor public places in January 2005.
Methods and Results— We analyzed acute coronary events (out-of-hospital deaths and hospital admissions) between 2000 and 2005 in city residents 35 to 84 years of age. We computed annual standardized rates and estimated rate ratios by comparing the data from prelegislation (2000–2004) and postlegislation (2005) periods. We took into account several time-related potential confounders, including particulate matter (PM10) air pollution, temperature, influenza epidemics, time trends, and total hospitalization rates. The reduction in acute coronary events was statistically significant in 35- to 64-year-olds (11.2%, 95% CI 6.9% to 15.3%) and in 65- to 74-year-olds (7.9%, 95% CI 3.4% to 12.2%) after the smoking ban. No evidence was found of an effect among the very elderly. The reduction tended to be greater in men and among lower socioeconomic groups.
Conclusions— We found a statistically significant reduction in acute coronary events in the adult population after the smoking ban. The size of the effect was consistent with the pollution reduction observed in indoor public places and with the known health effects of passive smoking. Design A cohort observational study.
Setting Renfrew and Paisley, two towns in west central Scotland.
Participants 8353 women and 7049 men aged 45-64 years recruited in 1972-6 (almost 80% of the population in this age group). The cohort was divided into 24 groups by sex (male, female), smoking status (current, former, or never smokers), and social class (classes I + II, III non-manual, III manual, and IV + V) or deprivation category of place of residence.
Main outcome measure Relative mortality (adjusted for age and other risk factors) in the different groups; Kaplan-Meier survival curves and survival rates at 28 years.
Results Of those with complete data, 4387/7988 women and 4891/6967 men died over the 28 years. Compared with women in social classes I + II who had never smoked (the group with lowest mortality), the adjusted relative mortality of smoking groups ranged from 1.7 (95% confidence interval 1.3 to 2.3) to 4.2 (3.3 to 5.5). Former smokers’ mortalities were closer to those of never smokers than those of smokers. By social class (highest first), age adjusted survival rates after 28 years were 65%, 57%, 53%, and 56% for female never smokers; 41%, 42%, 33%, and 35% for female current smokers; 53%, 47%, 38%, and 36% for male never smokers; and 24%, 24%, 19%, and 18% for male current smokers. Analysis by deprivation category gave similar results.
Conclusions Among both women and men, never smokers had much better survival rates than smokers in all social positions. Smoking itself was a greater source of health inequality than social position and nullified women’s survival advantage over men. Exposure to smoking in movies has been linked with adolescent smoking initiation in cross-sectional studies. We undertook a prospective study to ascertain whether exposure to smoking in movies predicts smoking initiation.
Method
We assessed exposure to smoking shown in movies in 3547 adolescents, aged 10–14 years, who reported in a baseline survey that they had never tried smoking. Exposure to smoking in movies was estimated for individual respondents on the basis of the number of smoking occurrences viewed in unique samples of 50 movies, which were randomly selected from a larger sample pool of popular contemporary movies. We successfully re-contacted 2603 (73%) students 13–26 months later for a follow-up interview to determine whether they had initiated smoking.
Findings
Overall, 10% (n=259) of students initiated smoking during the follow-up period. In the highest quartile of exposure to movie smoking, 17% (107) of students had initiated smoking, compared with only 3% (22) in the lowest quartile. After controlling for baseline characteristics, adolescents in the highest quartile of exposure to movie smoking were 2·71 (95% CI 1·73–4·25) times more likely to initiate smoking compared with those in the lowest quartile. The effect of exposure to movie smoking was stronger in adolescents with non-smoking parents than in those whose parent smoked. In this cohort, 52·2% (30·0–67·3) of smoking initiation can be attributed to exposure to smoking in movies.
Interpretation

Systematic assessments of Motivational Interviewing (MI) in smoking behavior have been rare to date. This study aimed to determine whether an integrated approach, involving staff training in MI techniques, was sufficient to affect change in smoking status or intensity in low-income pregnant and postpartum women.
Methods:
Overall, 500 consecutive smokers were recruited at first prenatal visit to public antenatal clinics. Following staff training,500 more were recruited (intervention group). Data were recorded at 28–32 weeks gestation, after birth, at 3–4 and 7–9 months postpartum.The primary outcome measure was self-reported continued abstinence from smoking verified by urinary cotinine analysis.
Changes in smoking intensity were also measured.
Results:
There was no significant difference in the proportion of smokers in the intervention and control groups who reported stopping smoking at 28–32 weeks gestation (8.2% vs. 8.8%; p = .73), 1 week after birth (8.6% vs. 11.4%; p = .14), 3–4 months after birth (5.8% vs. 4.8%; p = .48), or 7–9 months after birth (5.2% vs. 4.0%; p = .36). Although more cases were nonsmoking at the second visit, 14.8% [95% CI = 11.8–18.5] vs. 13.1% controls [95% CI = 10.3–16.6], this was not statistically significant.
Conclusions:
MI delivered at a number of time points during pregnancy and up to 9 months postpartum failed to affect quit rates. Smoking remains the leading preventable cause of premature deaths. Several pharmacological interventions now exist to aid smokers in cessation. These include Nicotine Replacement Therapy [NRT], bupropion, and varenicline. We aimed to assess their relative efficacy in smoking cessation by conducting a systematic review and meta-analysis.
METHODS:
We searched 10 electronic medical databases (inception to Sept. 2006) and bibliographies of published reviews. We selected randomized controlled trials [RCTs] evaluating interventions for smoking cessation at 1 year, through chemical confirmation. Our primary endpoint was smoking cessation at 1 year. Secondary endpoints included short-term smoking cessation (approximately 3 months) and adverse events. We conducted random-effects meta-analysis and meta-regression. We compared treatment effects across interventions using head-to-head trials and when these did not exist, we calculated indirect comparisons.
RESULTS:
We identified 70 trials of NRT versus control at 1 year, Odds Ratio [OR] 1.71, 95% Confidence Interval [CI], 1.55-1.88, P =< 0.0001). This was consistent when examining all placebo-controlled trials (49 RCTs, OR 1.78, 95% CI, 1.60-1.99), NRT gum (OR 1.60, 95% CI, 1.37-1.86) or patch (OR 1.63, 95% CI, 1.41-1.89). NRT also reduced smoking at 3 months (OR 1.98, 95% CI, 1.77-2.21). Bupropion trials were superior to controls at 1 year (12 RCTs, OR1.56, 95% CI, 1.10-2.21, P = 0.01) and at 3 months (OR 2.13, 95% CI, 1.72-2.64). Two RCTs evaluated the superiority of bupropion versus NRT at 1 year (OR 1.14, 95% CI, 0.20-6.42). Varenicline was superior to placebo at 1 year (4 RCTs, OR 2.96, 95% CI, 2.12-4.12, P =< 0.0001) and also at approximately 3 months (OR 3.75, 95% CI, 2.65-5.30). Three RCTs evaluated the effectiveness of varenicline versus bupropion at 1 year (OR 1.58, 95% CI, 1.22-2.05) and at approximately 3 months (OR 1.61, 95% CI, 1.16-2.21). Using indirect comparisons, varenicline was superior to NRT when compared to placebo controls (OR 1.66, 95% CI 1.17-2.36, P = 0.004) or to all controls at 1 year (OR 1.73, 95% CI 1.22-2.45, P = 0.001). This was also the case for 3-month data. Adverse events were not systematically different across studies.
CONCLUSION:
NRT, bupropion and varenicline all provide therapeutic effects in assisting with smoking cessation. We compared the associations between use of these substances and lung function in a population-based cohort (n = 1,037). Cannabis and tobacco use were reported at ages 18, 21, 26 and 32 yrs. Spirometry, plethysmography and carbon monoxide transfer factor were measured at 32 yrs. Associations between lung function and exposure to each substance were adjusted for exposure to the other substance. Cumulative cannabis use was associated with higher forced vital capacity, total lung capacity, functional residual capacity and residual volume. Cannabis was also associated with higher airway resistance but not with forced expiratory volume in 1 s, forced expiratory ratio or transfer factor. These findings were similar among those who did not smoke tobacco. In contrast, tobacco use was associated with lower forced expiratory volume in 1 s, lower forced expiratory ratio, lower transfer factor and higher static lung volumes, but not with airway resistance. Cannabis appears to have different effects on lung function from those of tobacco. Cannabis use was associated with higher lung volumes, suggesting hyperinflation and increased large-airways resistance, but there was little evidence for airflow obstruction or impairment of gas transfer. The purpose of this study was to use data from controlled trials to examine whether smoking cessation increases breastfeeding duration. Correlational studies have confirmedassociations between smoking status and breastfeeding duration, but whether smoking cessation increases breastfeeding duration has not been established.
Methods:
Participants ( N = 158) were smokers at the start of prenatal care who participated in controlled trials on smoking cessation. Women were assigned to either an incentive-based intervention wherein they earned vouchers exchangeable for retail items by abstaining from smoking or a control condition where they received comparable vouchers independent of smoking status. Treatments were provided antepartum through 12-week postpartum. Maternal reports of breastfeeding collected at 2-,
4-, 8-, 12-, and 24-week postpartum were compared between treatment conditions. Whether women were exclusively breastfeeding was not investigated.
Results:
The incentive-based treatment signifi cantly increased breastfeeding duration compared with rates observed among women receiving the control treatment, with signifi cant differences between treatment conditions observed at 8-week (41% vs. 26%; odds ratio [ OR ] = 2.7, 95% CI = 1.3– 5.6, p = .01) and 12-week
(35% vs. 17%; OR = 3.4, 95% CI = 1.5– 7.6, p = .002) postpartum. No significant treatment effects on breastfeeding were observed at other assessments. Changes in smoking status mediated the effects of treatment condition on breastfeeding duration.
Conclusions:

Objective: To evaluate smoking abstinence and changes in mood and anxiety levels in smokers with depression treated with varenicline versus placebo.
Design: Phase 4, multicenter, parallel, 1:1 allocation, double-blind, randomization trial. Randomization, stratified by antidepressant use and depression score at baseline, was blocked in sizes of 4. (ClinicalTrials.gov: NCT01078298)
Setting: 38 centers in 8 countries.
Participants: 525 adult smokers with stably treated current or past major depression and no recent cardiovascular events.
Intervention: Varenicline, 1 mg twice daily, or placebo for 12 weeks, with 40-week nontreatment follow-up.
Measurements: Primary outcome was carbon monoxide–confirmed continuous abstinence rate (CAR) for weeks 9 to 12. Other outcomes included CARs assessed during nontreatment follow-up and ratings of mood, anxiety, and suicidal ideation or behavior.
Results: 68.4% versus 66.5% of the varenicline and placebo groups, respectively, completed the study. Varenicline-treated participants had higher CARs versus placebo at weeks 9 to 12 (35.9% vs. 15.6%; odds ratio [OR], 3.35 [95% CI, 2.16 to 5.21]; P < 0.001), 9 to 24 (25.0% vs. 12.3%; OR, 2.53 [CI, 1.56 to 4.10]; P < 0.001), and 9 to 52 (20.3% vs. 10.4%; OR, 2.36 [CI, 1.40 to 3.98]; P = 0.001). There were no clinically relevant differences between groups in suicidal ideation or behavior and no overall worsening of depression or anxiety in either group. The most frequent adverse event was nausea (varenicline, 27.0%; placebo, 10.4%). Two varenicline-group participants died during the nontreatment phase.
Limitations: Some data were missing, and power to detect differences between groups was low in rare events. Smokers with untreated depression, with co-occurring psychiatric conditions, or receiving mood stabilizers and antipsychotics were not included.

Smoking is the most important risk factor for COPD and accelerates its progression. Despite the health implications, a large proportion of patients with COPD continue to smoke, so finding effective smoking cessation interventions for this population is paramount. To our knowledge, this is the first randomized clinical trial to compare the efficacy and safety of varenicline tartrate vs placebo in smokers with mild to moderate COPD.
METHODS:
In a 27-center, double-blind, multinational study, 504 patients with mild to moderate COPD (postbronchodilator FEV1/FVC, <70%; FEV1 percent predicted normal value, ≥50%) and without known psychiatric disturbances were randomized to receive varenicline (n=250) or placebo (n=254) for 12 weeks, with a 40-week nontreatment follow-up. The primary end point was carbon monoxide-confirmed continuous abstinence rate (CAR) for weeks 9 to 12. A secondary end point was CAR for weeks 9 to 52.
RESULTS:
CAR for weeks 9 to 12 was significantly higher for patients in the varenicline group (42.3%) than for those in the placebo group (8.8%) (OR, 8.40; 95% CI, 4.99-14.14; P<.0001). CAR in the patients treated with varenicline remained significantly higher than in those treated with placebo through weeks 9 to 52 (18.6% vs 5.6%) (OR, 4.04; 95% CI, 2.13-7.67; P<.0001). Nausea, abnormal dreams, upper-respiratory tract infection, and insomnia were the most commonly reported adverse events (AEs) for patients in the varenicline group. Serious AEs were infrequent in both treatment groups. Two patients in the varenicline group and one patient in the placebo group died during the study. Reports of psychiatric AEs were similar for both treatment groups.
CONCLUSIONS:

BACKGROUND:
The selective nicotinic acetylcholine receptor partial agonist, varenicline tartrate, represents a novel type of therapy for smoking cessation. This study evaluated the efficacy, safety, and tolerability of 4 varenicline dose regimens, 2 with progressive dosing over the first week (eg, titrated) and 2 with a fixed dosing schedule (eg, non-titrated), for promoting smoking cessation.
METHODS:
This multicenter, double-blind, placebo-controlled study randomized healthy smokers (aged 18-65 years) to varenicline tartrate, 0.5 mg twice daily nontitrated (n = 129), 0.5 mg twice daily titrated (n = 130), 1.0 mg twice daily nontitrated (n = 129), 1.0 mg twice daily titrated (n = 130), or placebo (n = 129) for 12 weeks to aid in smoking cessation. A 40-week follow-up period assessed long-term efficacy. The primary efficacy measures were the carbon monoxide-confirmed 4-week continuous quit rates by pooled dosage group for weeks 4 through 7 and 9 through 12 and the continuous abstinence rates for weeks 9 through 52.
RESULTS:
Weeks 9 through 12 continuous quit rates were greater in the 1.0-mg group (49.4%) and the 0.5-mg group (44.0%) vs placebo (11.6%; P<.001 vs both doses). Weeks 9 through 52 abstinence rates were greater in the 1.0-mg group (22.4%; P<.001) and the 0.5-mg group (18.5%; P<.001) vs placebo (3.9%). Varenicline was generally well tolerated, with nausea occurring in 16% to 42% of varenicline-treated subjects. Étant donné que le maintien du tabagisme après le diagnostic d'un cancer augmente le risque d'effets néfastes sur la santé, il est vivement conseillé aux patients atteints d'un cancer de cesser de fumer. Malgré le manque actuel de preuves quant à leur innocuité et à leur efficacité en tant qu'outil de cessation, les cigarettes électroniques (cigarettes électroniques) deviennent de plus en plus populaires. Pour orienter la communication des oncologues avec leurs patients sur l'utilisation de la cigarette électronique, cet article fournit aux auteurs les connaissances les plus récentes qui soient sur les premières données cliniques publiées concernant l'utilisation de la cigarette électronique et les résultats de l'arrêt du tabac chez les patients atteints de cancer.
MÉTHODES:
Parmi les 1074 participants, figuraient des fumeurs (patients atteints de cancer) récemment inscrits à un programme de traitement du tabac dans un centre de cancérologie intégré. Les résultats standard démographiques, les antécédents de consommation de tabac et l'abandon du tabac ont été évalués.
RÉSULTATS:
Une augmentation de 3 fois la consommation de cigarettes électroniques a été observée de 2012 à 2013 (10,6% contre 38,5%). Les utilisateurs de cigarettes électroniques étaient davantage dépendants de la nicotine que les non-utilisateurs, avaient eu plus de tentatives d'arrêter de fumer et étaient plus susceptibles de recevoir un diagnostic de cancer thoracique et de cancer de la tête ou du cou. En utilisant une analyse de cas complète, les utilisateurs de cigarettes électroniques étaient aussi susceptibles de fumer au moment du suivi que les non-utilisateurs (odds ratio, 1,0; intervalle de confiance à 95%, 0,5 à 1,7). En utilisant une analyse en intention de traiter, les utilisateurs de cigarettes électroniques étaient deux fois plus susceptibles de fumer au moment du suivi que les non-utilisateurs (odds ratio, 2,0; intervalle de confiance à 95%, 1,2-3,3).
