La fumée de tabac contient plusieurs milliers de substances chimiques, toxiques à la fois pour la femme enceinte, le fœtus et le nourrisson. La substance la plus dangereuse pour le fœtus est le monoxyde de carbone. La nicotine pure n’est pas exempte de risques, mais sa toxicité est négligeable par rapport à celle de la consommation de tabac. Beaucoup de femmes enceintes sont motivées à l’arrêt, il est donc utile de profiter de cette motivation pour instaurer l’arrêt du tabac le plus précocement possible, et le maintenir en post-partum pour protéger le nourrisson du tabagisme passif. Si l’approche psychologique seule ne permet pas d’obtenir l’arrêt (forte dépendance), un traitement nicotinique de substitution doit être institué rapidement. Bien que non recommandé actuellement chez la femme enceinte, le bupropion devrait être considéré lorsque les contre-indications sont bien prises en compte. La mise en place d’un registre permettant aux praticiens de partager leurs expériences de prises en charge pharmacologiques devrait encourager les professionnels de santé à y recourir plus fréquemment et lever les barrières psychologiques de leur utilisation dans cette population. Design: Cohort prospective study.
Setting: University ART program in Chile.
Patient(s): One hundred sixty-six couples seeking pregnancy through ART.
Intervention(s): Follicular fluid (FF) and serum cotinine concentrations were measured in female partners. Self-reported CS data were collected through personal interviews.
Main Outcome Measure(s): The association between female recent smoking, assessed by FF and serum cotinine concentrations, and ART outcomes, such as number of ova retrieved and implantation rates, and the association between self-reported male recent smoking and live birth rates.
Result(s): A significant age-adjusted association between increased FF cotinine level and decreased number of ova retrieved was found. The male partner’s smoking habit significantly decreased the live birth rate from 21.1% to 7.8%. Serum cotinine concentrations paralleled those of FF.
Conclusion(s): The hypothesis of a detrimental effect of recent female smoking over implantation rates is rejected. However, recent male smoking is associated with significantly decreased live birth rates even after adjusting for confounders. Toxic habits and their relationship with male factor infertility have been a matter of investigation in recent years, and smoking is one of the most common lifestyle toxic exposures to harmful substances. The analysis of sperm deoxyribonucleic acid fragmentation after capacitation detected a detrimental effect produced by tobacco, and this deleterious effect alters the sperm swim-up selection process in smokers, although the molecular and cellular basis of this phenomenon remain to be elucidated.

Cette étude visait à évaluer le rapport coût-efficacité d'un cours de longue durée (12 12 semaines) de la varénicline (en utilisant les avantages de l'abandon du tabac sur les résultats) BENESCO modèle de la cessation du tabagisme.
Méthodes: Les données sur l'efficacité de 12 12 semaines un traitement à la varénicline dans aider le sevrage tabagique ont été analysés conjointement avec les données d'efficacité pendant 12 semaines de la varénicline, le bupropion et le placebo étaient auparavant inclus dans le modèle BENESCO, en utilisant une comparaison mixte de traitement. INTRODUCTION:
Several meta-analyses have examined the efficacy of smoking cessation therapies in the general population. However, little is known about the efficacy of these therapies in cardiac patients. Therefore, a meta-analysis of randomized controlled trials (RCTs) was performed to determine the efficacy of behavioural therapy and pharmacotherapy for smoking cessation in cardiac patients.
METHODS:
The medical literature was systematically reviewed to identify smoking cessation RCTs in cardiac patients. Only RCTs that reported smoking abstinence at six or 12 months were included. Smoking abstinence was examined based on the 'most rigorous criterion', defined as the most conservative outcome reported in any given RCT.
RESULTS:
Eleven behavioural therapy RCTs that enrolled 2105 patients and four pharmacotherapy RCTs that enrolled 1542 patients were identified. RCTs differed in the type of behavioural therapy administered as well as the total length and duration of the intervention. RCTs differed in the type of pharmacotherapy administered (one nicotine patch RCT, one nicotine gum RCT and two bupropion RCTs). Behavioural therapy was associated with a significantly higher proportion of smoking abstinence than usual care (OR 1.97 [95% CI 1.37 to 2.85]). Pharmacotherapies were more efficacious than placebo (pooled OR 1.72 [95% CI 1.15 to 2.57]).
CONCLUSIONS:
Both behavioural therapy and pharmacotherapy are more efficacious than usual care for smoking cessation in cardiac patients. Design: Comparative study.
Setting: Public university and public university hospital.
Patient(s): Seventy-nine men (37 smokers and 42 nonsmokers) who visited an infertility clinic for diagnostic.
Intervention(s) :Tobacco and environmental exposure assessment, semen sample analysis, swim-up procedure, BPDE-DNA adduct immunolabeling.
Main Outcome Measure(s):
BPDE-DNA adduct quantification in selected (SEL-SPZ) and nonselected (NONSEL-SPZ) spermatozoa. Data were normalized by using a normalized fluorescence value (NFV).
Result(s) :
The mean NFV (±SD) in SEL-SPZ was significantly higher in smokers than in nonsmokers (18.9 ± 11.5 vs. 10.5 ± 10.4, respectively). Within smokers, a paired analysis (SEL-SPZ and NONSEL-SPZ) showed that NFV was significantly lower in SEL-SPZ than in NONSEL-SPZ (20.0 ± 11.3 vs. 31.5 ± 16.0, respectively). Conversely, within nonsmokers, the mean NFV was higher in SEL-SPZ than in NONSEL-SPZ (10.3 ± 10.6 vs 4.3 ± 7.1, respectively).
Conclusion(s) :
Tobacco consumption is associated with BPDE-DNA adducts in spermatozoa. In smokers, semen processing by swim-up recovers potentially fertilizing spermatozoa that show a significantly lower amount of BPDE-DNA adducts compared with NONSEL-SPZ. AIM:
To investigate whether varenicline use was associated with increased risk of psychiatric adverse events, compared with bupropion, another drug used for smoking cessation.
DESIGN, SETTING AND PARTICIPANTS:
We conducted a registry-based cohort study in Denmark, 2007-10, comparing new users of varenicline and bupropion in unmatched and 1 : 1 propensity score-matched analyses.
MEASUREMENTS:
Using Cox regression, we estimated the hazard ratio (HR) of any psychiatric adverse event (emergency department visit or in-patient admission with a psychiatric diagnosis) within 30 days following treatment initiation. The unmatched and matched analyses correspond to conventional crude and fully adjusted analyses, respectively.
FINDINGS:
In unmatched analyses, there were 106 (0.18%) psychiatric adverse events among 59 790 varenicline users (rate 22 events per 1000 person-years), compared with 46 (0.26%) events among 17 936 bupropion users (rate 31 per 1000); the HR was 0.69 [95% confidence interval (CI): 0.49-0.98]. In propensity score-matched analyses, 39 (0.22%) events occurred among 17 935 varenicline users (rate 27 per 1000), compared with 46 (0.26%) events among 17 935 bupropion users (rate 31 per 1000); varenicline was not associated with increased risk of psychiatric adverse events (HR 0.85, 95% CI: 0.55-1.30). The overall rate of psychiatric adverse events was substantially higher among participants with a history of psychiatric disorder than in patients without such history; the risk associated with varenicline did not differ significantly by history of psychiatric disorder.
CONCLUSIONS:
