Background
Health warning labels (HWLs) represent an evidence-based tobacco control strategy; however, their
application to heated tobacco products (HTPs) and related impacts are understudied. This study examined the impact of HTP HWLs on HTP use intentions and risk perceptions among current tobacco users.
Methods
We analyzed cross-sectional survey data from adults in the US and Israel reporting past-month tobacco use and awareness of HTPs (N = 424). Multivariate analyses examined: (1) sociodemographics in relation to self-reported impact of HTP HWLs (i.e., more concerned about HTP use, reassured, no effect [referent]) among those who noticed HTP HWLs (multinomial regressions); and (2) HWL impacts in relation to HTP use intentions and perceived addictiveness and harm (linear regressions).
Results Among participants who noticed HTP HWLs (n = 372, 87.7%), 27.7% reported HWLs increased their concerns about HTP use, 22.6% were reassured about use, and 49.7% reported no effect. Factors associated with increased concern (vs. no effect) included other tobacco product use (aOR = 2.10, 95% CI 1.21–3.64) and being female (aOR = 1.77, 95% CI 1.03–3.05). Factors associated with being reassured about HTPs use (vs. no effect) included current HTP use (aOR = 2.11, 95% CI 1.11–4.00) and being from Israel (vs. US: aOR = 3.85, 95% CI 1.85–7.69), female (aOR = 1.91, 95% CI 1.07–3.42), and less educated (< college education: aOR = 2.57, 95% CI 1.42–4.63). Reporting that HWLs on HTPs increased concern (β = 0.46, 95% CI 0.03–0.89) and reassured of use (β = 0.94, 95% CI 0.47–1.41) were positively associated with HTP use intentions; no associations with risk perceptions were found.
Conclusions
Findings indicate that most tobacco users noticed HWLs on HTPs, but the majority reported no effect or being reassured of using HTPs, effects that were magnified for specific subgroups. Both increased concern and reassurance correlated with greater use intentions. Additional research should evaluate HTP HWL impacts and ensure effectiveness in communicating risks and discouraging use. |

Rationale: Growing proportions of smokers in the USA do not smoke everyday and can be referred to as light and intermittent smokers (LITS). Despite a current prevalence of LITS in the USA estimated at 25-33% of all smokers, a systematic review of the literature on this group of smokers has yet to be written.
Objectives: The aim of this paper is to review and evaluate research on LITS and to identify, describe and discuss commonalities and differences between LITS and daily smokers.
Methods: The primary databases used to search for publications were Pub Med (National Library of Medicine) and SCOPUS (Elsevier).
Results: LITS inhale smoke and have post-smoking blood nicotine concentrations that are broadly equivalent to those found in daily smokers. However, LITS differ from daily smokers with regard to cigarette consumption and frequency of cigarette use, sociodemographic and socioeconomic characteristics, motives, personality traits, dependence, withdrawal and craving, response to smoking-related cues, quitting perception, past-smoking status, and initiation.
Conclusions: In contrast to daily smokers, LITS show few or no signs of dependence as currently defined by DSM-IV criteria, appear to exercise more self-control, seem to be less impulsive, and their smoking experience is primarily associated with positive rather than negative reinforcement. Conclusions drawn from the reviewed literature highlight the multivariate factors that must be taken into account when defining LITS and emphasize the importance of further research on this increasing fraction of smokers. The potential implications of increased LITS prevalence on smoking-related disease risks remain to be thoroughly investigated. |

Background
We assessed longitudinal effects of e-cigarette use on respiratory symptoms in a nationally representative sample of US adults by combustible tobacco smoking status.
Methods
We analyzed Waves 4–5 public-use data from the Population Assessment of Tobacco and Health Study. Study sample included adult respondents who reported no diagnosis of respiratory diseases at Wave 4, and completed Waves 4–5 surveys with no missing data on analytic variables (N = 15,291). Outcome was a validated index of functionally important respiratory symptoms based on 7 wheezing/cough questions (range 0–9). An index score of �2 was defined as having important respiratory symptoms. Weighted lagged logistic regression models were performed to examine the association between e-cigarette use status at Wave 4 (former/current vs. never use) and important respiratory symptoms at Wave 5 by combustible tobacco smoking status (i.e., never/former/current smokers), adjusting for Wave 4 respiratory symptom index, sociodemographic characteristics, secondhand smoke exposure, body mass index, and chronic disease.
Results
Among current combustible tobacco smokers, e-cigarette use was associated with increased odds of reporting important respiratory symptoms (former e-cigarette use:
adjusted odds ratio [AOR] = 1.39, 95% confidence interval [CI]: 1.07–1.81; current e-cigarette use: AOR = 1.55, 95% CI: 1.17–2.06). Among former combustible tobacco smokers, former e-cigarette use (AOR = 1.51, 95% CI: 1.06–2.15)—but not current e-cigarette use (AOR = 1.59, 95% CI: 0.91–2.78)—was associated with increased odds of important respiratory symptoms. Among never combustible tobacco smokers, no significant association was detected between e-cigarette use and important respiratory symptoms (former ecigarette use: AOR = 1.62, 95% CI: 0.76–3.46; current e-cigarette use: AOR = 0.82, 95% CI: 0.27–2.56). |

Objectives.
o comprehensively catalog and review state e-cigarette delivery sales laws as well as capture their scope and dimensions.
Methods.
We conducted an in-depth review to determine whether states had at least 1 form of e-cigarette delivery sales law. We coded laws for 5 key policy domains: (1) delivery terminology used in laws, (2) age verification requirements, (3) packaging label requirements, (4) permit or registration requirements, and (5) fines and penalties for violations.
Results.
Overall, 34 states had e-cigarette delivery sales laws with varying scopes and dimensions. In 27 states, these laws required at least 1 form of age verification requirements. We identified mandatory packaging labels in 12 states, and 7 states where permits were required. There were considerable
differences among states on the scale of fines and penalties for violations.
Conclusions. Our findings reveal extensive heterogeneity in e-cigarette delivery sales laws among states, particularly regarding the scope and dimensions of these laws. |

Objectives. We explored whether various key stakeholders considered cost sharing with state telephone-based tobacco cessation quitlines, because including tobacco cessation services as part of the required essential health benefits is a new requirement of the Patient Protection and Affordable Care Act (ACA).
Methods. We analyzed qualitative data collected from interviews conducted in April and May of 2014 with representatives of state health departments, quitline service providers, health plans, and insurance brokers in 4 US states.
Results. State health departments varied in the strategies they considered the role their state quitline would play in meeting the ACA requirements. Health plans and insurance brokers referred to state quitlines because they were perceived as effective and free, but in 3 of the 4 states, the private stakeholder groups did not consider cost sharing.
Conclusions. If state health departments are going to initiate cost-sharing agreements with private insurance providers, then they will need to engage a broad array of stakeholders and will need to overcome the perception that state quitline services are free. |

Clinical practice guidelines and evidence-based reviews confirm the efficacy of tobacco dependence treatment for pregnant women. The purpose of this study was to examine tobacco dependence treatment practices among certified nurse-midwives who treat pregnant women who smoke. Midwives were surveyed to determine knowledge, perceptions, and beliefs about tobacco cessation treatment and to identify practice environmental factors that support treatment practices. Half of all midwives had not heard of the US Public Health Service Guidelines (5 A's) to assist smokers in cessation treatment. We found varying levels of adherence to the clinical practice guidelines. Nearly all midwives routinely ask, advise, and assess; while fewer encourage patients to set a quit date or discuss medication options (assist) and perform follow-up activities (arrange). Barriers significantly associated with clinical tobacco treatment practice are lack of training and competing priorities in the visit. One-office support, a system in place to provide smoking cessation information and resources, was associated with increased practice. In summary, midwives believe they should be providing tobacco dependence treatment, yet they identify a need for training. The findings of this study also indicate that sustained practice change, which includes the entire practice environment, should be targeted to enhance tobacco dependence treatment. |

News coverage is associated with a variety of health behaviors, but studies have yet to assess the indirect effects of news coverage generated by a health promotion campaign. The Florida Tobacco Control Program (FTCP), a youth-led marketing and grassroots campaign launched in Florida in 1998, provides the opportunity to examine the effects of program-related newspaper coverage on youth smoking behavior. Articles were collected from Florida daily newspapers, content analyzed to identify the prevalence of topic-specific coverage, and merged by county and year to the Florida Youth Tobacco Survey, a statewide, representative survey of Florida middle school and high school students administered between 1998 and 2002. Trend comparisons and logistic regression models, controlling for alternate explanations, show that FTCP newspaper coverage (particularly coverage of youth advocacy efforts) contributed to observed declines in current smoking. Newspaper coverage of health communication campaigns may represent a meaningful, indirect source of campaign effects. |

Un examen de la réponse des médias de masse au problème du tabagisme au cours des 25 dernières années révèle que la participation soutenue des médias audiovisuels et imprimés a considérablement contribué à sensibiliser le public et à réduire les taux de tabagisme dans la population totale des États-Unis. La publicité de service public fait partie intégrante du mouvement de lutte contre le tabagisme depuis le début, mais l'environnement médiatique extrêmement concurrentiel d'aujourd'hui a contraint les promoteurs de la santé à regarder au-delà des messages d'intérêt public pour développer des programmes de communication globale. Le plaidoyer médiatique - utiliser les médias pour affiner la sensibilisation du public et façonner les politiques publiques pour servir l'intérêt public, une technique dérivée des campagnes politiques - apparaît comme un outil puissant dans le mouvement de lutte contre le tabagisme. Son objectif est de changer l'ensemble du contexte social de l'usage du tabac en Amérique, plutôt que le comportement tabagique des gens. Parce que le succès des défenseurs des médias dépend de leur accès aux médias, ils doivent être capables à la fois de créer des nouvelles et de réagir rapidement aux dernières nouvelles et aux événements inattendus. La nature opportuniste et risquée du plaidoyer médiatique exige que la plupart des efforts soient déployés aux niveaux national et local. Un nombre croissant de services de santé des États et d'autres organisations utilisent la publicité payante pour améliorer la fréquence et la portée des messages antitabac. La recherche vérifie que les campagnes médiatiques payantes augmentent l'exposition du public cible aux messages de lutte contre le tabagisme, mais la planification et la réalisation d'achats médiatiques efficaces nécessitent de la sophistication et, bien sûr, les fonds nécessaires. Des preuves médicales irréfutables liant le tabagisme à la maladie et à la toxicomanie, combinées à la puissante force sociale des non-fumeurs mouvement des droits de l'homme, offrent l'espoir qu'une société sans fumée est un objectif réalisable. Cependant, le succès ne sera atteint que si les militants de la lutte antitabac utilisent toute la gamme des technologies des médias de masse pour soutenir et nourrir cet élan. |

Les prédicteurs de la participation à un groupe de sevrage tabagique chez les femmes à faible revenu ont été examinés. Cinquante fumeurs ont été recrutés dans un établissement de soins primaires de la communauté du Midwest, desservant une population non assurée, sous-assurée et Medicaid. Les participantes ont rempli un questionnaire avant l'intervention et onze femmes ont choisi de participer aux sessions de six semaines. Il y avait peu de différences entre le groupe de contrôle (non participants) et le groupe d'intervention. Le groupe d'intervention avait des scores significativement plus élevés d'intention de quitter. L'analyse de régression logistique a été utilisée pour prédire l'appartenance à un groupe avec l'âge, l'auto-efficacité, l'optimisme, le soutien social, la dépendance à la nicotine, l'intention de cesser de fumer et les autres fumeurs à la maison comme variables prédictives. Les deux seuls prédicteurs significatifs de la participation à l'intervention de sevrage tabagique étaient l'auto-efficacité et l'intention de cesser de fumer. Plus le score d'intention de cesser de fumer est élevé et plus le score d'auto-efficacité est bas (la conviction qu'il est possible de réussir à ne pas fumer); plus ces femmes étaient susceptibles de participer à l’intervention de groupe. Les résultats sont discutés en termes de pertinence clinique et de recherche future dans le domaine. |

Background: Swedish male smokers are more likely than female smokers to switch to smokeless tobacco (snus) and males’ smoking cessation rate is higher than that of females. These results have fuelled international debate over promoting smokeless tobacco for harm reduction. This study examines whether similar results emerge in the United States, one of few other western countries where smokeless tobacco has long been widely available.
Methods: US data source: national sample in Tobacco Use Supplement to Current Population Survey, 2002, with 1-year follow-up in 2003. Analyses included adult self-respondents in this longitudinal sample (n = 15 056). Population-weighted rates of quitting smoking and switching to smokeless tobacco were computed for the 1-year period.
Results: Among US men, few current smokers switched to smokeless tobacco (0.3% in 12 months). Few former smokers turned to smokeless tobacco (1.7%). Switching between cigarettes and smokeless tobacco, infrequent among current tobacco users (<4%), was more often from smokeless to smoking. Men quit smokeless tobacco at three times the rate of quitting cigarettes (38.8% vs 11.6%, p<0.001). Overall, US men have no advantage over women in quitting smoking (11.7% vs 12.4%, p = 0.65), even though men are far likelier to use smokeless tobacco.
Conclusion: The Swedish results are not replicated in the United States. Both male and female US smokers appear to have higher quit rates for smoking than have their Swedish counterparts, despite greater use of smokeless tobacco in Sweden. Promoting smokeless tobacco for harm reduction in countries with ongoing tobacco control programmes may not result in any positive population effect on smoking cessation. |

BACKGROUND AND OBJECTIVES:
Non–cigarette tobacco marketing is less regulated and may abstract
promote cigarette smoking among adolescents. We quantified receptivity to advertising for multiple tobacco products and hypothesized associations with susceptibility to cigarette smoking.
METHODS:
Wave 1 of the nationally representative PATH (Population Assessment of Tobacco and Health) study interviewed 10 751 adolescents who had never used tobacco. A stratified random selection of 5 advertisements for each of cigarettes, e-cigarettes, smokeless products, and cigars were shown from 959 recent tobacco advertisements. Aided recall was classified as low receptivity, and image-liking or favorite ad as higher receptivity. The main dependent variable was susceptibility to cigarette smoking.
RESULTS:
Among US youth, 41% of 12 to 13 year olds and half of older adolescents were receptive to at least 1 tobacco advertisement. Across each age group, receptivity to advertising was highest for e-cigarettes (28%–33%) followed by cigarettes (22%–25%), smokeless tobacco (15%–21%), and cigars (8%–13%). E-cigarette ads shown on television had the highest recall. Among cigarette-susceptible adolescents, receptivity to e-cigarette advertising (39.7%; 95% confidence interval [CI]: 37.9%–41.6%) was higher than for cigarette advertising (31.7%; 95% CI: 29.9%–33.6%). Receptivity to advertising for each tobacco product was associated with increased susceptibility to cigarette smoking, with no significant difference across products (similar odds for both cigarette and e-cigarette advertising; adjusted odds ratio = 1.22; 95% CI: 1.09–1.37).
CONCLUSIONS: A large proportion of US adolescent never tobacco users are receptive to
tobacco advertising, with television advertising for e-cigarettes having the highest recall.
Receptivity to advertising for each non–cigarette tobacco product was associated with
susceptibility to smoke cigarettes.
