Disposable e-cigarettes are the predominant type of vaping product used by adolescents and pose a significant public health concern. Identifying factors contributing to this growing trend is essential to curbing the vaping epidemic among youths. This study aims to investigate the growing prevalence and correlates of disposable e-cigarette use among US students.
Material and Methods:
Data from 48437 US middle and high school students from the 2021 and 2022 National Youth Tobacco Survey (NYTS) were analyzed using logistic and ordinal regression models to evaluate disposable e-cigarette use and frequency of use (low, medium, and high) with demographic and psychosocial factors. Weighted prevalence of current e-cigarette use with 95% CIs by device types in 2021 and 2022, were calculated. Odds ratios (ORs) of correlations of disposable e-cigarette use and frequency of use with demographic and psychosocial factors were analyzed.
Results:
Disposable e-cigarette use increased from 3.9% (95% CI: 3.3–4.7) in 2021 to 5.1% (95% CI: 4.2–6.1) in 2022, and was associated with being female (OR=1.57; 95% CI: 1.29–1.91 vs male), high schoolers (OR=5.14; 95% CI: 3.96– 6.67 vs middle schoolers), having low harm perceptions of e-cigarettes (OR=7.75; 95% CI: 5.58–10.75 vs lot of harm), and high exposure to marketing (OR=1.57; 95% CI: 1.05–2.35 vs low exposure). Identifying as LGBTQ (OR=1.41; 95% CI: 1.00–2.00 vs straight), having low academic performance (OR=2.16; 95% CI: 1.15–4.07, D vs A grades), and having psychological distress (OR=2.01; 95% CI: 1.64–2.47, severe vs none) were also linked to increased frequency of use.
Conclusions:
This study underscores increasing disposable e-cigarette use among US students, noting existing disparities. It identifies high-risk adolescent subgroups vulnerable to disposable e-cigarette use. Disposable e-cigarette use has increased among United States (US) high school students in recent years. However, there is limited research on the profile of these users, how often they use these products, and whether they displace cigarette smoking. This study aimed to measure how disposable e-cigarette use among US youth varies according to demographic characteristics and whether there is any association between e-cigarette use and reduced use of traditional cigarettes.
Design
We used cross-sectional data from the 2022 National Youth Tobacco Survey and conducted a multinomial logistic regression to examine factors associated with the types of e-cigarette devices used in the prior 30 days, adjusting for sex, sexual identity, grade level and race/ethnicity. We also used a finite mixture model to account for unobserved differences among users and identify e-cigarette use patterns in different subgroups of users.
Setting
United States.
Participants
High school students in grades 9–12 (n = 14 389).
Measurements
Survey participants self-reported the type of e-cigarette device used, the frequency of e-cigarettes used and cigarettes smoked over the past 30 days.
Findings
Disposable e-cigarettes were the most popular e-cigarette type. Sex, sexual orientation, grade level and race/ethnicity were associated with disposable e-cigarette use. The odds of disposable e-cigarette use were lower in male students than in female students (odds ratio [OR] = 0.78, 95% confidence interval [CI] = [0.64–0.96]), and higher in students who identified as gay or lesbian (OR = 1.70, 95% CI = [1.11–2.61]) or bisexual (OR = 1.52, 95% CI = [1.16–1.99]) than in heterosexual students. The odds of disposable use were higher among students in higher grades (10th, 11th and 12th) than in 9th graders (OR = 1.71, 2.24 and 2.52, respectively). Disposable e-cigarette users had a lower frequency of traditional cigarette use than other e-cigarette users, both in the low-frequency class (incidence rate ratio [IRR] = 0.33, 95% CI = [0.12–0.92]) and the high-frequency class (IRR = 0.27, 95% CI = [0.08–0.92]).
Conclusions
Disposable e-cigarette use appears to be higher among United States high school students who are female, older and/or identify as gay, lesbian or bisexual. E-cigarettes, or electronic nicotine delivery systems (ENDs), are marketed as a ‘safe' alternative to tobacco products and a tool for ‘harm reduction'. Some public health experts are calling it a ‘game changer' and favour the ‘harm reduction' strategy, while others dispute this claim. In our opinion, the debate needs to be broadened to encompass other related concerns and effects on non-users and affected stakeholders. Health advocates have been reluctant to endorse a harm reduction approach out of fear that tobacco companies cannot be trusted to produce and market products that will reduce the risks associated with tobacco use. Recently, companies independent of the tobacco industry introduced electronic cigarettes, devices that deliver vaporized nicotine without combusting tobacco. We review the existing evidence on the safety and efficacy of electronic cigarettes. We then revisit the tobacco harm reduction debate, with a focus on these novel products. We conclude that electronic cigarettes show tremendous promise in the fight against tobacco-related morbidity and mortality. The assumption has been that electronic cigarettes are substantially safer than cigarettes because they avoid combustion. This assumption, combined with assumptions that electronic cigarettes are effective for cigarette cessation and do not appeal to youth, led many to argue electronic cigarettes are harm reduction. By 2023, evidence revealed higher risks than assumed, that electronic cigarettes as consumer products do not help smokers quit, and that electronic cigarettes have addicted millions of youth to nicotine. Dual use (using both electronic cigarettes and cigarettes) is more harmful than just smoking cigarettes. The failure of electronic cigarettes as consumer products to help people stop smoking makes their relative toxicity compared to cigarettes a moot point. Electronic cigarettes have also played a role in the multinational tobacco companies’ efforts to reposition themselves as socially responsible, which helps protect their financial and political interests. Countries, including Turkey, that have prohibited the import and avoided the sale of electronic cigarettes have done better overall at controlling the electronic cigarette epidemic than countries, including the United States and England, that have adopted more laissez-faire policies toward electronic cigarettes. Turkey should maintain and ensure effective enforcement of its current policies to continue to protect its population from electronic cigarettes. This research, utilizing content analysis methodology, examined three online news articles from Indonesian news portals: Kompas.com, IDNTimes.com, and Detik.com. The study aimed to describe how online news conveys the dangers of using electronic cigarettes using the Extended Parallel Process Model (EPPM). The research method employed was content analysis. EPPM explains how people manage the fear evoked by threat messages. There are two key factors that can explain how individuals respond to fear messages. The first factor is the perception of threat, consisting of the severity and vulnerability to the threat. The second factor is the perception of efficacy, consisting of response efficacy and self-efficacy. The research findings indicate that online news coverage prioritizes the perception of threat. Conversely, the aspect of efficacy perception still needs to be applied in reporting the dangers of electronic cigarettes. These results are also influenced by ethical codes, journalistic principles, and cultural influences. Le taux de tabagisme parmi les femmes enceintes qui consomment des substances psychoactives est particulièrement élevé en comparaison avec la population générale. Les consommations associées ont aussi pour effet de compliquer la prise en charge du tabagisme.
Ce travail souhaite tenir compte de ces particularités - à savoir la grossesse et la polyconsommation - pour aborder la question du tabagisme.
Après avoir défini la polyconsommation de substances psychoactives et présenté le tabagisme chez la femme enceinte, une revue de la littérature est proposée pour préciser les facteurs à prendre en compte et nécessitant une approche spécifique. La perception du tabagisme par les femmes enceintes et la place accordée au tabagisme dans les services d’assuétudes seront ensuite développées. Enfin, le dernier point abordera la question de la réduction des risques tabagiques chez la femme enceinte.
Une enquête qualitative réalisée à partir d’entretiens semi-directifs interrogera directement des femmes enceintes ou ayant un enfant de moins d’un an, fumeuses et consommatrices d’au moins une autre substance psychoactive. L’objectif étant d’identifier leurs connaissances, leurs ressentis et leurs souhaits en matière de tabagisme.
Ces conclusions sont limitées en raison du petit échantillon de femmes interrogées mais ces quelques vignettes cliniques constituent néanmoins des pistes de travail, autant au niveau des mères que des services qui les accompagnent.
