E-cigarettes, also referred to as electronic cigarettes, are gaining popularity as an alternative to tobacco use. Although their use has significantly increased recently, there is still much debate over their effectiveness and safety as smoking cessation aids.
Objective:
This review paper’s objective is to offer an inclusive evaluation of the body of research about the impact of ecigarettes on the oral microbiome and oral health.
Methodology: Data was extracted from PubMed, Google Scholar, and Scopus to compile scientific evidence for a narrative review article on the effects of e-cigarettes on the oral microbiome. The investigation was restricted to published research articles from 2001 to 2022, using pre-defined search phrases for e-cigarettes, vaping, quitting smoking, oral microbiota, and oral health.
Results:
The chemicals in e-cigarettes, including nicotine, propylene glycol, vegetable glycerin, flavorings, and chemical additives, cause a dysbiosis that can lead to a variety of conditions, including dental caries, periodontal diseases, and a variety of mental health issues, such as bone loss, gingival tissue inflammation, and increased plaque accumulation. The chemical toxicity of e-cigarettes, including nicotine and other components present in e-cigarette aerosols, has been implicated in various oral health problems that lead to systemic complications.
Conclusion:
A wide range of oral health sequelae may be associated with e-cigarette use. GroupA (n = 100) received 7.2 mg nicotine cartridges for 12 weeks; GroupB (n = 100), a 6-week 7.2 mg nicotine cartridges followed by a further 6-week 5.4 mg nicotine cartridges; GroupC (n = 100) received no-nicotine cartridges for 12 weeks. The study consisted of nine visits during which cig/day use and exhaled carbon monoxide (eCO) levels were measured. Smoking reduction and abstinence rates were calculated. Young people who become addicted to nicotine are at greater risk of becoming lifelong tobacco consumers. The use of nicotine-delivering electronic cigarettes has risen dramatically among youths worldwide. In addition to physical dependence, adolescents are susceptible to social and environmental influences to use electronic cigarettes. The product design, flavours, marketing, and perception of safety and acceptability have increased the appeal of electronic cigarettes to young people, thus leading to new generations addicted to nicotine. Moreover, there is growing evidence that electronic cigarettes in children and adolescents serve as a gateway to cigarette
smoking. There can be no argument for harm reduction in children. To protect this vulnerable population from electronic cigarettes and other nicotine delivery devices, we recommend that electronic cigarettes be regulated as tobacco products and included in smoke-free policies. Sale of electronic cigarettes should be
barred to youths worldwide. Flavouring should be prohibited in electronic cigarettes, and advertising accessible by youths and young adults be banned. In order to support the process of opinion formation on this issue, the German Cancer Research Center (Deutsches Krebsforschungszentrum, DKFZ) has issued the present report, which evaluates currently available studies on product quality of electronic cigarettes, potential health implications and their usefulness as tobacco cessation aids. The German Cancer Research Center welcomes the suggestion made by the European Commission to regulate electronic cigarettes as medicinal products. Health advocates have been reluctant to endorse a harm reduction approach out of fear that tobacco companies cannot be trusted to produce and market products that will reduce the risks associated with tobacco use. Recently, companies independent of the tobacco industry introduced electronic cigarettes, devices that deliver vaporized nicotine without combusting tobacco. We review the existing evidence on the safety and efficacy of electronic cigarettes. We then revisit the tobacco harm reduction debate, with a focus on these novel products. We conclude that electronic cigarettes show tremendous promise in the fight against tobacco-related morbidity and mortality. Methods: A total of 160 participants were divided into four groups according to the type of product used, including non-smokers, users of conventional combustible tobacco (cigarettes), heated tobacco, and electronic, tobacco-free vapor products (e-cigarettes). Buccal mucosa samples were analyzed using the micronucleus cytome assay
to assess cytotoxic and genotoxic damage.
Results: E-cigarette users showed significantly higher values for all tested parameters in the micronucleus test compared to non-smokers (p < 0.05). Similarly, users of tobacco heating products showed an increase in all parameters (p < 0.05), with the exception of the number of cells with micronuclei. Conventional cigarette smokers showed a notable increase in the number of binucleated cells and cells with karyorrhexis and karyolysis (p ≤ 0.05). When assessing the differences between users of traditional combustible tobacco products and non-combustible alternatives, these did not appear to be significant, except for e-cigarette users, who had significantly more cells with condensed chromatin (p ≤ 0.001), while users of tobacco heating products had more pyknotic cells (p ≤ 0.001).
Conclusion: The results of this study underscore the heightened occurrence of cytotoxic and genotoxic damage in users of both conventional combustible tobacco products and non-combustible alternatives compared to non-smokers, emphasizing the detrimental impact of these products on the oral mucosa.
