Pays développés (Amérique du Nord, Europe, Océanie) Résumé : In 2022, 635 tuberculosis (TB) patients were notified in the Netherlands. This is six per cent fewer than in 2021, when there were 673 patients. This decline is consistent with the downward trend in TB cases in the Netherlands over the past 20 years.
In 2020 patient numbers fell more sharply than in the years prior. Between March 2020 and May 2022, coronavirus measures including social distancing and less contact with others were in effect in the Netherlands. This most likely mitigated the spread of the TB bacterium. Another mitigating factor was the decline in the number of immigrants and asylum seekers who came to the Netherlands in 2020 as a result of global travel restrictions.
TB is more prevalent among those not born in the Netherlands. In 2022, this was the case for four out of every five patients. Most patients were born in Eritrea (70), followed by Morocco (59) and Somalia (36). TB is widespread in Africa and Asia.
TB is an infectious and potentially contagious disease caused by a bacterium. Of the 635 patients who had TB in 2022, more than 60 per cent had pulmonary TB and the remaining patients had extrapulmonary TB. A quarter of all patients had the most contagious type (open TB).
People who have become infected with the bacterium but have not developed TB (yet) are said to have contracted a TB infection. Early detection of TB infections makes it possible for these people to be treated before they become ill. This, in turn, prevents the TB from spreading to other people. In addition to the 635 people who became ill in 2022, almost 1,200 people were reported to have contracted a TB infection. More than a quarter of these people was traced using source and contact tracing among those known to have been in close contact with a TB patient. A third were traced by testing new arrivals from countries were TB is widespread.
RIVM reports the number of TB cases and TB infections each year in order to monitor the effect of measures to reduce the incidence of TB in the Netherlands. Pays en voie de développement (Amérique du sud, Afrique, Asie) Résumé : Globally, 10 million incident cases of tuberculosis (TB) are reported annually, and 95% of TB cases and 80% of tobacco users reside in low- and middle-income countries. Smoking approximately doubles the risk of TB disease and TB mortality. We estimated the proportion of annual incident TB cases and TB mortality attributable to tobacco smoking in 32 high-TB-burden countries. We obtained country-specific estimates of TB incidence, TB mortality, and smoking prevalence from the World Health Organization Global TB Report (2017), tobacco surveillance reports (2015), and the Tobacco Atlas. Risk ratios for the effect of smoking on TB incidence and TB mortality were obtained from published meta-analyses. An estimated 17.6% (95% confidence interval (CI): 8.4, 21.4) of TB cases and 15.2% (95% CI: 1.8, 31.9) of TB mortality were attributable to smoking. Among high-TB-burden countries, Russia had the highest proportion of smoking-attributable TB disease (31.6%, 95% CI: 15.9, 37.6) and deaths (28.1%, 95% CI: 3.8, 51.4). Men had a greater proportion of TB cases attributable to smoking (30.3%, 95% CI: 14.7, 36.6) than did women (4.3, 95% CI: 1.7, 5.7). Our findings highlight the need for tobacco control in high-TB-burden countries to combat TB incidence and TB mortality.

Historique Résumé : World War II (WWII) began on September 1, 1939, when the Nazi German army invaded Poland and progressed relentlessly towards Warsaw. After silence from Germany, the United Kingdom, France, and several members of the British Empire declared war on September 3, 1939. Belgium's involvement in WWII began on May 10, 1940, when the Nazi army invaded Belgium on its way to France and Belgium was occupied by the Germans from May 28, 1940 until liberation by the allies. In 1850, the TB mortality rate was 376.7/100,000 and fell to 155.2/100,000 in 1900. As in most of Europe, TB mortality increased strikingly in WWI though less in WWII. But in 1941, TB mortality rose sharply to 98.3/100,000, remained high in 1942 to 95.3/100,000 and then fell progressively to 77.8/100,000in 1945. Only in 1946 did the rate fall below that at the beginning of the war. The destruction and requisition of TB facilities worsened the plight of the rising number of active TB patients. The socioeconomic situation was desperate: 13% of men were prisoners of war; calories were restricted, malnutrition was prevalent.