CONCLUSIONS:
Le taux élevé d'utilisation de la cigarette électronique observé est cohérent avec les articles récents mettant en évidence une augmentation de l'utilisation de la cigarette électronique dans la population en général. Les résultats longitudinaux actuels soulèvent des doutes quant à l'utilité des cigarettes électroniques pour faciliter l'abandon du tabac chez les patients atteints de cancer. Of these, 5% were former users and 3% used smokeless tobacco almost daily in the past year. Rates of its use differed significantly by sex, age group, race, region, and metropolitan area size. Although females were far less likely to try it, those who did were as likely as males to be daily users. Smokeless tobacco users were also more likely to use alcohol, cigarettes, and marijuana. In general, those who used smokeless tobacco almost daily were more likely to report poor health and hospitalization for illness or injury in the past year, even when other substance use was controlled. Smokeless tobacco users also were more likely to report symptoms of depression. Finally, some substituted smokeless tobacco for cigarettes, but youths (12-17 yr old) were more likely than older tobacco users to use both forms of tobacco regularly. The number of quit attempts it takes a smoker to quit successfully is a commonly reported figure among smoking cessation programmes, but previous estimates have been based on lifetime recall in cross-sectional samples of successful quitters only. The purpose of this study is to improve the estimate of number of quit attempts prior to quitting successfully.
Design:
We used data from 1277 participants who had made an attempt to quit smoking in the Ontario Tobacco Survey, a longitudinal survey of smokers followed every 6 months for up to 3 years beginning in 2005. We calculated the number of quit attempts prior to quitting successfully under four different sets of assumptions. Our expected best set of assumptions incorporated a life table approach accounting for the declining success rates for subsequent observed quit attempts in the cohort.
Results:
The estimated average number of quit attempts expected before quitting successfully ranged from 6.1 under the assumptions consistent with prior research, 19.6 using a constant rate approach, 29.6 using the method with the expected lowest bias, to 142 using an approach including previous recall history.
Conclusions: Previous estimates of number of quit attempts required to quit may be underestimating the average number of attempts as these estimates excluded smokers who have greater difficulty quitting and relied on lifetime recall of number of attempts. Ce sentiment est un facteur d’influence important dans la motivation du sujet à mettre en place une action et à s’engager dans celle-ci.
Plusieurs études ont d’ailleurs démontré le rôle d’une faible estime de soi sur l’augmentation de l’utilisation de substances psychoactives tel que le tabac et ont montré également un lien entre la faible estime de soi et le maintien d’une consommation tabagique chez les femmes.
Peu à peu, les études tentent à lier motivation à l’arrêt tabagique et estime de soi.
Connaitre son rôle et son influence sur la motivation permettraient pour de nombreux professionnels de la santé d’améliorer la qualité de prise de charge de nos patients demandeurs d’arrêter. Dès lors et ce, dans le cadre de la lutte contre le tabac, il est nécessaire en tant que professionnel de la santé de pouvoir prendre en compte l’importance qu’à l’estime de soi du patient sur sa volonté et motivation à arrêter de fumer.
Pour ce faire, nous avons étudié deux populations, l’une fumeuse, l’autre vapoteuse. Dans le cadre de cette étude et suite à la littérature, nous considérons les personnes vapoteuses comme ayant mis en place un comportement en vue d’arrêter ou diminuer le tabac. Nous émettons comme hypothèse que les utilisateurs de cigarettes électroniques ont une meilleure estime que les fumeurs car le fait de passer à l’utilisation de la cigarette électronique est un comportement socialement valorisable. De plus, pour un patient souhaitant arrêter le tabac, passer à la cigarette électronique est un comportement qui vient toucher aux standards personnels de la personne et par conséquent, vient augmenter son sentiment de compétences et d’accomplissement à l’égard de ses standards propres à l’estime de soi. Une meilleure estime de soi permet à ce dernier de mettre en place plus facilement des stratégies de résolutions de problèmes plus adaptées, d’être plus résilient ainsi qu’à ressentir un plus grand bien-être et de s’engager dans des comportements qui protègent ou améliorent leur santé. Par conséquent, avoir une plus grande motivation pour maintenir ce nouveau comportement qu’est l’arrêt tabagique.
