In the circulation of all species, the majority of drug-related material was composed of unchanged varenicline. In all four species, drug-related material was primarily excreted in the urine. A large percentage was excreted as unchanged parent drug (90, 84, 75, and 81% of the dose in mouse, rat, monkey, and human, respectively). Metabolites observed in excreta arose via N-carbamoyl glucuronidation and oxidation. These metabolites were also observed in the circulation, in addition to metabolites that arose via N-formylation and formation of a novel hexose conjugate. Experiments were conducted using in vitro systems to gain an understanding of the enzymes involved in the formation of the N-carbamoylglucuronide metabolite in humans. N-Carbamoyl glucuronidation was catalyzed by UGT2B7 in human liver microsomes when incubations were conducted under a CO2 atmosphere. The straightforward dispositional profile of varenicline should simplify its use in the clinic as an aid in smoking cessation. The total population of licensed Ohio nurse-midwives (n = 300) was surveyed by using a cross-sectional survey design. Only 4 of 10 items that assessed a nationally recommended method of promoting smoking cessation were done by 50% or more of respondents. Few nurse-midwives identified barriers to counseling pregnant patients who smoked, but the most common were lack of time (14%) and not knowing where to send pregnant smokers for treatment (14%). Most respondents believed that nicotine replacement therapy (NRT) would be most likely to reduce the number of pregnant smokers (74%), yet few (26%) were confident in their ability to prescribe/recommend nicotine replacement therapy. Respondents more likely to use 5 A's can be characterized as the following: had higher efficacy expectations in their ability to communicate issues about the 5 A's and had higher outcome expectations regarding the effects of using the 5 A's. Nurse-midwife training programs and continuing education needs to include current research in nicotine replacement therapy use in pregnant patients and 5 A's techniques to assist pregnant smokers in quitting. To examine whether mood, personality and coping predict smoking cessation and whether the associations of personality and coping are mediated through depressed mood. Setting Multicenter (n = 8) smoking cessation trial. Participants A total of 600 smokers ( 15 cigarettes/day) without current depressionwho participated in a smoking cessation study.
Measurements
The outcome was continuous abstinence during the last 4 weeks of the 3-month trial: depressed mood was measured by the Beck Depression Inventory (BDI), personality by the Revised NEO Personality Inventory (NEO-PI-R) and coping by the Revised Ways of Coping Checklist (RWCC).
Findings
A total of 14.7% (88/600) were abstainers. Controlling for potential confounders, baseline BDI independently predicted smoking cessation. Smokers with BDI 10 were less likely to quit than those with BDI < 10 (odds ratio: 6.39, 95% CI: 1.44–28.3, P = 0.01). Compared to BDI < 10 smokers, BDI 10 smokers had significantly higher scores for neuroticism and lower scores for extraversion and conscientiousness (NEO-PI-R). On the RWCC, BDI 10 smokers scored higher for blame self, wishful thinking and problem avoidance and they scored lower on problem focus than smokers with BDI < 10. A mediational analysis showed that neither personality traits nor coping skills predicted directly smoking cessation. However, low level of problem focusing and social support seeking predicted a negative outcome via depressed mood.
Conclusion
A BDI score 10, even in smokers who do not meet a current diagnosis of major depression, directly predicts inability to quit. Setting: Shamsuipo district, Hong Kong. Subject: A total of 1,318 elderly were interviewed (response rate = 83%).
Measures: A structured questionnaire was used for data collection. The questionnaire sought information on the subject’s socio- demographic background and smoking habits. The predictors for successful quitting and intention to quit were assessed by chi-square tests and multiple logistic regres- sion.
Results: Of the respondents, 20.2% were current smokers, 25.4% were ex-smokers, and 54.4% were never smokers. Of the smoker (current and ex-smokers) respondents, 55.7% (335/601) had successfully quit at the time of enumeration. The predictors of quitting were living with others, receiving assistance for mobility, being nondrinkers, smoking for shorter duration, and smoking more cigarettes per day. Having health problems in the past, smoking for shorter duration, and smoking fewer cigarettes per day were predictors of intention to quit smoking.
Conclusion: The study identified several predictors of successful quitting that could help improve the provision of current smoking cessation services. Population-based smoking cessation programs, especially those targeted to elderly, should take these predictors into consideration in the design of interventions.

