Les calculateurs de risque considèrent que les anciens fumeurs ne sont à risque que pendant 5 ans.
Objectif Évaluer l'association entre les années depuis l'arrêt du tabac et les MCV incidentes.
Conception, cadre et participants Analyse rétrospective des données collectées de manière prospective auprès des participants à l'étude Framingham Heart sans maladie cardiovasculaire initiale (cohorte d'origine : ayant participé à leur quatrième examen en 1954-1958 ; cohorte de progéniture : ayant participé à leur premier examen en 1971-1975) qui ont été suivis jusqu'à décembre 2015.
Expositions Statut tabagique autodéclaré actualisé dans le temps, nombre d'années depuis l'arrêt et nombre d'années-pack cumulé.
Principaux résultats et mesures Incident MCV (infarctus du myocarde, accident vasculaire cérébral, insuffisance cardiaque ou décès cardiovasculaire). Les analyses primaires comprenaient les deux cohortes (regroupées) et se limitaient aux gros fumeurs (≥20 paquets-années).
Résultats La population étudiée comprenait 8 770 individus (cohorte d'origine : n = 3805 ; cohorte de progéniture : n = 4965) avec un âge moyen de 42,2 (SD : 11,8) ans et 45 % d'hommes. Il y avait 5 308 fumeurs ayant déjà fumé avec une médiane de 17,2 (intervalle interquartile, 7-30) pack-années de base, dont 2 371 gros fumeurs (406 [17 %] anciens et 1965 [83 %] actuels). Sur 26,4 années de suivi médian, 2435 premiers événements cardiovasculaires se sont produits (cohorte d'origine : n = 1612 [n = 665 chez les gros fumeurs] ; cohorte des descendants : n = 823 [n = 430 chez les gros fumeurs]). Dans la cohorte regroupée, par rapport au tabagisme actuel, l'arrêt dans les 5 ans était associé à des taux significativement plus faibles de MCV incidentes (taux d'incidence pour 1 000 années-personnes : tabagisme actuel, 11,56 [IC à 95 %, 10,30-12,98] ; arrêt dans les 5 ans , 6,94 [IC à 95 %, 5,61-8,59] ; différence, -4,51 [IC à 95 %, -5,90 à -2. 77]) et un risque plus faible de MCV incidente (rapport de risque, 0,61 ; IC à 95 %, 0,49-0,76). Comparativement au fait de ne jamais fumer, l'arrêt du tabac a cessé d'être significativement associé à un plus grand risque de MCV entre 10 et 15 ans après l'arrêt dans la cohorte regroupée (taux d'incidence pour 1000 personnes-années : n'ayant jamais fumé, 5,09 [IC à 95 % : 4,52-5,74] ; arrêt dans les 10 à <15 ans, 6,31 [IC à 95 %, 4,93-8,09] ; différence, 1,27 [IC à 95 %, -0,10 à 3,05] ; hazard ratio, 1,25 [IC à 95 %, 0,98 à 1,60]). 10 à 3,05] ; risque relatif, 1,25 [IC à 95 %, 0,98-1,60]). 10 à 3,05] ; risque relatif, 1,25 [IC à 95 %, 0,98-1,60]).
Conclusions et pertinence Parmi les gros fumeurs, l'arrêt du tabac était associé à un risque significativement plus faible de MCV dans les 5 ans par rapport aux fumeurs actuels. Pictorial warnings on cigarette packs draw attention and increase quit intentions, but their effect on smoking behavior remains uncertain.
OBJECTIVE
To assess the effect of adding pictorial warnings to the front and back of cigarette packs.
DESIGN, SETTING, AND PARTICIPANTS
This 4-week between-participant randomized clinical trial was carried out in California and North Carolina.We recruited a convenience sample of adult cigarette smokers from the general population beginning September 2014 through August 2015. Of 2149 smokers who enrolled, 88%completed the trial. No participants withdrew owing to adverse events.
INTERVENTIONS
We randomly assigned participants to receive on their cigarette packs for 4 weeks either text-only warnings (one of the Surgeon General’s warnings currently in use in the United States on the side of the cigarette packs) or pictorial warnings (one of the Family Smoking Prevention and Tobacco Control Act’s required text warnings and pictures that showed harms of smoking on the top half of the front and back of the cigarette packs).
MAIN OUTCOMES AND MEASURES
The primary trial outcomewas attempting to quit smoking during the study.We hypothesized that smokers randomized to receive pictorial warnings would be more likely to report a quit attempt during the study than smokers randomized to
receive a text-only Surgeon General’s warning.
