These two components contribute about equally to the success of the intervention. Doctors and other health professionals should therefore be familiar with what these strategies offer, encourage smokers to use them, and be able at least to provide simple advice and behavioural support to smokers. They also need to be familiar with other sources of support, such as written materials, telephone helplines, and strategies for preventing relapses. They have therefore not been widely prescribed by doctors who help smokers wanting to quit. Recent changes in funding policy in the United Kingdom and new guidance from the National Institute for Clinical Excellence (which covers England and Wales) mean that NRT products can and should now be made available to all smokers who want to stop smoking. Like other pharmacological interventions for helping smokers to quit (see the next article in this series), NRT is most effective when used in conjunction with behavioural and other types of non-pharmacological cessation interventions. [TABAC] tabagisme:risque:facteur associé:précaritéIndex. décimale : TA 6.2.3.1 Substitution nicotinique Résumé : Les fumeurs vivant dans des conditions de précarité cumulent un ensemble de difficultés qui les rendent particulièrement vulnérables au niveau de leur parcours tabagique. Dès l’initiation – souvent précoce –, leur trajectoire est parsemée d’obstacles : à cause d’un niveau plus élevé de consommation et de dépendance, en raison de l’environnement social ainsi que de ressources personnelles moins mobilisables, au seuil puis au coeur de l’arrêt, de tentatives où l’accès à l’aide peut être également compromis. C’est donc un groupe de fumeurs vulnérables qui nécessite, de la part des professionnels, une attention renouvelée qui prenne en considération les différents facteurs entravant leur projet d’arrêt. En tant que professionnel de la santé, en tant que tabacologue, à quels niveaux, de quelles manières pouvons-nous soutenir ces fumeurs précarisés ? Dès lors que les chances de réussite d’un sevrage augmentent considérablement si l'on combine un moyen d'aide au sevrage et une forme de soutien comportemental, quel type d’action simple et abordable peut venir incliner favorablement leur cheminement ? L’objectif de ce mémoire est d’explorer comment un accès facilité aux substituts nicotiniques (SN) au cours d’un accompagnement tabacologique – voire dans un processus de sensibilisation / mise en projet – peut influencer la trajectoire et la réussite des consommateurs affectés par la précarité. Les projets-pilotes présentés dans ce mémoire visant la mise à disposition gratuite de SN dans le cas d’accompagnements individuels au cours de consultations ou via la ligne gratuite téléphonique tabacstop ont montré des taux de sevrage significativement élevés, traduisant l’impact de la gratuité des substituts sur le parcours du fumeur. Les usagers envisagent d’avoir recours aux SN à l’avenir, si besoin. Les données vérifient l’hypothèse que l’usage préalable d’un SN exerce une influence utile en facilitant la démarche de sevrage, qu’elle soit faite sans aide, avec un tabacologue ou via une ligne téléphonique gratuite : le fumeur qui a fait l’expérience des substituts lors d’un sevrage se sent muni pour faire face à un arrêt. Cette évolution du sentiment d’efficacité personnelle au départ de la (re)qualification des substituts par l’usager vulnérable est recoupée par les réponses à un questionnaire sur la représentation sur les médicaments et sur les SN proposé à un public de fumeurs en précarité ayant bénéficié d’animation de groupes au cours desquelles sont assumés divers défis de privation volontaire avec l’aide de substituts nicotiniques. Arterial stiffness can be assessed noninvasively based on 2 indices: pulse-wave velocity (PWV), indicating aortic stiffness, and the augmentation index (AIx), indicating aortic wave reflection. The impact of smoking cessation (SC) with nicotine replacement therapies (NRT) on arterial stiffness remains unknown.
Methods: AIx and PWV were studied prospectively (using a SphygmoCor system) in 26 long-term smokers (>10 cigarettes/day; mean age 43 ± 6 years) before (visit 1) and 28 and 56 days (visit 2 and visit 3) after SC supported by NRT. Abstinence was ascertained when exhaled carbon monoxide (CO) was <10 ppm; the quantity of NRT absorbed was assessed based on the serum cotinine level.
Results: Sixteen of 24 patients (67%) were abstinent at the end of the study; 8/24 had cut down on smoking, and 2 were lost to follow-up. CO and serum cotinine levels fell after SC. The mean AIx for the population as a whole was 23.4% at visit 1, with a significant early reduction by visit 2 (16.2%) and visit 3 (13.9%) (ANOVA, p < 0.001). The PWV, peripheral blood pressure, and heart rate were unchanged.
Conclusions: The improvement in AIx and the stability of PWV after SC with NRT indicate improved peripheral vascular tone. This impact may account for the early clinical benefit of SC observed even when associated with NRT.

[TABAC] tabagisme:risque:facteur associé:alcoolIndex. décimale : TA 6.2.3.1 Substitution nicotinique Résumé : This article reviews and comments on studies of treatment and prevention of cigarette smoking in individuals with comorbid psychiatric and non-nicotine substance abuse disorders. Despite a high prevalence of cigarette smoking in these populations and interest in quitting, treatment interventions and studies of these interventions are sparse. Multiple barriers to implementation of interventions exist. Existing data suggest that provision of cigarette-smoking interventions in substance abuse treatment patients is efficacious and does not appear to interfere with abstinence from alcohol or illicit drugs, but more research is needed. There are few studies in populations with psychiatric disorders, with the exception of studies of individuals with a history of major depressive disorder. The available data suggest at least moderate efficacy and little evidence of exacerbation of these disorders. Integration of interventions into existing treatment clinics appears desirable. Despite the identification of subgroups that are especially likely to adopt cigarette smoking, there have been no targeted prevention efforts. Further research is recommended in both the treatment and prevention of cigarette smoking in individuals with psychiatric and substance abuse disorders. It is reasonable to offer existing treatments to these subgroups of smokers, since there is some evidence of efficacy and little evidence of harm.

A homogenous group of medical doctors, researchers, and other experts have met in Castres, France with the patronage of Pierre Fabre Laboratories. This consensus and exchange meeting of high scientific level, gather experts with the aim of drawing up recommendations and guidelines thanks to the contribution of distinguished specialists. This consensus of expert will have the task to draw up proposals so as to standardize Indications and Contraindications of Nicotine Replacement Therapies. This involves reduction in consumption, temporary cessation, increase in the dose prescribed and the ability to combine 2 NRTs, NRTs in persons under the age of 18, the use during pregnancy, and in case of cardio-vascular diseases. All of these exciting topics will surely lead to discussions and conclusions that will benefit smokers. to review the literature of women's use of nicotine replacement therapy (NRT) during pregnancy. The review will examine NRT and nicotine, interventions and barriers to smoking cessation and the literature exploring women's views on using NRT in pregnancy.
METHOD:
a structured review of the literature was undertaken by accessing internet and library sources, restricted to English written articles between 1989 and 2003. Keywords were used to refine the search: pregnancy, smoking, NRT and smoking cessation.
FINDINGS:
several papers had examined interventions employed to aid pregnant women in smoking cessation and the efficacy of NRT. However, papers exploring the views of women on the use of NRT during pregnancy were limited.
CONCLUSIONS:
smoking cessation is unique to individual women; as such, appropriate support and advice should be tailored accordingly. Further research is needed to explore the views of women using NRT during pregnancy to aid smoking cessation.
IMPLICATIONS FOR PRACTICE:
the findings from this structured review raise awareness of the importance of smoking cessation during pregnancy, and the inclusion of women's views when discussing the use of NRT.
