Artificial sweeteners are listed as ingredients of oral nicotine pouches (ONPs), a new product category with rapidly growing market share. The exact sweetener contents of ONPs remain unknown. Artificial sweeteners in ONPs may facilitate initiation and encourage consumption behavior.
Aims and Methods:
Artificial sweetener contents in major US-marketed ONP brands (Zyn, on!, Velo) were determined by Liquid Chromatography-Mass Spectrometry (LC-MS). Sweetener effects during the initiation of ONP consumption were modeled in single- and two-bottle tests, offering mice ONP extracts calibrated to contain nicotine levels similar to saliva of people who use smokeless tobacco. To examine the contribution of sweet taste perception, consumption behavior was compared between wild-type mice and mice deficient in the sweet taste receptor (Tas1r2-/-).
Results:
Acesulfame-K was detected in on!, Zyn and Velo ONPs (~0.3-0.9 mg/pouch), including products marketed as “Unflavored” or “Flavor ban approved”. In Velo ONPs, sweetened with sucralose (0.6-1.2 mg/pouch), higher nicotine strength products contained higher sucralose levels. Tas1r2-/- mice consumed less ONP extracts than wild-type mice in both sexes. ONP extracts with both higher nicotine and sweetener strengths were tolerated by wildtype mice, but produced stronger aversion in Tas1r2-/- mice.
Conclusions:
ONPs contain significant amounts of artificial sweeteners, with some brands adding more sweetener to ONPs with higher nicotine strengths. Artificial sweeteners, at levels present in ONPs, increase nicotine consumption. Increasing sweetener contents facilitates consumption of ONPs with higher nicotine strengths. Sweetness is a key determinant of ONP use initiation, likely reducing the aversive sensory effects of nicotine and other ONP constituents.
Implications:
Artificial sweeteners such as acesulfame-K or sucralose reduce aversion and facilitate initiation and continued consumption of ONPs. The marketing of some artificially sweetened ONPs as
“Unflavored” of “Flavor ban-approved” suggests that the tobacco industry rejects sweet taste as a determinant for the presence of a characterizing flavor. Sweetness as imparted by artificial
sweeteners in tobacco products needs to be addressed by regulators as a component of a characterizing flavor, with the aim to reduce product appeal and initiation by never users, and

PURPOSE:
Smoking is the leading controllable risk factor for heart disease. Only about 69% of U.S. indoor workers are currently covered by a smoke-free workplace policy. This analysis projects the cardiovascular health and economic effects of making all U.S. workplaces smoke free after 1 year and at steady state.
METHODS:
We estimated the number of U.S. indoor workers not covered by smoke-free workplace policies, and the effects of making all workplaces smoke free on smoking behavior and on the relative risks of acute myocardial infarctions and strokes. One-year and steady-state results were calculated using an exponential decline model. A Monte Carlo simulation was performed for a sensitivity analysis.
RESULTS:
The first-year effect of making all workplaces smoke free would produce about 1.3 million new quitters and prevent over 950 million cigarette packs from being smoked annually, worth about 2.3 billion dollars in pretax sales to the tobacco industry. In 1 year, making all workplaces smoke free would prevent about 1500 myocardial infarctions and 350 strokes, and result in nearly $60 [corrected] in savings in direct medical costs. At steady state, 6250 myocardial infarctions and 1270 strokes would be prevented, and $279 million [corrected] would be saved in direct medical costs annually. Reductions in passive smoking would account for 60% of effects among acute myocardial infarctions.
CONCLUSION:
Making all U.S. workplaces smoke free would result in considerable health and economic benefits within 1 year. Reductions in passive smoking would account for a majority of these savings. Individuals experienced increased social isolation resulting from the COVID-19 pandemic. Studies have found social isolation and loneliness to be strongly associated with anxiety and depression, which have been associated with increased smoking and vaping rates among young
adults, including college students.
Objectives:
To examine relationships between psychological distress and nicotine use within the context of the COVID-19 pandemic.
Methods:
A cross-sectional online survey (n = 4634; 77.9% female) was used to collect nicotine use and psychological measures from students enrolled at a large Midwestern university. Timeline follow-back data were collected from students reporting current cigarette or electronic cigarette (e-cigarette) use in the week before and immediately following the closure of campus due to the pandemic. Generalized estimating equations were used to examine the interaction between nicotine use and psychological symptoms across the 2-week period.
Results:
Both cigarette (Rate ratio (RR) = 1.115, 95% CI = 1.061, 1.171, p < .0001) and e-cigarette (β = 0.258, 95% CI = 0.166, 0.351, p < .0001) use increased significantly following campus closure. Students experiencing higher levels of depression reported greater increases in e-cigarette use frequency over time as compared to students reporting fewer symptoms of depression (β = 0.018, 95% CI = 0.006, 0.030, p = .004).
Conclusions: Increases in nicotine use were found immediately following the implementation of public health safety measures that closed most university campuses. Additional and/or increased stressors have potentially impacted young adults who are college students as a result of campus closures resulting from the pandemic, which may have contributed to further increases in nicotine

Pictorial warnings on cigarette packs draw attention and increase quit intentions, but their effect on smoking behavior remains uncertain.
OBJECTIVE
To assess the effect of adding pictorial warnings to the front and back of cigarette packs.
DESIGN, SETTING, AND PARTICIPANTS
This 4-week between-participant randomized clinical trial was carried out in California and North Carolina.We recruited a convenience sample of adult cigarette smokers from the general population beginning September 2014 through August 2015. Of 2149 smokers who enrolled, 88%completed the trial. No participants withdrew owing to adverse events.
INTERVENTIONS
We randomly assigned participants to receive on their cigarette packs for 4 weeks either text-only warnings (one of the Surgeon General’s warnings currently in use in the United States on the side of the cigarette packs) or pictorial warnings (one of the Family Smoking Prevention and Tobacco Control Act’s required text warnings and pictures that showed harms of smoking on the top half of the front and back of the cigarette packs).
MAIN OUTCOMES AND MEASURES
The primary trial outcomewas attempting to quit smoking during the study.We hypothesized that smokers randomized to receive pictorial warnings would be more likely to report a quit attempt during the study than smokers randomized to
receive a text-only Surgeon General’s warning.
RESULTS
Of the 2149 participants who began the trial (1039 men, 1060 women, and 34 transgender people; mean [SD] age, 39.7 [13.4] years for text-only warning, 39.8 [13.7] for pictorial warnings), 1901 completed it. In intent-to-treat analyses (n = 2149), smokers whose packs had pictorial warnings were more likely than those whose packs had text-only warnings to attempt to quit smoking during the 4-week trial (40% vs 34%; odds ratio [OR], 1.29; 95% CI, 1.09-1.54). The findings did not differ across any demographic groups. Having quit smoking for at least the 7 days prior to the end of the trial was more common among smokers who received pictorial than those who received text-only warnings (5.7%vs 3.8%; OR, 1.53; 95%CI, 1.02-2.29). Pictorial warnings also increased forgoing a cigarette, intentions to quit
smoking, negative emotional reactions, thinking about the harms of smoking, and conversations about quitting.
CONCLUSIONS AND RELEVANCE
Pictorialwarnings effectively increased intentions to quit, forgoing cigarettes, quit attempts, and successfully quitting smoking over 4 weeks. Our trial
findings suggest that implementing pictorial warnings on cigarette packs in the United States would discourage smoking.

Overall, 11% of the general population have "ever tried" chewing tobacco, snuff, or other smokeless tobacco. Of these, 5% were former users and 3% used smokeless tobacco almost daily in the past year. Rates of its use differed significantly by sex, age group, race, region, and metropolitan area size. Although females were far less likely to try it, those who did were as likely as males to be daily users. Smokeless tobacco users were also more likely to use alcohol, cigarettes, and marijuana. In general, those who used smokeless tobacco almost daily were more likely to report poor health and hospitalization for illness or injury in the past year, even when other substance use was controlled. Smokeless tobacco users also were more likely to report symptoms of depression. Finally, some substituted smokeless tobacco for cigarettes, but youths (12-17 yr old) were more likely than older tobacco users to use both forms of tobacco regularly. Despite a current prevalence of LITS in the USA estimated at 25-33% of all smokers, a systematic review of the literature on this group of smokers has yet to be written.
Objectives: The aim of this paper is to review and evaluate research on LITS and to identify, describe and discuss commonalities and differences between LITS and daily smokers.
Methods: The primary databases used to search for publications were Pub Med (National Library of Medicine) and SCOPUS (Elsevier).
Results: LITS inhale smoke and have post-smoking blood nicotine concentrations that are broadly equivalent to those found in daily smokers. However, LITS differ from daily smokers with regard to cigarette consumption and frequency of cigarette use, sociodemographic and socioeconomic characteristics, motives, personality traits, dependence, withdrawal and craving, response to smoking-related cues, quitting perception, past-smoking status, and initiation.
Conclusions: In contrast to daily smokers, LITS show few or no signs of dependence as currently defined by DSM-IV criteria, appear to exercise more self-control, seem to be less impulsive, and their smoking experience is primarily associated with positive rather than negative reinforcement. Conclusions drawn from the reviewed literature highlight the multivariate factors that must be taken into account when defining LITS and emphasize the importance of further research on this increasing fraction of smokers. We conducted an in-depth review to determine whether states had at least 1 form of e-cigarette delivery sales law. We coded laws for 5 key policy domains: (1) delivery terminology used in laws, (2) age verification requirements, (3) packaging label requirements, (4) permit or registration requirements, and (5) fines and penalties for violations.
Results.
Overall, 34 states had e-cigarette delivery sales laws with varying scopes and dimensions. In 27 states, these laws required at least 1 form of age verification requirements. We identified mandatory packaging labels in 12 states, and 7 states where permits were required. There were considerable
differences among states on the scale of fines and penalties for violations.
Conclusions. We explored whether various key stakeholders considered cost sharing with state telephone-based tobacco cessation quitlines, because including tobacco cessation services as part of the required essential health benefits is a new requirement of the Patient Protection and Affordable Care Act (ACA).
Methods. We analyzed qualitative data collected from interviews conducted in April and May of 2014 with representatives of state health departments, quitline service providers, health plans, and insurance brokers in 4 US states.
Results. State health departments varied in the strategies they considered the role their state quitline would play in meeting the ACA requirements. Health plans and insurance brokers referred to state quitlines because they were perceived as effective and free, but in 3 of the 4 states, the private stakeholder groups did not consider cost sharing.
Conclusions. The purpose of this study was to examine tobacco dependence treatment practices among certified nurse-midwives who treat pregnant women who smoke. Midwives were surveyed to determine knowledge, perceptions, and beliefs about tobacco cessation treatment and to identify practice environmental factors that support treatment practices. Half of all midwives had not heard of the US Public Health Service Guidelines (5 A's) to assist smokers in cessation treatment. We found varying levels of adherence to the clinical practice guidelines. Nearly all midwives routinely ask, advise, and assess; while fewer encourage patients to set a quit date or discuss medication options (assist) and perform follow-up activities (arrange). Barriers significantly associated with clinical tobacco treatment practice are lack of training and competing priorities in the visit. One-office support, a system in place to provide smoking cessation information and resources, was associated with increased practice. In summary, midwives believe they should be providing tobacco dependence treatment, yet they identify a need for training. The findings of this study also indicate that sustained practice change, which includes the entire practice environment, should be targeted to enhance tobacco dependence treatment. The Florida Tobacco Control Program (FTCP), a youth-led marketing and grassroots campaign launched in Florida in 1998, provides the opportunity to examine the effects of program-related newspaper coverage on youth smoking behavior. Articles were collected from Florida daily newspapers, content analyzed to identify the prevalence of topic-specific coverage, and merged by county and year to the Florida Youth Tobacco Survey, a statewide, representative survey of Florida middle school and high school students administered between 1998 and 2002. Trend comparisons and logistic regression models, controlling for alternate explanations, show that FTCP newspaper coverage (particularly coverage of youth advocacy efforts) contributed to observed declines in current smoking. La publicité de service public fait partie intégrante du mouvement de lutte contre le tabagisme depuis le début, mais l'environnement médiatique extrêmement concurrentiel d'aujourd'hui a contraint les promoteurs de la santé à regarder au-delà des messages d'intérêt public pour développer des programmes de communication globale. Le plaidoyer médiatique - utiliser les médias pour affiner la sensibilisation du public et façonner les politiques publiques pour servir l'intérêt public, une technique dérivée des campagnes politiques - apparaît comme un outil puissant dans le mouvement de lutte contre le tabagisme. Son objectif est de changer l'ensemble du contexte social de l'usage du tabac en Amérique, plutôt que le comportement tabagique des gens. Parce que le succès des défenseurs des médias dépend de leur accès aux médias, ils doivent être capables à la fois de créer des nouvelles et de réagir rapidement aux dernières nouvelles et aux événements inattendus. La nature opportuniste et risquée du plaidoyer médiatique exige que la plupart des efforts soient déployés aux niveaux national et local. Un nombre croissant de services de santé des États et d'autres organisations utilisent la publicité payante pour améliorer la fréquence et la portée des messages antitabac. La recherche vérifie que les campagnes médiatiques payantes augmentent l'exposition du public cible aux messages de lutte contre le tabagisme, mais la planification et la réalisation d'achats médiatiques efficaces nécessitent de la sophistication et, bien sûr, les fonds nécessaires. The total population of licensed Ohio nurse-midwives (n = 300) was surveyed by using a cross-sectional survey design. Only 4 of 10 items that assessed a nationally recommended method of promoting smoking cessation were done by 50% or more of respondents. Few nurse-midwives identified barriers to counseling pregnant patients who smoked, but the most common were lack of time (14%) and not knowing where to send pregnant smokers for treatment (14%). Most respondents believed that nicotine replacement therapy (NRT) would be most likely to reduce the number of pregnant smokers (74%), yet few (26%) were confident in their ability to prescribe/recommend nicotine replacement therapy. Respondents more likely to use 5 A's can be characterized as the following: had higher efficacy expectations in their ability to communicate issues about the 5 A's and had higher outcome expectations regarding the effects of using the 5 A's. Nurse-midwife training programs and continuing education needs to include current research in nicotine replacement therapy use in pregnant patients and 5 A's techniques to assist pregnant smokers in quitting. Cinquante fumeurs ont été recrutés dans un établissement de soins primaires de la communauté du Midwest, desservant une population non assurée, sous-assurée et Medicaid. Les participantes ont rempli un questionnaire avant l'intervention et onze femmes ont choisi de participer aux sessions de six semaines. Il y avait peu de différences entre le groupe de contrôle (non participants) et le groupe d'intervention. Le groupe d'intervention avait des scores significativement plus élevés d'intention de quitter. L'analyse de régression logistique a été utilisée pour prédire l'appartenance à un groupe avec l'âge, l'auto-efficacité, l'optimisme, le soutien social, la dépendance à la nicotine, l'intention de cesser de fumer et les autres fumeurs à la maison comme variables prédictives. Les deux seuls prédicteurs significatifs de la participation à l'intervention de sevrage tabagique étaient l'auto-efficacité et l'intention de cesser de fumer. Plus le score d'intention de cesser de fumer est élevé et plus le score d'auto-efficacité est bas (la conviction qu'il est possible de réussir à ne pas fumer); plus ces femmes étaient susceptibles de participer à l’intervention de groupe. Les résultats sont discutés en termes de pertinence clinique et de recherche future dans le domaine.