C’est à travers cette étude que nous voulions étudier le lien entre motivation à l’arrêt tabagique et estime de soi tant chez les vapoteurs que chez les personnes fumeuses. Nous attelant à cet objectif et pour garder une certaine modestie, nous spécifions que nous recherchions « des traits du psychisme» qui seraient spécifiques à ce groupe, et non « un type de personnalité ». Pour ce faire, nous nous sommes premièrement appuyée sur la littérature concernant notre sujet de recherche. Dans un premier chapitre, nous avons alors abordé le statut tabagique afin de comprendre comment former nos groupes de sujets constituant ainsi notre population. Par la suite, nous avons créé un historique et nous nous sommes intéressée aux statistiques touchant au tabac afin d’avoir une idée de l’évolution de la consommation et de son ampleur. Nous avons également traité du sujet de l’aide médicamenteuse afin de se rendre compte de l’importance de celle-ci dans une démarche d’arrêt. Dans notre second chapitre, nous avons appréhendé le tabagisme d’un point de vue psychanalytique afin de saisir ce que peut apporter la cigarette à un niveau intra-psychique. Nous serons ainsi parvenue à établir un lien entre nos deux chapitres théoriques. Nous avons alors appris que la cigarette favoriserait l’obtention d’un plaisir qui peut se décliner tant sur une problématique orale, anale, phallique que génitale, une réassurance narcissique, et un apaisement de l’angoisse d’abandon. Le polymorphisme symbolique de la cigarette expliquant que chacun, quelles que soit ses fixations, peut trouver moyen en fumant d’y retourner et les répéter via la cigarette, nous nous sommes alors concentrée sur les deux aspects suivants. Nous avons alors élaboré deux hypothèses, reprenant chacune une spécificité du fonctionnement psychique (le narcissisme et l’angoisse d’abandon) mais avons tenu à en mentionner d’autres et les observer (l’expression pulsionnelle, les imagos parentales, l’identité de base et sexuée) au cas où le cas échéant il puisse y avoir une significativité quelconque. Afin de les tester, nous avons constitué trois groupes de sujets à qui nous avons fait passer un entretien semi-directif axé sur la sphère médicale, muni d’un test de Fagerström, un Rorschach et un TAT. Le groupe 1 représentait les adultes « fumeurs », le groupe 2 « les non-fumeurs » et enfin le groupe 3 « les ex-fumeurs ». Nous avons alors débuté par faire une analyse cas par cas, puis fait une analyse inter-groupe et enfin intra-groupe. Il apparait finalement que, concernant l’angoisse de perte, il s’est avéré que les sujets du groupe 1 (fumeurs) aient des défenses moins efficaces que le deuxième (non-fumeurs) et le troisième (ex-fumeurs). De plus, les sujets du groupe 1 ont montré une plus grande fragilité narcissique que les sujets du groupe 2 et 3. Il prévoit ainsi éliminer certaines exceptions quant à l’usage du tabac dans les lieux fréquentés par le public, réduire le nombre de points de vente et mieux encadrer les détaillants autorisés, ainsi que restreindre ou interdire certaines pratiques promotionnelles. Le projet de loi touchera de près ou de loin tous les établissements commerciaux où il est présentement permis de fumer ou ceux où sont vendus des produits du tabac. Les mesures proposées pourraient affecter positivement ou négativement les revenus des établissements touchés en affectant leurs ventes et la clientèle. Cette étude calcule l’impact économique des mesures proposées en déterminant le nombre d’entreprises touchées et en mesurant, pour différentes catégories d’entreprises, l’impact sur les profits en étudiant comment les mesures affecteront les revenus et les coûts des établissements visés. L’impact de ces mesures est ensuite comparé à certains bénéfices attendus de la réduction anticipée du taux de tabagisme afin de voir si les bénéfices attendus se comparent aux coûts calculés. Le calcul d’impact est basé sur les mesures proposées en date du 21 mars 2005 et pourrait donc ne pas refléter en tout point celles du projet de loi éventuellement déposé. Tout médecin quelle que soit sa spécialité est confronté à la problématique du tabagisme et devrait normalement jouer un rôle majeur dans la prévention et les conseils en matière de sevrage tabagique. Est-il suffisamment formé à cette problématique ? Objectif : évaluer le niveau de formation des étudiants à la prise en charge du patient tabagique. Matériel et méthode : l'étude a été menée en 2012 auprès des