Cinquante fumeurs ont été recrutés dans un établissement de soins primaires de la communauté du Midwest, desservant une population non assurée, sous-assurée et Medicaid. Les participantes ont rempli un questionnaire avant l'intervention et onze femmes ont choisi de participer aux sessions de six semaines. Il y avait peu de différences entre le groupe de contrôle (non participants) et le groupe d'intervention. Le groupe d'intervention avait des scores significativement plus élevés d'intention de quitter. L'analyse de régression logistique a été utilisée pour prédire l'appartenance à un groupe avec l'âge, l'auto-efficacité, l'optimisme, le soutien social, la dépendance à la nicotine, l'intention de cesser de fumer et les autres fumeurs à la maison comme variables prédictives. Les deux seuls prédicteurs significatifs de la participation à l'intervention de sevrage tabagique étaient l'auto-efficacité et l'intention de cesser de fumer. Plus le score d'intention de cesser de fumer est élevé et plus le score d'auto-efficacité est bas (la conviction qu'il est possible de réussir à ne pas fumer); plus ces femmes étaient susceptibles de participer à l’intervention de groupe. Fifty smokers were recruited from a Midwestern community primary care setting serving an uninsured, underinsured, and Medicaid population. Participants completed a questionnaire before the intervention, and eleven women chose to participate in the
6-week sessions. There were few differences between the control group (nonparticipants) and the intervention group. The intervention group had significantly higher intention-to-quit scores. Logistic regression analysis was used to predict group membership with age, self-efficacy, optimism, social support, nicotine dependence, intention-to-quit, and other smokers in the home as the predictor variables. The only two significant predictors of participation in the cessation intervention were self-efficacy and intention-to-quit smoking. The higher the intention-to-quit score and the lower the self-efficacy score (the belief that one can be successful in not smoking); the more likely these women were to participate in the group intervention. [TABAC] prévention:évaluationIndex. décimale : TA 0.4 Évaluations, enquêtes et questionnaire Résumé : Le rapport ' Lifting the SmokeScreen, 10 reasons for Smoke Free Europe ' a été lancé en 2004 et publié en février 2006 sous l’égide de l’European Respiratory Society, de Cancer Research UK, de l’European Heart Network et de l’Institut National du Cancer. Pour la France, il dénombre 5863 décès (au mort près !) par exposition au tabagisme passif. Cela fait plus que doubler les évaluations précédentes les plus pessimistes. Ce 'travail' a ainsi justifié les mesures draconiennes d’interdiction de fumer en Europe. Les estimations pour tous le pays européens sont à l’unisson. Un 'détail' devrait cependant,avoir soulevé un tollé dans les milieux scientifiques. Le rapport a insidieusement changé la définition du tabagisme dit passif (ou involontaire, ou environnemental) en cours depuis 30 ans. C’était le risque pris par les non-fumeurs inhalant involontairement la fumée des autres. Sous prétexte qu’il est difficile de fumer loin de soi-même, un fumeur inhale fatalement la fumée de la pièce où il fume. Le rapport inclut donc parmi les victimes du tabagisme passif une majorité de fumeurs actifs, dont il calcule ainsi la part 'passive' du risque ! Quant aux vrais non-fumeurs, le rapport les estime à 1114 ! Encore calcule-t-il le nombre de non-fumeurs en retranchant de la population adulte le nombre de fumeurs, évalué selon les pourcentages donnés par quelques sondages. Il inclut donc parmi les non-fumeurs une bonne proportion d’anciens fumeurs, dont le risque ne s’annule pas du jour au lendemain (le risque de cancer est encore doublé 10 ans après l’arrêt). Mais selon le lieu d’exposition, 1007 décès seraient liés à l’exposition à domicile au tabagisme du conjoint. Tant que des policiers n’auront pas le droit d’enfoncer les portes des appartements pour verbaliser le délinquant, les législations restrictives n’ont guère de chances de limiter ce risque. Restent 107 expositions au travail, dont 6 dans le groupe des hôtels, restaurants, bars et discothèques qui ont fait l’objet de la dernières vague d’interdictions. On croit rêver en entendant affirmer sans vergogne que l’interdiction de fumer dans ces lieux aurait provoqué en janvier dernier une baisse spectaculaire de 17% des hospitalisations pour infarctus ! Parmi les obstétriciens et les sages-femmes, évaluer le pourcentage de fumeurs, apprécier la proportion des soignants qui évoquent le tabagisme lors de la surveillance d'une grossesse, la qualité de l'information apportée aux patientes, ainsi que leur expérience en tabacologie.
Patientes et méthodes
L'échantillon de 188 personnes était constitué des gynécologues–obstétriciens, sages-femmes et élèves sages-femmes du centre hospitalo-universitaire (CHU) et des établissements privés de Limoges ainsi que des centres hospitaliers généraux du Limousin. Un questionnaire postal leur a été envoyé en février et mars 2002.
Résultats
Cent quarante-deux réponses ont été obtenues (taux de réponse 75,5 %). L'âge moyen des répondants était de 37 ans (20–59 ans) avec un tabagisme actif de 43,6 %. La question du tabagisme pergravidique et de ses risques est abordée par la grande majorité des soignants. Le conseil minimal est connu et prodigué par 34,4 % des gynécologues et 26,5 % des sages-femmes. Soixante-deux pour cent des gynécologues et 79 % des sages femmes n'ont jamais proposé à leurs patientes de consultation de tabacologie. Soixante-dix pour cent des personnes interrogées n'ont pas reçu de formation spécifique en tabacologie durant leurs études.
Discussion et conclusion
La prise en charge du tabagisme pergravidique semble être insuffisante en milieu gynécologique en Limousin. This paper reports the program outcomes based on pretest, six-month, and one year follow-up surveys. Methods: An 8 session classroom-based curriculum was tested with a clustered randomized controlled trial that involved a total of 1097 students in 6 program and 6 control alternative high schools. Weekly and monthly smoking was assessed at the three time points. Outcome effects were analyzed with multi-level random coefficients models. Results: Students in the programcondition experienced a greater reduction in weekly smoking and monthly smoking, at 6-and-12-month follow-ups. The net change varied between -5.1% and -7.6%, comparing the programcondition to the control condition. Conclusions: The implementation of Project EX in a classroom setting produced decreases in smoking among students in the program, relative to those in the standard care control condition. It is likely that a classroom-based smoking prevention/cessation program can lead to lower overall smoking prevalence than a cessation program that is implemented in a school-based smoking cessation clinic format. Alcohol was significantly correlated with endurance capacity among joggers in univariate analysis, but lost its significance in multivariate analysis. Among army conscripts, the distance covered in a 12-min endurance run was inversely related to daily cigarette consumption and years of smoking (P less than 0.001). This association was apparent even among light smokers who had been smoking for less than 2 years when they were compared with nonsmokers. Among joggers, even when the lower training activity of the smokers was controlled for, smoking retained a negative, independent association with endurance capacity, as measured by 16-km race time. In multiple regression analysis of endurance capacity, the standardized regression coefficient for smoking was -0.14 for distance covered in the 12-min run and 0.10 for 16-km running time, the latter despite the low prevalence (6.9%) of regular cigarette smokers among the joggers. Seventy percent of the 16-km race participants who smoked around the time they took up jogging quit smoking as joggers. Within the limits of this cross-sectional study design, these results suggest that smoking exerts a direct, biologically mediated, deleterious effect on endurance capacity. The lower levels of exercise of the smokers did not entirely explain the difference in endurance between smokers and nonsmokers. This observation of a short-term negative association between even light smoking and endurance capacity may have implications for health education and promotion efforts. In total, 65 questionnaires were completed (52.42% return rate). Health visitors, midwives and practice nurses were aware of the reasons why pregnant women smoke as well as the health risks associated with smoking. All claimed to intervene by asking if their patients smoked. Most respondents claimed to record smoking status on health records and to give cessation advice to their patients. However, fewer of the health professionals provided advice to partners of women in their care and only a minority had read the NICE clinical guidelines on nicotine replacement therapies. There were limitations to the health professionals’ attitudes, knowledge and practice that could affect the quality of smoking cessation service offered to patients. To examine usage rates of smoking-cessation assistance and to compare the success rate of those who used assistance with the success rate of those who did not.
Methods:
The data come from the 1996 California Tobacco Survey, a random sample of 4480 individuals (18 years or older) who tried to quit smoking in the 12 months before the
survey. We calculated population estimates for demographics, smoking histories, rate of using assistance, and abstinence rates.
Results:
One fifth (19.9%) of those who attempted to quit smoking used one or more forms of assistance: self-help, counseling, and/or nicotine replacement therapy (NRT). Heavy smokers were more likely to use assistance than were light smokers. Women were more likely to use assistance than were men, and usage increased with age. Whites were more likely to use NRT than were other ethnic groups. Overall, those who used assistance had a higher success rate than those who did not; the 12-month abstinence rates were 15.2% and 7.0%, respectively.
Conclusions:
Use of assistance for smoking cessation has increased over recent years, from 7.9% in 1986 to 19.9% in 1996. The use of assistance is associated with a greater success rate. Cette étude a consisté à montrer à des personnes 24 vidéos de 30 secondes montrant soit des éléments neutres (lecture d'un bouquin p.e.), soit un élément incitant au tabagisme (allumer une cigarette p.e.). to describe the qualitatively different ways in which midwives make sense of how to approach women smokers.
DESIGN, SETTING AND PARTICIPANTS:
a more person-centred national project 'Smoke-free pregnancy' has been in progress in Sweden since 1992. Using a phenomenographic approach, 24 midwives who have been regularly working in antenatal care were interviewed about addressing smoking during pregnancy.
FINDINGS:
four different story types of how the midwives made sense of their experiences in addressing smoking in pregnancy were identified: 'avoiding', 'informing', 'friend-making', 'co-operating'.
KEY CONCLUSION:
the midwives' story types about how they approached women who smoke illustrated the difficulties of changing from being an expert who gives information and advice to being an expert on how to enable a woman in finding out why she smoked and how to stop smoking.
IMPLICATIONS FOR PRACTICE:
health education about smoking that is built on co-operation and dialogue was seen by the midwives as a productive way of working.
