Le sevrage est difficile, car la nicotine contenue dans la fumée de tabac induit une dépendance. L’aide au sevrage, pour être efficace, devra donc comporter un volet de soutien comportemental et un volet pharmacologique. La substitution nicotinique a été durant des années le premier choix pour aider le sevrage des fumeurs. De très nombreuses études et méta-analyses ont montré l’efficacité de cette méthode pour l’aide au sevrage tabagique. Les avantages et inconvénients des différentes formes sont bien connus des médecins et tabacologues. Ce mémoire étudie quelques médicaments non nicotiniques utilisés pour l’aide au sevrage, apparus ou remis à jour récemment : ils agissent en contrecarrant les effets de la nicotine au niveau du système nerveux central (bupropion, nortryptiline, varénicline) ou en périphérie (vaccin anti-nicotine). Dans l’ensemble, les médications actuellement disponibles multiplient par 2 ou 3 la probabilité de sevrage à long terme (6 à 12 mois), ce qui est un résultat important, étant donné le grand nombre de fumeurs souhaitant arrêter leurs habitudes tabagiques. Cependant, le taux de sevrage global à long terme ne dépasse pas 30 à 40 %, soulignant la nécessité d’efforts et de progrès soutenus dans les années à venir. Exposure to cigarette smoke is the greatest risk factor for COPD in the developed world. Smoking cessation improves respiratory symptoms and lung function and reduces mortality among patients with COPD. Cigarette smokers with COPD and other co-morbid conditions such as cardiovascular disease and psychiatric illnesses should receive comprehensive tobacco treatment interventions incorporating efficacious pharmacotherapies. Varenicline, an alpha(4)beta(2) nicotinic acetylcholine receptor partial agonist, is the newest and most effective drug currently available to promote smoking cessation. In conjunction with behavioral interventions and clinical monitoring for potential side effects, varenicline offers great hope for reducing smoking-attributable death and disability. Trials of antidepressant medications for smoking cessation have had mixed results. We conducted a double-blind, placebo-controlled trial of a sustained-release form of bupropion for smoking cessation. We excluded smokers with current depression, but not those with a history of major depression. The 615 subjects were randomly assigned to receive placebo or bupropion at a dose of 100, 150, or 300 mg per day for seven weeks. The target quitting date (or "target quit date") was one week after the beginning of treatment. Brief counseling was provided at base line, weekly during treatment, and at 8, 12, 26, and 52 weeks. Self-reported abstinence was confirmed by a carbon monoxide concentration in expired air of 10 ppm or less.
RESULTS:
At the end of seven weeks of treatment, the rates of smoking cessation as confirmed by carbon monoxide measurements were 19.0 percent in the placebo group, 28.8 percent in the 100-mg group, 38.6 percent in the 150-mg group, and 44.2 percent in the 300-mg group (P<0.001). At one year the respective rates were 12.4 percent, 19.6 percent, 22.9 percent, and 23.1 percent. The rates for the 150-mg group (P=0.02) and the 300-mg group (P=0.01) -- but not the 100-mg group (P=0.09) -- were significantly better than those for the placebo group. Among the subjects who were continuously abstinent through the end of treatment, the mean absolute weight gain was inversely associated with the dose (a gain of 2.9 kg in the placebo group, 2.3 kg in 100-mg and 150-mg groups, and 1.5 kg in the 300-mg group; P= 0.02). No effects of treatment were observed on depression scores as measured serially by the Beck Depression Inventory. Thirty-seven subjects stopped treatment prematurely because of adverse events; the frequency was similar among all groups.
CONCLUSIONS:
A sustained-release form of bupropion was effective for smoking cessation and was accompanied by reduced weight gain and minimal side effects. Electronic cigarettes (e-cigarettes) are often promoted to assist with cigarette smoking cessation. In 2016–2017, the relationship between e-cigarette use and having stopped smoking among ever (current and former) smokers was assessed in the European Union and Great Britain by itself.
Methods:
Cross-sectional logistic regression of the association between being a former smoker and e-cigarette use was applied to the 2014 Eurobarometer survey of 28 European Union countries controlling for demographics.
Results:
Among all ever smokers, any regular ever use of nicotine e-cigarettes was associated with
lower odds of being a former smoker (unadjusted OR¼0.34, 95% CI¼0.26, 0.43, AOR¼0.43, 95% CI¼0.32, 0.58) compared with smokers who had never used e-cigarettes. In unadjusted models, daily use (OR¼0.42, 95% CI¼0.31, 0.56); occasional use (OR¼0.25, 95% CI¼0.18, 0.35); and experimentation (OR¼0.24, 95% CI¼0.19, 0.30) of nicotine e-cigarettes were associated with lower odds of being a former smoker compared with having never used nicotine-containing e-cigarettes. Comparable results were found in adjusted models. Results were similar in Great Britain alone. Among current smokers, daily cigarette consumption was 15.6 cigarettes/day (95% CI¼14.5, 16.7) among those who also used e-cigarettes versus 14.4 cigarettes/day (95% CI¼13.4, 15.4) for those who did not use them (po0.05).
Conclusions:
These results that e-cigarettes are associated with inhibiting rather than assisting in smoking cessation. On the population level, the net effect of the entry of e-cigarettes into the European Union (and Great Britain) is associated with depressed smoking cessation of conventional cigarettes.

Smoking cessation is a key component of secondary cardiovascular disease prevention. Varenicline, a partial alpha4beta2 nicotinic acetylcholine receptor agonist, is effective for smoking cessation in healthy smokers, but its efficacy and safety in smokers with cardiovascular disease are unknown.
METHODS AND RESULTS:
A multicenter, randomized, double-blind, placebo-controlled trial compared the efficacy and safety of varenicline with placebo for smoking cessation in 714 smokers with stable cardiovascular disease. Participants received varenicline (1 mg twice daily) or placebo, along with smoking-cessation counseling, for 12 weeks. Follow-up lasted 52 weeks. The primary end point was carbon monoxide-confirmed continuous abstinence rate for weeks 9 through 12 (last 4 weeks of treatment). The continuous abstinence rate was higher for varenicline than placebo during weeks 9 through 12 (47.0% versus 13.9%; odds ratio, 6.11; 95% confidence interval [CI], 4.18 to 8.93) and weeks 9 through 52 (19.2% versus 7.2%; odds ratio, 3.14; 95% CI, 1.93 to 5.11). The varenicline and placebo groups did not differ significantly in cardiovascular mortality (0.3% versus 0.6%; difference, -0.3%; 95% CI, -1.3 to 0.7), all-cause mortality (0.6% versus 1.4%; difference, -0.8%; 95% CI, -2.3 to 0.6), cardiovascular events (7.1% versus 5.7%; difference, 1.4%; 95% CI, -2.3 to 5.0), or serious adverse events (6.5% and 6.0%; difference, 0.5%; 95% CI, -3.1 to 4.1). As a result of adverse events, 9.6% of varenicline and 4.3% of placebo participants discontinued study drug.
CONCLUSIONS:
Varenicline is effective for smoking cessation in smokers with cardiovascular disease. CONTEXTE:
Étant donné que le maintien du tabagisme après le diagnostic d'un cancer augmente le risque d'effets néfastes sur la santé, il est vivement conseillé aux patients atteints d'un cancer de cesser de fumer. Malgré le manque actuel de preuves quant à leur innocuité et à leur efficacité en tant qu'outil de cessation, les cigarettes électroniques (cigarettes électroniques) deviennent de plus en plus populaires. Pour orienter la communication des oncologues avec leurs patients sur l'utilisation de la cigarette électronique, cet article fournit aux auteurs les connaissances les plus récentes qui soient sur les premières données cliniques publiées concernant l'utilisation de la cigarette électronique et les résultats de l'arrêt du tabac chez les patients atteints de cancer.
MÉTHODES:
Parmi les 1074 participants, figuraient des fumeurs (patients atteints de cancer) récemment inscrits à un programme de traitement du tabac dans un centre de cancérologie intégré. Les résultats standard démographiques, les antécédents de consommation de tabac et l'abandon du tabac ont été évalués.
RÉSULTATS:
Une augmentation de 3 fois la consommation de cigarettes électroniques a été observée de 2012 à 2013 (10,6% contre 38,5%). Les utilisateurs de cigarettes électroniques étaient davantage dépendants de la nicotine que les non-utilisateurs, avaient eu plus de tentatives d'arrêter de fumer et étaient plus susceptibles de recevoir un diagnostic de cancer thoracique et de cancer de la tête ou du cou. En utilisant une analyse de cas complète, les utilisateurs de cigarettes électroniques étaient aussi susceptibles de fumer au moment du suivi que les non-utilisateurs (odds ratio, 1,0; intervalle de confiance à 95%, 0,5 à 1,7). En utilisant une analyse en intention de traiter, les utilisateurs de cigarettes électroniques étaient deux fois plus susceptibles de fumer au moment du suivi que les non-utilisateurs (odds ratio, 2,0; intervalle de confiance à 95%, 1,2-3,3).
CONCLUSIONS:
Le taux élevé d'utilisation de la cigarette électronique observé est cohérent avec les articles récents mettant en évidence une augmentation de l'utilisation de la cigarette électronique dans la population en général. Les résultats longitudinaux actuels soulèvent des doutes quant à l'utilité des cigarettes électroniques pour faciliter l'abandon du tabac chez les patients atteints de cancer. It has been reported that adult smokers (AS) may be considering smokeless tobacco products as an alternative to smoking. The objective of this study was to evaluate the change in exposure in AS using Marlboro snus (MSNUS) (a tobacco pouch product in test market in June 2007).
METHODS:
AS were randomized into the following groups--CS: subjects (n = 30) continue smoking their own brand; DU: subjects (n = 60) reduced their daily cigarette consumption by >or=50% and were allowed to use MSNUS; SN: subjects (n = 15) stopped smoking their cigarettes but were allowed to use MSNUS; NT: subjects (n = 15) were not allowed to use any tobacco products for the entire duration of the 8-day study. Biomarkers of smoke exposure (BOE) measured at baseline and postbaseline were 24-hr urinary excretion of metabolites of N-nitrosamines, nicotine (urine and plasma), aromatic amines, benzene, and polycyclic aromatic hydrocarbon; urine mutagenicity; and carboxyhemoglobin at various timepoints.
RESULTS:
Statistically significant (p < .05) reductions in all the urinary BOE were observed in the DU group compared with the CS group. After correcting for the residual effect, a proportionate reduction (approximately 50%) in most of the biomarkers was observed. Even larger reductions, similar to the NT group, were observed in the SN group.
DISCUSSION:
The proportionate reduction in exposure when reducing the number of cigarettes by 50% and using MSNUS, under the consumption patterns observed, suggest that the AS did not appear to alter their smoking behavior. The added exposure from MSNUS usage in this group was minimal. In the circulation of all species, the majority of drug-related material was composed of unchanged varenicline. In all four species, drug-related material was primarily excreted in the urine. A large percentage was excreted as unchanged parent drug (90, 84, 75, and 81% of the dose in mouse, rat, monkey, and human, respectively). Metabolites observed in excreta arose via N-carbamoyl glucuronidation and oxidation. These metabolites were also observed in the circulation, in addition to metabolites that arose via N-formylation and formation of a novel hexose conjugate. Experiments were conducted using in vitro systems to gain an understanding of the enzymes involved in the formation of the N-carbamoylglucuronide metabolite in humans. N-Carbamoyl glucuronidation was catalyzed by UGT2B7 in human liver microsomes when incubations were conducted under a CO2 atmosphere. The straightforward dispositional profile of varenicline should simplify its use in the clinic as an aid in smoking cessation. Effective smoking cessation treatments are needed for patients with schizophrenia, who, compared with the general population, have high rates of cigarette smoking and more difficulty quitting. We evaluated the safety and efficacy of varenicline for smoking cessation in outpatients with stable schizophrenia or schizoaffective disorder.
METHOD:
In this 12-week, randomized, double-blind, multicenter trial (May 8, 2008, to April 1, 2010), 127 smokers (≥ 15 cigarettes/d) with DSM-IV-confirmed schizophrenia or schizoaffective disorder received varenicline or placebo (2:1 ratio). The primary outcome was safety and tolerability of varenicline assessed by adverse events frequency and changes in ratings on the Positive and Negative Syndrome Scale and other psychiatric scales from baseline to 24 weeks. Abstinence was defined as no smoking 7 days prior to weeks 12 and 24, verified by carbon monoxide level.