RESULTS
Of the 2149 participants who began the trial (1039 men, 1060 women, and 34 transgender people; mean [SD] age, 39.7 [13.4] years for text-only warning, 39.8 [13.7] for pictorial warnings), 1901 completed it. In intent-to-treat analyses (n = 2149), smokers whose packs had pictorial warnings were more likely than those whose packs had text-only warnings to attempt to quit smoking during the 4-week trial (40% vs 34%; odds ratio [OR], 1.29; 95% CI, 1.09-1.54). The findings did not differ across any demographic groups. Having quit smoking for at least the 7 days prior to the end of the trial was more common among smokers who received pictorial than those who received text-only warnings (5.7%vs 3.8%; OR, 1.53; 95%CI, 1.02-2.29). Pictorial warnings also increased forgoing a cigarette, intentions to quit
smoking, negative emotional reactions, thinking about the harms of smoking, and conversations about quitting.
CONCLUSIONS AND RELEVANCE
Pictorialwarnings effectively increased intentions to quit, forgoing cigarettes, quit attempts, and successfully quitting smoking over 4 weeks. Our trial
findings suggest that implementing pictorial warnings on cigarette packs in the United States would discourage smoking.

BACKGROUND:
The selective nicotinic acetylcholine receptor partial agonist, varenicline tartrate, represents a novel type of therapy for smoking cessation. This study evaluated the efficacy, safety, and tolerability of 4 varenicline dose regimens, 2 with progressive dosing over the first week (eg, titrated) and 2 with a fixed dosing schedule (eg, non-titrated), for promoting smoking cessation.
METHODS:
This multicenter, double-blind, placebo-controlled study randomized healthy smokers (aged 18-65 years) to varenicline tartrate, 0.5 mg twice daily nontitrated (n = 129), 0.5 mg twice daily titrated (n = 130), 1.0 mg twice daily nontitrated (n = 129), 1.0 mg twice daily titrated (n = 130), or placebo (n = 129) for 12 weeks to aid in smoking cessation. A 40-week follow-up period assessed long-term efficacy. The primary efficacy measures were the carbon monoxide-confirmed 4-week continuous quit rates by pooled dosage group for weeks 4 through 7 and 9 through 12 and the continuous abstinence rates for weeks 9 through 52.
RESULTS:
Weeks 9 through 12 continuous quit rates were greater in the 1.0-mg group (49.4%) and the 0.5-mg group (44.0%) vs placebo (11.6%; P<.001 vs both doses). Weeks 9 through 52 abstinence rates were greater in the 1.0-mg group (22.4%; P<.001) and the 0.5-mg group (18.5%; P<.001) vs placebo (3.9%). Varenicline was generally well tolerated, with nausea occurring in 16% to 42% of varenicline-treated subjects. A large literature links both prenatal maternal smoking and children's ETS exposure to decreased lung growth and increased rates of respiratory tract infections, otitis media, and childhood asthma, with the severity of these problems increasing with increased exposure. Sudden infant death syndrome, behavioral problems, neurocognitive decrements, and increased rates of adolescent smoking also are associated with such exposures. Studies of each of these problems suggest independent effects of both pre- and postnatal exposure for each, with the respiratory risk associated with parental smoking seeming to be greatest during fetal development and the first several years of life. [TABAC] tabagismeIndex. décimale : TA 6.2.3 Approche pharmacologique Résumé : Since the 1996 publication of guidelines on smoking cessation from the Agency for Health Care Policy and Research and the American Psychiatric Association, several new treatments have become available, including nicotine nasal spray, nicotine inhaler, and bupropion hydrochloride. In addition, nicotine gum and patch have become available over-the-counter. This article reviews the published literature and US Food and Drug Administration and pharmaceutical company reports on these therapies. Based on this review, clinical logic, and experience, we conclude that pharmacotherapy should be made available to all smokers. All currently available therapies appear to be equally efficacious, approximately doubling the quit rate compared with placebo. Concomitant behavioral or supportive therapy increases quit rates and should be encouraged but not required. Combining patch with gum or patch with bupropion may increase the quit rate compared with any single treatment. Because patient characteristics predictive of success with a particular therapy are not yet known, the best treatment choice for an individual patient should be guided by the patient's past experience and preference and the product's adverse effect profile. Les participants ont été appariés pour leur âge et plusieurs dimensions des antécédents de tabagisme ont été déterminées à l'aide d'un ensemble de questionnaires standardisés (y compris l'âge au début et à la fin du tabac et la quantité moyenne de cigarettes, de cigares ou de pipe fumés par jour). Les antécédents professionnels et les antécédents de maladie pulmonaire antérieure de chaque individu ont été capturés pour contrôler les facteurs de confusion possibles. Les enquêteurs ont découvert que les personnes atteintes d'un cancer du poumon avaient des antécédents de tabagisme plus nombreux que les personnes sans cancer du poumon.
