[NICO] ~termes rejetés :tabagisme environnementalIndex. décimale : TA 7.4.2.2 chez l'adulte Résumé : The objective of this paper was to review the epidemiological literature examining the association between secondhand smoke (SHS) and cardiovascular disease (CVD). Specifically, we examined the various screening methods available in assessing smoking behaviour and quantifying nicotine absorption. Further, we considered the natural history of those exposed to SHS and the associated risk of CVD. We reviewed routine methods used to assess exposure to SHS; evaluated the utility of subjective screening questions regarding smoking behaviour and examined the efficacy of nicotine and cotinine biomarkers used to quantify SHS exposure in epidemiological and clinical-based research. Self-reporting is practical and cost-effective in identifying smoking behaviour patterns, but is subject to recall bias and underestimation of exposure, especially in the presence of children. Nicotine and cotinine biomarkers have proven valuable in quantifying tobacco smoke absorption and establishing biological plausibility. A combination of SHS self-reported and biomarker evaluation provide the most stringent method of establishing exposure. Sufficient evidence is reported in epidemiological research to support a causal association between SHS exposure and increased risks of CVD morbidity and mortality among both men and women. The risk of developing an acute cardiac syndrome or chronic lifetime coronary events is at least 30%. Similarly, reduction in the incidence of a myocardial infarction decreases by nearly 50% in the absence of SHS. Considering the biological plausibility and dose-response relationship between SHS and CVD, effective interventions that incorporate a comprehensive screening method of behavioral and biological measures of exposure coupled with efficacious treatment should elicit favorable change for at-risk populations.

Titre : Hookah (shisha, narghile) smoking and environmental tobacco smoke (ETS) : a critical review of the relevant literature and the public health consequences Type de document : texte imprimé Auteurs : Kamal Chaouachi, Auteur Editeur : Bâle [Suisse] : MDPI AG (Molecular Diversity Preservation International) Année de publication : 2009 Collection : International Journal of Environmental Research and Public Health, ISSN 1660-4601 num. 6(2) Importance : p.798–843 Langues : Français (fre) Catégories : [NICO] chimie du tabac:fumée

[NICO] ~termes rejetés :tabagisme environnementalIndex. décimale : TA 1.1.4 Chicha – Narguilé Résumé : Hookah (narghile, shisha, “water-pipe”) smoking is now seen by public health officials as a global tobacco epidemic. Cigarette Environmental Tobacco Smoke (ETS) is classically understood as a combination of Side-Stream Smoke (SSS) and Exhaled Main-Stream Smoke (EMSS), both diluted and aged. Some of the corresponding cigarette studies have served as the scientific basis for stringent legislation on indoor smoking across the world. Interestingly, one of the distinctive traits of the hookah device is that it generates almost no SSS. Indeed, its ETS is made up almost exclusively by the smoke exhaled by the smoker (EMSS), i.e. which has been filtered by the hookah at the level of the bowl, inside the water, along the hose and then by the smoker’s respiratory tract itself. The present paper reviews the sparse and scattered scientific evidence available about hookah EMSS and the corresponding inferences that can be drawn from the composition of cigarette EMSS. The reviewed literature shows that most of hookah ETS is made up of EMSS and that the latter qualitatively differs from MSS. Keeping in mind that the first victim of passive smoking is the active smoker her/himself, the toxicity of hookah ETS for non-smokers should not be overestimated and hyped in an unscientific way. [NICO] ~termes rejetés :tabagisme parentalIndex. décimale : TA 7.4.2.1 chez l'enfant Résumé : Background: Many children are exposed to ETS (environmental tobacco smoke), which has both immediate and long‐term adverse health effects. The aim was to determine the prevalence and nature of smoking among parents with infants and the association of indoor or outdoor smoking with the health of their children.
Methods: Mail‐questionnaire study, which was performed in a county in the south‐east of Sweden, as a retrospective cross‐sectional survey including 1990 children, 12–24 months old.
Results: 20% of the children had at least one smoking parent; 7% had parents who smoked indoors and 13% parents who smoked only outdoors. Indoor smoking was most prevalent among single and blue‐collar working parents. In the case of smoking cessation during pregnancy, smoking was usually resumed after delivery or at the end of the breast‐feeding period. Coughing more than two weeks after a URI (upper respiratory infection), wheezing without a URI as well as pooled respiratory symptoms differed significantly between children of non‐smokers and indoor smokers.
Conclusion: Further research of the common belief that outdoor smoking is sufficient to protect infants from health effects due to ETS exposure is warranted.