RESULTS:
Eighty-four participants received varenicline; 43, placebo. At 12 weeks (end of treatment), 16/84 varenicline-treated patients (19.0%) met smoking cessation criteria versus 2/43 (4.7%) for placebo (P = .046). At 24 weeks, 10/84 (11.9%) varenicline-treated and 1/43 (2.3%) placebo-treated patients, respectively, met abstinence criteria (P = .090). Total adverse event rates were similar between groups, with no significant changes in symptoms of schizophrenia or in mood and anxiety ratings. Rates of suicidal ideation adverse events were 6.0% (varenicline) and 7.0% (placebo) (P = 1.0). There was 1 suicide attempt by a varenicline patient with a lifetime history of similar attempts and no completed suicides.
CONCLUSIONS:
Varenicline was well tolerated, with no evidence of exacerbation of symptoms, and was associated with significantly higher smoking cessation rates versus placebo at 12 weeks. Nicotine addiction is one of the most important health concerns for patients with schizophrenia. An extensive body of evidence points to a high prevalence rate of comorbid nicotine addiction in people with schizophrenia (70–90%), which contributes to significant cardiovascular and cancer risks in this vulnerable population. Therefore, effective smoking cessation strategies could play a major role in preventing significant morbidity and mortality in this population. Two of the most common pharmacological approaches to smoking cessation, bupropion and nicotine replacement therapy (NRT), have been used in psychiatric patients to reduce their smoking. In 2006, varenicline, a partial agonist of α4β2 acetylcholine receptor, was approved for smoking cessation by the FDA. This drug not only has the beneficial effects on withdrawal symptoms, but also reduces craving and rewarding effects of smoking. While varenicline has been shown to be an effective, safe medication for the general population, its efficacy and safety for subjects with schizophrenia is less well characterized. A number of case studies have prompted FDA warnings about the potential exacerbation of psychiatric symptoms. However, other case studies and pilot studies have shown varenicline to be a safe and effective treatment for smoking cessation in subjects with schizophrenia. Des événements cardiovasculaires indésirables associés à l'utilisation de la varénicline, un médicament anti-tabac largement utilisé, ont été rapportés après la commercialisation. Nous avons effectué une revue systématique et une méta-analyse d'essais contrôlés randomisés pour déterminer les effets cardiovasculaires indésirables graves de la varénicline par rapport au placebo chez les consommateurs de tabac.
MÉTHODES:
Nous avons cherché dans MEDLINE, EMBASE, la base de données de revues systématiques Cochrane, les sites Web d'autorités de réglementation et les registres d'essais cliniques, sans restriction de date ou de langue, jusqu'en septembre 2010 (mise à jour en mars 2011) pour les études publiées et non publiées. Nous avons sélectionné des essais contrôlés randomisés en double aveugle d'une durée d'au moins une semaine impliquant des fumeurs ou des personnes ayant consommé du tabac sans fumée et rapportant des événements cardiovasculaires (ischémie, arythmie, insuffisance cardiaque congestive, mort subite ou décès d'origine cardiovasculaire) comme événements indésirables graves associés à l'utilisation de la varénicline.
RÉSULTATS:
Nous avons analysé les données de 14 essais contrôlés randomisés à double insu portant sur 8 216 participants. Les essais ont duré de 7 à 52 semaines. La varénicline était associée à un risque significativement accru d'événements cardiovasculaires indésirables graves par rapport au placebo (1,06% [52/4908] dans le groupe varénicline vs 0,82% [27/3308] dans le groupe placebo; rapport de cotes de Peto [OR] 1,72, 95% intervalle de confiance [IC] 1,09-2,71; I (2) = 0%). Les résultats de diverses analyses de sensibilité concordaient avec ceux de l'analyse principale et un graphique en entonnoir n'a révélé aucun biais de publication. Il y avait trop peu de décès pour permettre des comparaisons significatives de la mortalité.
