[TABAC] tabagisme:tabagisme actifIndex. décimale : TA 3.2.1 Mortalité Résumé : L'objectif de cette étude est de comparer les risques du tabagisme chez les hommes qui ont commencé à fumer à différentes périodes et l'ampleur de la réduction du risque quand on cesse de fumer à différents âges. Étude qui a continué de 1951 à 2001 au Royaume-Uni, dans laquelle 34439 médecins britanniques masculins ont participé. Des informations sur leurs habitudes de consommation du tabac ont été obtenues en 1951, et périodiquement ensuite. La mortalité spécifique causée par la tabac a été surveillée pendant 50 années. La diminution progressive substantielle des taux de mortalité chez les non-fumeurs durant la second moitié du siècle passé (due à la prévention et au traitement amélioré des maladies) a été complètement compensée, parmi les fumeurs de cigarette, par une augmentation progressive des taux de mortalité, due à une utilisation plus intensive des cigarettes et à un rajeunissement de l'âge du début du tabagisme. Parmi les hommes nés autour de 1920, le tabagisme a triplé les taux de mortalité, mais arrêter de fumer à l'âge 50 ans divise en deux le risque, et arrêter à l'âge de 30 ans élimine presque tout le risque. It increases the risk of growth restriction, preterm birth, miscarriage, and perinatal death but despite this over a quarter of pregnant women in the United Kingdom smoke. Pregnancy motivates a minority to stop for at least part of the pregnancy, but most start again after giving birth. The largest part of most cigarette butts is a non-biodegradable plastic filter made of cellulose acetate.3 Filters first appeared in the 1950s following early health concerns about cigarettes, and their rapid adoption was helped by the post-war explosion in manufacturing of plastics.4 The tobacco industry portrayed filters as a way to make cigarettes safer by absorbing some of the “tar” that was implicated in the lung cancer epidemic. Design Randomised controlled trial.
Setting Five general practices in Hertfordshire, England.
Participants 561 current smokers aged over 35.
Intervention All participants were offered spirometric assessment of lung function. Participants in intervention group received their results in terms of “lung age” (the age of the average healthy individual who would perform similar to them on spirometry). Those in the control group received a raw figure for forced expiratory volume at one second (FEV1). Both groups were advised to quit and offered referral to local NHS smoking cessation services.
Main outcome measures The primary outcome measure was verified cessation of smoking by salivary cotinine testing 12 months after recruitment. Secondary outcomes were reported changes in daily consumption of cigarettes and identification of new diagnoses of chronic obstructive lung disease.
Results Follow-up was 89%. Independently verified quit rates at 12 months in the intervention and control groups, respectively, were 13.6% and 6.4% (difference 7.2%, P=0.005, 95% confidence interval 2.2% to 12.1%; number needed to treat 14). People with worse spirometric lung age were no more likely to have quit than those with normal lung age in either group. Cost per successful quitter was estimated at £280 (€366, $556). A new diagnosis of obstructive lung disease was made in 17% in the intervention group and 14% in the control group; a total of 16% (89/561) of participants.
Conclusion Telling smokers their lung age significantly improves the likelihood of them quitting smoking, but the mechanism by which this intervention achieves its effect is unclear.Permalink : https://biblio.fares.be/opac_css/index.php?lvl=notice_display&id=8133 Exemplaires (1)

The tobacco industry predicted that, with the removal of border controls in the European Union, price differences between neighbouring countries would lead to a diversion of tobacco trade, legally and illegally, to countries with cheaper cigarettes. According to them this diversion would be through increased cross border shopping for personal consumption or through increased smuggling of cheap cigarettes from countries with low tax to countries with high tax, where cigarettes are more expensive. These arguments have been used to urge governments not to increase tax on tobacco products. Le BMJ y consacre une bonne partie de sa dernière édition. Beaucoup avaient dit qu’avec cette législation on avait ouvert la boîte de Pandore. En fait, les bars n’ont pas été désertés et aucun des malheurs prédits n’est venu frapper le monde. Le seul constat à court terme que l’on a pu faire est une diminution de la cotinine, un métabolite de la nicotine, chez les fumeurs comme chez les non-fumeurs. Trois études sont disponibles en ligne pour le démontrer. Le week-end dernier, on a appris que de plus en plus de chirurgiens refusaient d’intervenir chez des patients fumeurs ou obèses… Lorsque la loi permet de réduire le tabagisme, on applaudit, lorsqu’elle stigmatise des personnes au point qu’on leur refuse des soins, on doute de son intérêt.