étudiants de la faculté de médecine de l'Université Catholique de Louvain à partir d'un questionnaire explorant leurs comportements, leurs attitudes et leurs connaissances. Résultats : l'étude a intéressé 44.2% (N= 92 étudiants) de l'ensemble des étudiants inscrits en 7ième année médecine. La prévalence du tabagisme est faible (10%) par rapport à la prévalence générale du tabagisme en Belgique, avec une prévalence importante de fumeurs occasionnels (plus de la moitié des fumeurs). Seulement 5% de nos étudiants estimaient avoir eu suffisamment l'occasion d'apprendre les techniques de sevrage tabagique. Ils ne connaissent pas les outils pour mesurer le niveau de dépendance des patients en consultation ni les marqueurs biologiques permettant l'exposition au produit du tabac. Si une majorité de nos étudiants connaissent l'existence des substituts nicotiniques seulement 9% savent quand et comment prescrire ces produits. Conclusions : nos étudiants ne sont pas suffisamment formés à la prise en charge du patient tabagique. Les résultats de ce travail doivent inciter les responsables facultaires à concevoir et à intégrer une formation appropriée de base afin de mieux préparer l'étudiant à la prise en charge et la prévention du tabagisme. It has been reported that adult smokers (AS) may be considering smokeless tobacco products as an alternative to smoking. The objective of this study was to evaluate the change in exposure in AS using Marlboro snus (MSNUS) (a tobacco pouch product in test market in June 2007).
METHODS:
AS were randomized into the following groups--CS: subjects (n = 30) continue smoking their own brand; DU: subjects (n = 60) reduced their daily cigarette consumption by >or=50% and were allowed to use MSNUS; SN: subjects (n = 15) stopped smoking their cigarettes but were allowed to use MSNUS; NT: subjects (n = 15) were not allowed to use any tobacco products for the entire duration of the 8-day study. Biomarkers of smoke exposure (BOE) measured at baseline and postbaseline were 24-hr urinary excretion of metabolites of N-nitrosamines, nicotine (urine and plasma), aromatic amines, benzene, and polycyclic aromatic hydrocarbon; urine mutagenicity; and carboxyhemoglobin at various timepoints.
RESULTS:
Statistically significant (p < .05) reductions in all the urinary BOE were observed in the DU group compared with the CS group. After correcting for the residual effect, a proportionate reduction (approximately 50%) in most of the biomarkers was observed. Even larger reductions, similar to the NT group, were observed in the SN group.
DISCUSSION:
The proportionate reduction in exposure when reducing the number of cigarettes by 50% and using MSNUS, under the consumption patterns observed, suggest that the AS did not appear to alter their smoking behavior. The added exposure from MSNUS usage in this group was minimal. Methods: A total of 160 participants were divided into four groups according to the type of product used, including non-smokers, users of conventional combustible tobacco (cigarettes), heated tobacco, and electronic, tobacco-free vapor products (e-cigarettes). Buccal mucosa samples were analyzed using the micronucleus cytome assay
to assess cytotoxic and genotoxic damage.
Results: E-cigarette users showed significantly higher values for all tested parameters in the micronucleus test compared to non-smokers (p < 0.05). Similarly, users of tobacco heating products showed an increase in all parameters (p < 0.05), with the exception of the number of cells with micronuclei. Conventional cigarette smokers showed a notable increase in the number of binucleated cells and cells with karyorrhexis and karyolysis (p ≤ 0.05). When assessing the differences between users of traditional combustible tobacco products and non-combustible alternatives, these did not appear to be significant, except for e-cigarette users, who had significantly more cells with condensed chromatin (p ≤ 0.001), while users of tobacco heating products had more pyknotic cells (p ≤ 0.001).
Conclusion: The results of this study underscore the heightened occurrence of cytotoxic and genotoxic damage in users of both conventional combustible tobacco products and non-combustible alternatives compared to non-smokers, emphasizing the detrimental impact of these products on the oral mucosa.

This study explored whether HTP retailer proximity and density to secondary schools were associated with youth use of HTPs in four Canadian provinces.
Methods: An online search between November 2020 and March 2021 identified retailers selling IQOS devices and HEETS (tobacco sticks used in IQOS) within 500 m, 1000 m and 1500 m radius circular buffer zones around high schools (N = 120) participating in the COMPASS study in 2020–2021. Retailer proximity/density data were linked to crosssectional student-level data (N = 40 636 students), and multilevel regression models examined the association between HTP retailer proximity and density and current HTP use, controlling for relevant covariates.
Results: While only 10.0% of schools had at least one retailer selling IQOS devices within 1000 m of the school, 65.0% of schools had at least one retailer selling HEETS. The school a student attended accounted for 23.7% of the variability in the likelihood of currently using an HTP. However, HTP retailer proximity to and density around schools were not significantly associated with the likelihood of students currently using HTPs.
Conclusion: While the school a student attended accounted for a significant amount of variability in HTP use, these findings suggest that students may be obtaining HTPs through other, non-retail sources. Methods: A cross-sectional national survey of a random sample of 6,021 Israeli residents over the age of 21 were interviewed by telephone. The sample consisted of 4248 Jews, 858 Arabs and 915 immigrants, 856 of them were from the former Soviet Union.
Results: Compared to the Jewish population the Jews who immigrated from the former Soviet Union began smoking at an earlier age and the Arabs started smoking at an older age. The young respondents in all ethnic groups reported starting to smoke at a younger age compared to the older respondents, and there was a decrease in smoking initiation at an older age. Father smoking during childhood predicted earlier age of smoking initiation but not mother or sibling smoking, significantly among Jews and immigrants and non-significantly among Arabs.
Conclusions: Age of smoking initiation is dependent on the ethnic background in which the smokers grow up, however, the influence of the father smoking seems to be similar in all population groups. Cette étude vise à identifier le niveau de consensus et à comparer les opinions de deux groupes d’experts (des chercheurs et des coachs qui dispensent des cours de sevrage tabagique) sur des facteurs déjà étudiés en rapport avec la rechute du tabagisme, des facteurs qui n’ont pas encore été abordés et des idées sur la améliorer les programmes de prévention. Une méthode Delphi à trois cycles a été utilisée. Lors du premier tour, 15 chercheurs ont rempli un questionnaire électronique sur les facteurs associés à la rechute. Les résultats ont été utilisés pour élaborer un questionnaire structuré pour le second tour, qui a été complété par 47 chercheurs et 61 entraîneurs. Les résultats du second tour ont ensuite été présentés aux mêmes experts lors du troisième tour, leur permettant de réévaluer leurs réponses. Les résultats ont révélé un consensus élevé sur certains facteurs déjà identifiés comme prédictifs de la rechute (par exemple, l'efficacité personnelle), de nouveaux facteurs (par exemple, la planification de l'action) et plusieurs méthodes pour améliorer les programmes de prévention. De manière générale, les chercheurs et les entraîneurs ont formulé des suggestions similaires. Les résultats brossent un tableau de l'état actuel des connaissances sur les facteurs liés aux rechutes et ouvrent la voie à des domaines de recherche plus poussés. The purpose of this study was to investigate the effects of 14 days of e-cigarette vapor exposure in adolescent rats on lung function and inflammatory cytokine expression. Seventeen male Long-Evans rats were assigned to vape or air groups. The animals were exposed to either air or 5% nicotine vapor using a whole-body exposure chamber, once a day for ten minutes for fourteen consecutive days. Ventilation recordings were completed on day 0 (before exposure) and day 15 (after exposure) using unrestrained whole-body plethysmography. Both lung tissue and blood was collected for mo-lecular assays. Baseline and post-exposure ventilation data were compared between air and vape groups across three different parameters: frequency, tidal volume, and minute ventilation. These parameters were compared resulting in three distinct 2x2 (time x treatment) Mixed Model ANOVAs. Between baseline and post-treatment measurements there was a significant decrease in values in minute ventilation (p=0.0312) and tidal volume (p=0.0031). Between treatment groups there was a significant difference in minute ventilation (p=0.0128) and frequency (p=0042). There were also significant alterations to the presence of IL-1β in the serum (p=0.0003), but not lung tissue (p=0.6525). In conclusion, following two weeks of e-cigarette vapor exposure, ventilation patterns were altered in the vapor exposed animals which decreased in tidal volume and minute ventilation suggesting possible impairment of lung function and e-cigarette exposure modified the expression of pro-inflammatory cytokine, IL-1ꞵ.
