Yet, at this time, there remains limited data regarding the safety and efficacy in using these nicotine products during pregnancy. Notably, reports assessing the effect of nicotine exposure on postnatal health outcomes in humans, including reproductive health, are severely lacking. Our current understanding regarding the consequences of nicotine exposure during pregnancy is limited to a few animal studies, which do not comprehensively address the underlying cellular mechanisms involved. This paper aims to critically review the current knowledge from human and animal studies regarding the direct and indirect effects (e.g. obesity) of maternal nicotine exposure, regardless of its source, on reproductive outcomes in pregnancy and postnatal life. Furthermore, this review highlights several key cellular mechanisms involved in these adverse reproductive deficits including oxidative stress, inflammation, and endoplasmic reticulum (ER) stress. Cependant il apparait qu’environ une femme enceinte sur quatre fume durant sa grossesse (BDMS5 ONE, 2006) et d’autant plus lorsqu’elle évolue dans un environnement socio-économique fragilisé. Les enquêtes dirigées vers les gynécologues et les sages-femmes montrent notamment que ceux-ci manquent de repères et de recommandations claires pour informer et soutenir la femme enceinte dans un arrêt tabagique. Un travail de sensibilisation et de mobilisation des professionnels s’est donc mis en place avec le soutien du Service Public Fédéral Santé publique et l’INAMI. Il en est ressorti une volonté commune de changer la donne et de fournir des recommandations éclairées en cette matière. Ainsi, début 2010, sur une initiative du FARES6 asbl, un groupe de travail s’est penché sur la question et, sur base d’une revue de la littérature scientifique, propose des outils applicables à la réalité de terrain du gynécologue obstétricien afin que ce facteur de risque puisse bénéficier d’une prise en charge professionnelle. Is there an association between maternal smoking and the development of complicated gastroschisis? A retrospective population-based review of all cases of gastroschisis over 11 years was conducted in three Canadian provinces. Complicated cases were defined as those with an associated intestinal atresia or other vascular compromise of the bowel, those requiring a prolonged time to full enteral feeding (>42 days). Univariate and multivariate regression analyses were conducted. Fifty-four cases of gastroschisis were treated. Seventeen patients had complicated gastroschisis (CG). 47.1% of infants were born to smoking mothers of those 56.25% were in the CG group. On univariate analysis statistically significant associations with complicated cases were young maternal age (<19 years) (OR = 6.0, CI: 1.3, 28.1), low maternal weight gain (<12 kg) (OR = 4.6, CI: 2.5, 8.5), low birth weight (<2,400 g), (OR = 7.7, CI: 2.0, 28.9), and gestational age under 36 weeks (OR = 11.0, CI: 2.6, 46.2). Seven of 24 cases involving maternal smoking developed atresias or vascular intestinal compromise versus 4 of 27 cases with no maternal smoking, with an odds ratio of 2.1 (CI: 0.60, 7.2). On multivariate analysis, atresias and vascular compromise remained significantly associated with young maternal age, low gestational age and low 1-min Apgar score. Although a positive trend is seen, no statistically significant relationship between maternal smoking and the development of complicated gastroschisis was found. Further clinical and epidemiologic studies are needed to validate our findings. The authors' aim in this study was to disentangle the effects of prenatal and postnatal smoking on early childhood respiratory health. Most parents who smoke during pregnancy continue to smoke postpartum, and it is difficult to identify sufficiently large subgroups of children who were exclusively exposed in utero or after birth. This study was based on the first 22,390 children born between 2000 and 2004 in the Norwegian Mother and Child Cohort, a pregnancy cohort designed to eventually include 100,000 pregnancies. Data were collected through detailed questionnaires administered at various stages of pregnancy, starting in early pregnancy. Because of the large study population, the authors were able to disentangle the preand postnatal effects of parental smoking on wheeze and lower respiratory tract infection in the children's first 18 months of life. They found maternal smoking in pregnancy to be an independent risk factor for wheeze and respiratory infection. STUDY DESIGN: Folate and cotinine levels were evaluated in maternal serum (n = 125) and coelomic fluid (n = 42), and immunostaining was used to identify folate receptors in placental villi, decidual tissue, and secondary yolk sacs from normal pregnancies at 6-12 weeks' gestation. RESULTS: Folate receptors-_were found on the apical surface of syncytiotrophoblast, uterine glandular epithelial cells, and mesothelial layer of the yolk sac. Significantly higher (P<.05) median folate levels were found in maternal serum than in coelomic fluid. The median folate maternal serum and coelomic levels were significantly (P<.0001 and P<.005) lower in smokers, compared with nonsmokers. CONCLUSION: Uterine glands and the secondary yolk sac play key roles in supplying folic acid to the developing fetus before the placental circulations are established, and maternal smoking does not seem to impair directly its transfer mechanism in early pregnancy. In total, 65 questionnaires were completed (52.42% return rate). Health visitors, midwives and practice nurses were aware of the reasons why pregnant women smoke as well as the health risks associated with smoking. All claimed to intervene by asking if their patients smoked. Most respondents claimed to record smoking status on health records and to give cessation advice to their patients. However, fewer of the health professionals provided advice to partners of women in their care and only a minority had read the NICE clinical guidelines on nicotine replacement therapies. There were limitations to the health professionals’ attitudes, knowledge and practice that could affect the quality of smoking cessation service offered to patients.
