It is only in the past decade or so that the fundamental role of nicotine in sustaining smoking behaviour has begun to be more widely accepted. It is now recognised that cigarette smoking is primarily a manifestation of nicotine addiction and that smokers have individually characteristic preferences for their level of nicotine intake. Smokers regulate the way they puff and inhale to achieve their desired nicotine dose.
The link with nicotine addiction does not imply that pharmacological factors drive smoking behaviour in a simple way and to the exclusion of other influences. Social, economic, personal, and political influences all play an important part in determining patterns of smoking prevalence and cessation. Acupuncture and related techniques are promoted as a treatment for smoking cessation in the belief that they may reduce nicotine withdrawal symptoms.
Objectives
The objectives of this review are to determine the eBectiveness of acupuncture and the related interventions of acupressure, laser therapy and electrostimulation in smoking cessation, in comparison with no intervention, sham treatment, or other interventions.
Search methods
We searched the Cochrane Tobacco Addiction Group Specialized Register (which includes trials of smoking cessation interventions identified from the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE, EMBASE, and PsycINFO) and AMED in October 2013. We also searched four Chinese databases in September 2013: Sino-Med, China National Knowledge Infrastructure, Wanfang Data and VIP.
Selection criteria
Randomized trials comparing a form of acupuncture, acupressure, laser therapy or electrostimulation with either no intervention, sham treatment or another intervention for smoking cessation.
Data collection and analysis
We extracted data in duplicate on the type of smokers recruited, the nature of the intervention and control procedures, the outcome measures, method of randomization, and completeness of follow-up. We assessed abstinence from smoking at the earliest time-point (before six weeks) and at the last measurement point between six months and one year. We used the most rigorous definition of abstinence for each trial, and biochemically validated rates if available. Those lost
to follow-up were counted as continuing smokers. Where appropriate, we performed meta-analysis pooling risk ratios using a fixed-eBect model.
Main results
We included 38 studies. Based on three studies, acupuncture was not shown to be more eBective than a waiting list control for longterm abstinence, with wide confidence intervals and evidence of heterogeneity (n = 393, risk ratio [RR] 1.79, 95% confidence interval [CI] 0.98 to 3.28, IO = 57%). Compared with sham acupuncture, the RR for the short-term eBect of acupuncture was 1.22 (95% CI 1.08 to 1.38),and for the long-term eBect was 1.10 (95% CI 0.86 to 1.40). The studies were not judged to be free from bias, and there was evidence of funnel plot asymmetry with larger studies showing smaller eBects. The heterogeneity between studies was not explained by the technique used. Acupuncture was less eBective than nicotine replacement therapy (NRT). There was no evidence that acupuncture is superior to
psychological interventions in the short- or long-term. There is limited evidence that acupressure is superior to sham acupressure for shortterm outcomes (3 trials, n = 325, RR 2.54, 95% CI 1.27 to 5.08), but no trials reported long-term eBects, The pooled estimate for studies testing an intervention that included continuous auricular stimulation suggested a short-term benefit compared to sham stimulation (14 trials, n = 1155, RR 1.69, 95% CI 1.32 to 2.16); subgroup analysis showed an eBect for continuous acupressure (7 studies, n = 496, RR 2.73, 95% CI 1.78 to 4.18) but not acupuncture with indwelling needles (6 studies, n = 659, RR 1.24, 95% CI 0.91 to 1.69). At longer follow-up the CIs did not exclude no eBect (5 trials, n = 570, RR 1.47, 95% CI 0.79 to 2.74). The evidence from two trials using laser stimulation was inconsistent and could not be combined. The combined evidence on electrostimulation suggests it is not superior to sham electrostimulation (shortterm
abstinence: 6 trials, n = 634, RR 1.13, 95% CI 0.87 to 1.46; long-term abstinence: 2 trials, n = 405, RR 0.87, 95% CI 0.61 to 1.23).
Authors' conclusions
Although pooled estimates suggest possible short-term eBects there is no consistent, bias-free evidence that acupuncture, acupressure, or laser therapy have a sustained benefit on smoking cessation for six months or more. However, lack of evidence and methodological problems mean that no firm conclusions can be drawn. Electrostimulation is not eBective for smoking cessation. Well-designed research into acupuncture, acupressure and laser stimulation is justified since these are popular interventions and safe when correctly applied, though these interventions alone are likely to be less eBective than evidence-based interventions.

Addiction to nicotine gum has never been described in never smokers or in never users of tobacco.
Methods
Internet questionnaire in 2004–2006 in a self-selected sample of 434 daily users of nicotine gum. To assess dependence on nicotine gum, we used modified versions of the Nicotine Dependence Syndrome Scale (NDSS), the Fagerström Test for Nicotine Dependence and the Cigarette Dependence Scale.
Results
Five never smokers used the nicotine gum daily. They had been using the nicotine gum for longer than the 429 ever smokers (median = 6 years vs 0.8 years, p = 0.004), and they had higher NDSS-gum Tolerance scores (median = 0.73 vs = -1.0, p = 0.03), a difference of 1.5 standard deviation units. Two never smokers had never used smokeless tobacco, both answered "extremely true" to: "I use nicotine gums because I am addicted to them", both "fully agreed" with: "after a few hours without chewing a nicotine gum, I feel an irresistible urge to chew one" and: "I am a prisoner of nicotine gum".
Conclusion
This is to our knowledge the first report of addiction to nicotine gum in never users of tobacco. Yet, at this time, there remains limited data regarding the safety and efficacy in using these nicotine products during pregnancy. Notably, reports assessing the effect of nicotine exposure on postnatal health outcomes in humans, including reproductive health, are severely lacking. Our current understanding regarding the consequences of nicotine exposure during pregnancy is limited to a few animal studies, which do not comprehensively address the underlying cellular mechanisms involved. This paper aims to critically review the current knowledge from human and animal studies regarding the direct and indirect effects (e.g. obesity) of maternal nicotine exposure, regardless of its source, on reproductive outcomes in pregnancy and postnatal life. Furthermore, this review highlights several key cellular mechanisms involved in these adverse reproductive deficits including oxidative stress, inflammation, and endoplasmic reticulum (ER) stress. Electronic cigarettes (e-cigarettes) are often considered a “safe substitute” for conventional cigarette cessation. The composition of the fluid is not always clearly defined and shows a large variation within brands and manufacturers. More than 80 compounds were detected in liquids and aerosols. E-cigarettes contain nicotine, and the addition of flavorings increases the toxicity of e-cigarette vapour in a significant manner. The heat generated by the e-cigarette leads to the oxidation
and decomposition of its components, eventually forming harmful constituents in the inhaled vapour. The effects of these toxicants on male and female reproduction are well established in conventional cigarette smokers. Although toxins were measured at much lower levels in e-cigarette aerosols compared to smoke from a conventional cigarette, there are concerns about their potential impact on male and female reproduction. The information available was mainly obtained from studies conducted in animal models, and investigations in humans are scarce. However, the effects observed in animal models suggest that caution should be taken when vaping and that more research needs to be conducted to identify its potential adverse effects on fertility. The prevalence of e-cigarette usage is alarming, and warnings should be made about the impact of vaping on reproductive health. To assess levels of nicotine, nicotine degradation products and some specific impurities in commercial refill liquids for electronic cigarettes.
Design and setting
We analyzed 20 models of 10 of the most popular brands of refill liquids, using gas and liquid chromatography.
Measurements
We assessed nicotine content, content of the known nicotine degradation products and impurities, and presence of ethylene glycol and diethylene glycol.
Findings
The nicotine content in the bottles corresponded closely to the labels on the bottles. The levels of nicotine degradation products represented 0–4.4% of those for nicotine, but for most samples the level was 1–2%. Cis-N-oxide, trans-N-oxide, myosmine, anatabine and anabasine were the most common additional compounds found. Neither ethylene glycol nor diethylene glycol were detected.
Conclusion
The nicotine content of electronic cigarette refill bottles is close to what is stated on the label. Le médecin qui accompagne un fumeur dans sa tentative d'arrêt doit être conscient du problème et en parler ouvertement avec le fumeur prêt à entreprendre une tentative d'arrêt. Les régimes stricts sont probablement contre-indiqués, mais des conseils diététiques sont utiles. Artificial sweeteners are listed as ingredients of oral nicotine pouches (ONPs), a new product category with rapidly growing market share. The exact sweetener contents of ONPs remain unknown. Artificial sweeteners in ONPs may facilitate initiation and encourage consumption behavior.
Aims and Methods:
Artificial sweetener contents in major US-marketed ONP brands (Zyn, on!, Velo) were determined by Liquid Chromatography-Mass Spectrometry (LC-MS). Sweetener effects during the initiation of ONP consumption were modeled in single- and two-bottle tests, offering mice ONP extracts calibrated to contain nicotine levels similar to saliva of people who use smokeless tobacco. To examine the contribution of sweet taste perception, consumption behavior was compared between wild-type mice and mice deficient in the sweet taste receptor (Tas1r2-/-).
Results:
Acesulfame-K was detected in on!, Zyn and Velo ONPs (~0.3-0.9 mg/pouch), including products marketed as “Unflavored” or “Flavor ban approved”. In Velo ONPs, sweetened with sucralose (0.6-1.2 mg/pouch), higher nicotine strength products contained higher sucralose levels. Tas1r2-/- mice consumed less ONP extracts than wild-type mice in both sexes. ONP extracts with both higher nicotine and sweetener strengths were tolerated by wildtype mice, but produced stronger aversion in Tas1r2-/- mice.
Conclusions:
ONPs contain significant amounts of artificial sweeteners, with some brands adding more sweetener to ONPs with higher nicotine strengths. Artificial sweeteners, at levels present in ONPs, increase nicotine consumption. Increasing sweetener contents facilitates consumption of ONPs with higher nicotine strengths. Sweetness is a key determinant of ONP use initiation, likely reducing the aversive sensory effects of nicotine and other ONP constituents.
Implications:
Artificial sweeteners such as acesulfame-K or sucralose reduce aversion and facilitate initiation and continued consumption of ONPs. The marketing of some artificially sweetened ONPs as
“Unflavored” of “Flavor ban-approved” suggests that the tobacco industry rejects sweet taste as a determinant for the presence of a characterizing flavor. Sweetness as imparted by artificial
sweeteners in tobacco products needs to be addressed by regulators as a component of a characterizing flavor, with the aim to reduce product appeal and initiation by never users, and

a broad range of different electronic devices with which to inhale aerosols. The e-liquid vaporisation process comprises a chain of several chemical reactions which result in hundreds of chemical compounds. The composition and concentration of chemicals contained in e-liquids or, more precisely, in the resulting aerosol,are difficult to identify and quantify. This review article includes a summary of current evidence about the toxicity of selected chemical substances contained in
electronic cigarette liquids. Forty initial sources were selected in online databases on the basis of a nonsystematic
literature review using a set of keywords.
This overview presents selected categories of e-liquid ingredients according to their functions and chemical composition. The most frequent substances include bulking agents (propylene glycol and glycerol). So far,the least-explored category consists of a wide range of flavourings. A separate category is represented by nicotine, the cause of the development of physical dependence. Another, highly diverse group of compounds is chemical substances formed as byproducts of vaporisation. To review the available evidence evaluating the chemicals in refill solutions, cartridges, aerosols and environmental emissions of electronic cigarettes (e-cigarettes).
Methods
Systematic literature searches were conducted to identify research related to e-cigarettes and chemistry using 5 reference databases and 11 search terms. The search date range was January 2
007 to September 2013. The search yielded 36 articles, of which 29 were deemed relevant for analysis.
Results
The levels of nicotine, tobacco-specific nitrosamines (TSNAs), aldehydes, metals, volatile organic compounds (VOCs), flavours, solvent carriers and tobacco alkaloids in e-cigarette refill solutions, cartridges, aerosols and environmental emissions vary considerably. The delivery of nicotine and the release of TSNAs, aldehydes and metals are not consistent across products. Furthermore, the nicotine level listed on the labels of e-cigarette cartridges and refill solutions is often significantly different from measured values. Phenolic compounds, polycyclic aromatic hydrocarbons and drugs have also been reported in e-cigarette refill solutions, cartridges and aerosols. Varying results in particle size distributions of particular matter emissions from e-cigarettes across studies have been observed. Methods applied for the generation and chemical analyses of aerosols differ across studies. Performance characteristics of e-cigarette devices also vary across and within brands.
Conclusions Additional studies based on knowledge of e-cigarette user behaviours and scientifically validated aerosol generation and chemical analysis methods would be helpful in generating reliable measures of chemical quantities. This would allow comparisons of e-cigarette aerosol and traditional smoke constituent levels and would inform an evaluation of the toxicity potential of e-cigarettes

However, there is a major knowledge gap surrounding the effects of e-cig aerosols on pregnancy and fetal development. We aimed to evaluate the effects of vaping e-cigs during gestation on offspring growth and to asses if growth deficits are accompanied by altered maternal and fetal vascular hemodynamics. Sprague ?Dawley dams were assigned to Pair-Fed Control, Pair-Fed Juice, or Juice+Nicotine groups, and then underwent either a prenatal or prenatal+postnatal exposure paradigm in a custom-engineered vaping system. Mass spectrometry identified major aerosolized constituents from e-cig vaping. The Juice+Nicotine group exhibited significantly decreased fetal weight and crown-rump length (#46.56%, and #23.83%, respectively). Pre- and postnatal exposure to Juice+Nicotine resulted in decreased pup weight at postnatal day (PND) 4?10. Crown-rump length was decreased by 24.71% on PND 10. Blood flow in the Juice+Nicotine group was decreased in the maternal uterine and fetal umbilical circuits by 49.50% and 65.33%, respectively. We conclude that chronic exposure to e-cig aerosols containing nicotine during early development can have deleterious health effects on the exposed offspring. Vaping e-cigs containing nicotine during pregnancy lead to a reduction in offspring weight and crown-rump length, associated with a marked decrease in blood flow in both the maternal uterine and fetal umbilical circulation (a strong indicator of growth restriction). Individuals experienced increased social isolation resulting from the COVID-19 pandemic. Studies have found social isolation and loneliness to be strongly associated with anxiety and depression, which have been associated with increased smoking and vaping rates among young
adults, including college students.
Objectives:
To examine relationships between psychological distress and nicotine use within the context of the COVID-19 pandemic.
Methods:
A cross-sectional online survey (n = 4634; 77.9% female) was used to collect nicotine use and psychological measures from students enrolled at a large Midwestern university. Timeline follow-back data were collected from students reporting current cigarette or electronic cigarette (e-cigarette) use in the week before and immediately following the closure of campus due to the pandemic. Generalized estimating equations were used to examine the interaction between nicotine use and psychological symptoms across the 2-week period.
Results:
Both cigarette (Rate ratio (RR) = 1.115, 95% CI = 1.061, 1.171, p < .0001) and e-cigarette (β = 0.258, 95% CI = 0.166, 0.351, p < .0001) use increased significantly following campus closure. Students experiencing higher levels of depression reported greater increases in e-cigarette use frequency over time as compared to students reporting fewer symptoms of depression (β = 0.018, 95% CI = 0.006, 0.030, p = .004).
Conclusions: Increases in nicotine use were found immediately following the implementation of public health safety measures that closed most university campuses. Additional and/or increased stressors have potentially impacted young adults who are college students as a result of campus closures resulting from the pandemic, which may have contributed to further increases in nicotine

Cigarette smoking—highly prevalent during pregnancy—is associated with addiction and withdrawal syndrome in adults. We conducted a prospective, two-group parallel study on 17 consecutive newborns of heavy-smoking mothers and 16 newborns of nonsmoking, unexposed mothers (controls). Neurologic examinations were repeated at days 1, 2, and 5. Finnegan withdrawal score was assessed every 3 h during their first 4 d. Newborns of smoking mothers had significant levels of cotinine in the cord blood (85.8 ± 3.4 ng/mL), whereas none of the controls had detectable levels. Similar findings were observed with urinary cotinine concentrations in the newborns (483.1 ± 2.5 μg/g creatinine versus 43.6 ± 1.5 μg/g creatinine; p = 0.0001). Neurologic scores were significantly lower in newborns of smokers than in control infants at days 1 (22.3 ± 2.3 versus 26.5 ± 1.1; p = 0.0001), 2 (22.4 ± 3.3 versus 26.3 ± 1.6; p = 0.0002), and 5 (24.3 ± 2.1 versus 26.5 ± 1.5; p = 0.002). Neurologic scores improved significantly from day 1 to 5 in newborns of smokers (p = 0.05), reaching values closer to control infants. Withdrawal scores were higher in newborns of smokers than in control infants at days 1 (4.5 ± 1.1 versus 3.2 ± 1.4; p = 0.05), 2 (4.7 ± 1.7 versus 3.1 ± 1.1; p = 0.002), and 4 (4.7 ± 2.1 versus 2.9 ± 1.4; p = 0.007). Significant correlations were observed between markers of nicotine exposure and neurologic-and withdrawal scores. Since e-cigarettes appeared in the mid-2000s, some practitioners, researchers, and policy makers have embraced them as a safer alternative to conventional cigarettes and an effective way to stop smoking. While e-cigarettes deliver lower levels of carcinogens than do conventional cigarettes, they still expose users to high levels of ultrafine particles and other toxins that may substantially increase cardiovascular and noncancer lung disease risks, which account for more than half of all smoking-caused deaths, at rates similar to conventional cigarettes. Moreover, rather than stimulating smokers to switch from conventional cigarettes to less dangerous e-cigarettes or quitting altogether, e-cigarettes are reducing smoking cessation rates and expanding the nicotine market by attracting youth.

E-cigarettes, also referred to as electronic cigarettes, are gaining popularity as an alternative to tobacco use. Although their use has significantly increased recently, there is still much debate over their effectiveness and safety as smoking cessation aids.
Objective:
This review paper’s objective is to offer an inclusive evaluation of the body of research about the impact of ecigarettes on the oral microbiome and oral health.
Methodology: Data was extracted from PubMed, Google Scholar, and Scopus to compile scientific evidence for a narrative review article on the effects of e-cigarettes on the oral microbiome. The investigation was restricted to published research articles from 2001 to 2022, using pre-defined search phrases for e-cigarettes, vaping, quitting smoking, oral microbiota, and oral health.
Results:
The chemicals in e-cigarettes, including nicotine, propylene glycol, vegetable glycerin, flavorings, and chemical additives, cause a dysbiosis that can lead to a variety of conditions, including dental caries, periodontal diseases, and a variety of mental health issues, such as bone loss, gingival tissue inflammation, and increased plaque accumulation. The chemical toxicity of e-cigarettes, including nicotine and other components present in e-cigarette aerosols, has been implicated in various oral health problems that lead to systemic complications.
Conclusion:
A wide range of oral health sequelae may be associated with e-cigarette use. Young people who become addicted to nicotine are at greater risk of becoming lifelong tobacco consumers. The use of nicotine-delivering electronic cigarettes has risen dramatically among youths worldwide. In addition to physical dependence, adolescents are susceptible to social and environmental influences to use electronic cigarettes. The product design, flavours, marketing, and perception of safety and acceptability have increased the appeal of electronic cigarettes to young people, thus leading to new generations addicted to nicotine. Moreover, there is growing evidence that electronic cigarettes in children and adolescents serve as a gateway to cigarette
smoking. There can be no argument for harm reduction in children. To protect this vulnerable population from electronic cigarettes and other nicotine delivery devices, we recommend that electronic cigarettes be regulated as tobacco products and included in smoke-free policies. Sale of electronic cigarettes should be
barred to youths worldwide. Flavouring should be prohibited in electronic cigarettes, and advertising accessible by youths and young adults be banned. Materials and methods: HaCaT, UMSCC10B, and HN30 were treated with nicotine-containing and
nicotine-free vapor extract from two popular e-cigarette brands for periods ranging from 48 h to 8 weeks.
Cytotoxicity was assessed using Annexin V flow cytometric analysis, trypan blue exclusion, and clono?genic assays. Genotoxicity in the form of DNA strand breaks was quantified using the neutral comet assay and c-H2AX immunostaining.
Results: E-cigarette-exposed cells showed significantly reduced cell viability and clonogenic survival, along with increased rates of apoptosis and necrosis, regardless of e-cigarette vapor nicotine content. They also exhibited significantly increased comet tail length and accumulation of c-H2AX foci, demon?strating increased DNA strand breaks.
Conclusion: E-cigarette vapor, both with and without nicotine, is cytotoxic to epithelial cell lines and is a DNA strand break-inducing agent. The GEO DataSet search was obtained from the National Center for Biotechnology Information (NCBI) Gene Expression Omnibus (GEO), a relevant biomedical data source. The results of our investigation revealed a total of 11 GEO DataSets of Homo sapiens that met the inclusion and exclusion criteria for identifying Differentially Expressed Genes (DEGs). These datasets, namely GSE125217, GSE148812, GSE125416, GSE105445, GSE56398, GSE51284, GSE71795, GSE56383, GSE40689, GSE11208, and GSE11142, contained genes that underwent altered expression in response to nicotine exposure. The results of the gene analysis were further categorized based on their functional classifications. They encompass receptor genes such as CHRNA9, nAChRs, and TLR4, regulatory genes including CDK1, CHK1, ERBB2, EGFR, and E2F1, structural genes like H-Caldesmon, L-Caldesmon, SM22, CDH1/3, BDNF/NT-3, and MLL3, immunological genes such as TNF-α, IL-1β, IL-6, IL-10, MCSF, MCP-1, and ICAM-1, metabolic genes like CYP2A6 and APOE, and enzymatic genes such as PITRM1, DDR2, DHRS7, and SLC16A7. This review search provides a comprehensive insight into the molecular-level impact of nicotine, with potential implications for the development of treatment strategies and the discovery of relevant biomarkers associated with nicotine use.

Introduction: Electronic cigarettes' (e-cigarettes) viability as a public health strategy to end smoking will likely be determined by their ability to mimic the pharmacokinetic profile of a cigarette while also exposing users to significantly lower levels of harmful/potentially harmful constituents (HPHCs). The present study examined the nicotine delivery profile of third- (G3) versus second-generation (G2) e-cigarette devices and their users' exposure to nicotine and select HPHCs compared with cigarette smokers.
Methods: 30 participants (10 smokers, 9 G2 and 11 G3 users) completed baseline questionnaires and provided exhaled carbon monoxide (eCO), saliva and urine samples. Following a 12-hour nicotine abstinence, G2 and G3 users completed a 2-hour vaping session (ie, 5 min, 10-puff bout followed by ad libitum puffing for 115 min). Blood samples, subjective effects, device characteristics and e-liquid consumption were assessed.
Results: Smokers, G2 and G3 users had similar baseline levels of cotinine, but smokers had 4 and 7 times higher levels of eCO (p<0.0001) and total 4-(Methylnitrosamino)-1-(3-pyridyl)-1-butanol (i.e., NNAL, p<0.01), respectively, than G2 or G3 users. Compared with G2s, G3 devices delivered significantly higher power to the atomiser, but G3 users vaped e-cigarette liquids with significantly lower nicotine concentrations. During the vaping session, G3 users achieved significantly higher plasma nicotine concentrations than G2 users following the first 10 puffs (17.5 vs 7.3 ng/mL, respectively) and at 25 and 40 min of ad libitum use. G3 users consumed significantly more e-liquid than G2 users. Vaping urges/withdrawal were reduced following 10 puffs, with no significant differences between device groups.
Discussion: Under normal use conditions, both G2 and G3 devices deliver cigarette-like amounts of nicotine, but G3 devices matched the amount and speed of nicotine delivery of a conventional cigarette. Compared with cigarettes, G2 and G3 e-cigarettes resulted in significantly lower levels of exposure to a potent lung carcinogen and cardiovascular toxicant. These findings have significant implications for understanding the addiction potential of these devices and their viability/suitability as aids to smoking cessation.

Plusieurs études récentes soulignent l’impact de l’E-cig sur la santé, en ce compris sur le Système CardioVasculaire (SCV). Toutefois, les connaissances relatives à la nocivité de la E-cig sont à ce jour encore limitées car ce produit reste récent. Notre étude a pour objectif de compléter les connaissances existantes en investiguant l’impact de l’utilisation aiguë de la E-cig et de l’arrêt succinct du vapotage sur la Variabilité de la Fréquence Cardiaque (VFC). Ce biomarqueur reflète la balance entre le SCV contrôlé par les parties parasympathique (SNP) et sympathique (SNS) du Système Nerveux Autonome (SNA). En effet, une diminution importante de la VFC est associée à l’augmentation de l’équilibre sympathico-vagal. Celle-ci, aboutit à l’augmentation des risques cardiovasculaires. Sur base de ces études, et de l’effet sympathomimétique de la E-cig, nous avons voulu compléter des données préexistantes provenant de l’article « Chaumont et al., 2020 ». Ce travail étudie l’impact du vapotage aigu et sa réversibilité sur plusieurs paramètres cardio-respiratoires chez des consommateurs réguliers de E-cig. Trois sessions séparées par un minimum de 7 jours ont été étudiées. Une session durant laquelle les utilisateurs ont vapoté des E-cigs contenant de la nicotine (Nicotine session); ou ne contenant pas de nicotine (Nicotine free-session); ou se sont sevrés de la E-cig (Stop-session). Conformément à la littérature, nos résultats suggèrent que (1) l’exposition aiguë à la nicotine contenue dans le e-liquide s’accompagne d’une augmentation directe de l’activité du SNS ; (2) la prédominance du SNS est probablement due à la nicotine ; (3) le vapotage de E-cig contenant de la nicotine ne déclenche pas de prédominance du SNS à l’état de base par rapport à la session de sevrage. Cependant, des études complémentaires seront nécessaires pour étudier l’impact de l’arrêt à long terme de la E-cig et sa possible réversibilité de sa prédominance sur le SNS. Son électronique sophisitiquée vaporise, séparément pour chaque bouffée, de très petites quantités de nicotine dissolues dans du propylene glycol, deux petite molécules bien connues ayant des profils de sécurité excellents - dans un bon aérosol. Chaque bouffée contient d'un tiers à la moitié de la nicotine d'une bouffée de cigarette au tabac. Le liquide de la cartouche est sans tabac et aucune combustion n'a lieu. Une fois sur le marché, le maintien de son profil de sécurité dépend : 1) de bonnes pratiques de fabrication et d'une pureté pharmaceutique de la nicotine et du propylene glycol utilisés dans le liquide de la cartouche 2) de la prévention d'un usage partagé dont pourrait résulter une infection croisée. Début 2008, l'unité de recherche des essais cliniques de l'Université d'Auckland est en train de tester l'efficacité de l'e-cigarette en ce qui concerne l'augmentation du taux de nicotine dans le sang et sur la réduction du manque de la cigarette. There is a close relationship between tobacco smoking and psychiatric disorders, and a higher proportion of individuals with mental health conditions smoke compared with the general population. Due to the increased smoking prevalence in this population, patients with psychiatric conditions are at greater risk of smoking-related morbidity and mortality and experience detrimental effects on their quality of life. However, while the majority of individuals with a history of mental health conditions appreciate that smoking is detrimental to their health, they are less likely to quit smoking and have a lower success rate during quit attempts compared with the general population.
SCOPE:
Peer-reviewed articles were identified from PubMed using the inclusive date-range of 1990 - October 2008 and the search terms; depression, mental health, psychiatric disorders, schizophrenia, and smoking cessation. Articles were selected from the search results to provide a general overview of some of the main issues for smokers with psychiatric disorders in general and specifically, those with schizophrenia and depression. The evidence from smoking cessation trials within these populations was also reviewed.
FINDINGS:
Nicotine has some positive effects on symptoms of psychiatric disorders and it has been proposed that patients with mental health conditions may smoke as a form of self-medication. Further, several studies have shown that some symptoms of psychiatric disorders may be exacerbated by nicotine withdrawal. Therefore, attempts to quit smoking pose additional problems to patients with mental health problems.
CONCLUSION:
Traditional programmes for smoking cessation may not always be suitable for psychiatric patients due to their neuropsychological profile. Preliminary evidence suggests that more flexible, open-ended, combination approaches of pharmacotherapy and counselling may be more successful. Although most biomarkers of tobacco-smoke exposure have a relatively short half-life, hair nicotine can measure several months of cumulative SHS exposure. Design: A cross-sectional study of hospitalityindustry workers. Method: Hair samples were obtained from 207 bar and restaurant workers and analyzed by the reversed-phase high-performance liquid chromatography with electrochemical detection (HPLCECD) method. Self-reported tobacco use and sources of SHS exposure were assessed. Findings: Higher hair-nicotine levels were associated with more cigarettes smoked per day among smokers and a greater number of SHS-exposure sources among nonsmokers. Number of SHS exposure sources, gender, number of cigarettes smoked per day, and type of establishment predicted hair-nicotine levels. Discussion: Hair nicotine is a valid measure of SHS exposure. It may be used as an alternative biomarker to measure longer term SHS exposure.