INTERPRÉTATION:

Cette étude visait à évaluer le rapport coût-efficacité d'un cours de longue durée (12 12 semaines) de la varénicline (en utilisant les avantages de l'abandon du tabac sur les résultats) BENESCO modèle de la cessation du tabagisme.
Méthodes: Les données sur l'efficacité de 12 12 semaines un traitement à la varénicline dans aider le sevrage tabagique ont été analysés conjointement avec les données d'efficacité pendant 12 semaines de la varénicline, le bupropion et le placebo étaient auparavant inclus dans le modèle BENESCO, en utilisant une comparaison mixte de traitement. Le coût moyen de cours de 12 12 semaines de la varénicline a été estimé à $ 603,89, sur la base d'un cours de 12 semaines suivie par une période de 12 semaines pour les ex-fumeurs. Cours de la vie de tous les sujets, 12 12 semaines de varénicline est moins coûteux et plus efficace que (domine) toutes les autres stratégies par rapport au modèle BENESCO mise à jour, à l'exception de 12 semaines de la varénicline. Dans cette comparaison, 12 12 semaines de varénicline est une alternative très rentable pour le cours de 12 semaines, avec un coût supplémentaire de moins de 1000 $ par année de vie ajustée sur la qualité (QALY) gagnée.
CONCLUSIONS:

AIM:
To investigate whether varenicline use was associated with increased risk of psychiatric adverse events, compared with bupropion, another drug used for smoking cessation.
DESIGN, SETTING AND PARTICIPANTS:
We conducted a registry-based cohort study in Denmark, 2007-10, comparing new users of varenicline and bupropion in unmatched and 1 : 1 propensity score-matched analyses.
MEASUREMENTS:
Using Cox regression, we estimated the hazard ratio (HR) of any psychiatric adverse event (emergency department visit or in-patient admission with a psychiatric diagnosis) within 30 days following treatment initiation. The unmatched and matched analyses correspond to conventional crude and fully adjusted analyses, respectively.
FINDINGS:
In unmatched analyses, there were 106 (0.18%) psychiatric adverse events among 59 790 varenicline users (rate 22 events per 1000 person-years), compared with 46 (0.26%) events among 17 936 bupropion users (rate 31 per 1000); the HR was 0.69 [95% confidence interval (CI): 0.49-0.98]. In propensity score-matched analyses, 39 (0.22%) events occurred among 17 935 varenicline users (rate 27 per 1000), compared with 46 (0.26%) events among 17 935 bupropion users (rate 31 per 1000); varenicline was not associated with increased risk of psychiatric adverse events (HR 0.85, 95% CI: 0.55-1.30). The overall rate of psychiatric adverse events was substantially higher among participants with a history of psychiatric disorder than in patients without such history; the risk associated with varenicline did not differ significantly by history of psychiatric disorder.
CONCLUSIONS:

Smoking rates are up to five times higher in people with schizophrenia than in the general population, placing these individuals at high risk for smoking-related health problems. Varenicline, an α4β2 nicotinic acetylcholine receptor partial agonist, is a promising aid for smoking cessation in this population. To maximize treatment efficacy while minimizing risks, it is critical to identify reliable predictors of positive response to varenicline in smokers with schizophrenia.
OBJECTIVES:
Negative symptoms of schizophrenia are related to dysfunctions in the brain reward system, are associated with nicotine dependence, and may be improved by nicotine or nicotinic receptor agonists, suggesting that smoking cessation may be especially difficult for patients with substantial negative symptoms. The purpose of the study was to evaluate negative symptoms as predictors of response to varenicline.
METHODS:
Patients with schizophrenia (N = 53) completed a 12-week smoking cessation trial combining varenicline with cognitive behavioral therapy. Negative symptoms were assessed via the Scale for the Assessment of Negative Symptoms (Andreasen 1983). Outcomes included smoking abstinence as assessed by self-report and expired carbon monoxide. Change in performance on a probabilistic reward task was used as an index of change in reward sensitivity during treatment.
RESULTS:
At week 12, 32 participants met criteria for 14-day point-prevalence abstinence. Patients with lower baseline symptoms of affective flattening (more typical affect) were more likely to achieve smoking abstinence and demonstrated larger increases in reward sensitivity during treatment.
CONCLUSIONS:
