Methods: Relevant databases were searched for randomized controlled trials (RCTs) of preoperative smoking cessation interventions. Trial inclusion, risk of bias assessment and data extraction were performed by two authors. Risk ratios for the above outcomes were calculated and pooled effects estimated using the fixed-effect method. Results: Eleven RCTs were included containing 1194 patients. Smoking interventions were intensive, medium intensity and less intensive. Follow-up for postoperative complications was 30 days. For smoking cessation it was from the day of surgery to 12 months thereafter. Overall, the interventions significantly reduced the occurrence of complications (pooled risk ratio 0·56 (95 per cent confidence interval 0·41 to 0·78) P < 0·001). Intensive interventions increased smoking cessation rates both before operation and up to 12 months thereafter. The effects of medium to less intensive interventions were not significant. Meta-analysis of the effect on smoking cessation was not done owing to heterogeneity of data. Conclusion: Surgical patients may benefit from intensive preoperative smoking cessation interventions. These include individual counselling initiated at least 4 weeks before operation and nicotine replacement therapy. Subjects were requested to abstain from smoking for 18 hours. The results demonstrated the superiority of the low-ball technique. Smoking cessation has major health benefits for men and women of all ages. However, most smokers are addicted to nicotine and fail repeatedly in their attempts to quit. Stimulation of nicotinic receptors in the brain, particularly alpha4beta2 receptors, releases dopamine in the meso-limbic area of the brain and is reinforcing. Nicotine abstinence reduces dopamine release, and this is associated with withdrawal symptoms and craving for nicotine. Eight current pharmacotherapies--bupropion, nortriptyline, clonidine and nicotine patch, gum, inhaler, lozenge and nasal spray--are moderately effective aids to smoking cessation. Each is significantly better than placebo, but approximately 80% of patients using one of these medications return to smoking within the first year. Varenicline, a specific alpha4beta2 nicotinic receptor partial agonist, is a new pharmacotherapy that stimulates dopamine and simultaneously blocks nicotine receptors. Phase II and III trials have yielded promising results suggesting that varenicline could be an important advance in the treatment of nicotine dependence.

[TABAC] tabagisme:aspect socialIndex. décimale : TA 6.4.1 Tests psychologiques Résumé : While a strategy of compliance without pressure (Joule, 1987) had the effect of inducing almost all of a group of smoking subjects to stop smoking first for 18 hours then for 3 days, simply observing someone (an accomplice) break his or her own initial agreement to abstain from smoking for 18 hours was enough to bring about a substantial reduction in the willingness of other subjects to later abstain for 3 days. However, subjects did not follow the lead of the accomplice immediately, and persisted in their agreement to abstain for 18 hours. This pattern of indirect, but not direct influence, suggests that there may be a type of minority influence at work here that represents a sort of behavioral conversion.