The primary objectives of this double-blind, placebo-controlled, single-dose, dose-escalation study were to determine the clinical pharmacology of single doses of varenicline in healthy smokers and nonsmokers under fed and fasted conditions and to determine the clinical pharmacology of varenicline administered in the morning and in the evening to smokers. Within each subject group, 4 subjects were randomized to varenicline and 2 subjects to placebo. Subjects received one single oral administration of varenicline or placebo: 6 doses (0.01, 0.03, 0.1, 0.3, 1.0, and 3.0 mg) were investigated in nonsmokers and 7 doses in smokers (0.01, 0.03, 0.1, 0.3, 1.0, 3.0, and 10.0 mg). Varenicline was well tolerated after single doses up to 3.0 mg in smokers and 1.0 mg in nonsmokers. Nausea and vomiting at doses above 3.0 mg in smokers and 1.0 mg in nonsmokers were dose limiting. Systemic exposure to varenicline and pharmacokinetic variability were similar between smokers and nonsmokers. Coadministration with food, smoking restriction, and time-of-day dosing did not affect the pharmacokinetics of varenicline. In total, 65 questionnaires were completed (52.42% return rate). Health visitors, midwives and practice nurses were aware of the reasons why pregnant women smoke as well as the health risks associated with smoking. All claimed to intervene by asking if their patients smoked. Most respondents claimed to record smoking status on health records and to give cessation advice to their patients. However, fewer of the health professionals provided advice to partners of women in their care and only a minority had read the NICE clinical guidelines on nicotine replacement therapies. There were limitations to the health professionals’ attitudes, knowledge and practice that could affect the quality of smoking cessation service offered to patients. Varenicline tartrate is a novel, selective nicotinic receptor partial agonist developed specifically for smoking cessation. This study evaluated the efficacy, tolerability, and safety of 3 varenicline doses for smoking cessation. Bupropion hydrochloride was included as an active control.
Methods A phase 2, multicenter, randomized, double-blind, placebo-controlled study of healthy smokers (18-65 years old). Subjects were randomized to varenicline tartrate, 0.3 mg once daily (n = 128), 1.0 mg once daily (n = 128), or 1.0 mg twice daily (n = 127), for 6 weeks plus placebo for 1 week; to 150-mg sustained-release bupropion hydrochloride twice daily (n = 128) for 7 weeks; or to placebo (n = 127) for 7 weeks.
Results During the treatment phase, the continuous quit rates for any 4 weeks were significantly higher for varenicline tartrate, 1.0 mg twice daily (48.0%; P<.001) and 1.0 mg once daily (37.3%; P<.001), than for placebo (17.1%). The bupropion rate was 33.3% (P = .002 vs placebo). The carbon monoxide–confirmed continuous quit rates from week 4 to week 52 were significantly higher in the varenicline tartrate, 1.0 mg twice daily, group compared with the placebo group (14.4% vs 4.9%; P = .002). The bupropion rate was 6.3% (P = .60 vs placebo). Discontinuation owing to treatment-emergent adverse events was 15.9% for bupropion, 11.2% to 14.3% for varenicline, and 9.8% for placebo. No dose-related increases occurred in adverse events for varenicline.
Conclusions Varenicline tartrate demonstrated both short-term (1 mg twice daily and 1 mg once daily) and long-term efficacy (1 mg twice daily) vs placebo. Varenicline was well tolerated and may provide a novel therapy to aid smoking cessation.
Cigarette smoking remains the world's leading cause of preventable death,1 contributing to 5 million premature deaths in 2000,2 which is estimated to increase to 10 million by 2020.1 Surveys show that most smokers want to quit,3 but most attempts are unaided, with success rates of only 3% to 5% at 1 year.3 Current pharmacotherapies, such as nicotine replacement therapy (NRT), bupropion hydrochloride, and nortriptyline hydrochloride, have shown moderate success, typically doubling short-term quit rates vs placebo,4- 7 with success at 1 year averaging approximately 7% to 30%, depending on the level of adjunctive behavioral counseling.8,9 Consequently, additional, more efficacious smoking cessation medications are needed.
Varenicline tartrate is a novel, nonnicotine agent developed expressly for smoking cessation. It is a selective nicotinic acetylcholine receptor partial agonist that binds specifically at the α4β2 nicotinic receptor subtype.10 The α4β2 receptor is thought to mediate the rewarding properties of nicotine by modulating the release of dopamine in the nucleus accumbens.11- 13 Cytisine, a plant-derived α4β2 partial agonist used for many years as a smoking cessation aid in eastern Europe,14 provided a structural starting point for the development of the higher-affinity varenicline. The agonist effect of oral varenicline on dopamine release is 35% to 60% of that observed with nicotine,10 theoretically sufficient to attenuate craving and withdrawal without producing its own dependence syndrome. The slower release of dopamine with varenicline compared with smoking would also reduce any potential for abuse.10 Varenicline also has a competitive antagonist effect on nicotine due to a substantially higher affinity for the α4β2 receptor.10 Starting therapy 1 week before the target quit day could potentially lead to at least partial extinction of smoking behavior by blocking the rewarding effects of smoked nicotine.15,16 In addition, the blockade of reward could reduce the chance that a “slip” while still undergoing treatment would lead to a full-blown relapse.
The current study was part of a phase 2 program conducted to select the optimal dose for larger-scale, phase 3 studies. The primary objectives were to assess the efficacy, tolerability, and safety of 3 doses of varenicline administered for 6 weeks. [TABAC] tabagisme:tabagisme passif:tabagisme environnementalIndex. décimale : TA 7.1 Généralités Résumé : The objective of this study was to describe the exposure of nonsmokers in the U.S. population to secondhand smoke (SHS) using serum cotinine concentrations measured over a period of 14 years, from October 1988 through December 2002. This study consists of a series of National Health and Nutrition Examination Surveys (NHANES) measuring serum cotinine as an index of SHS exposure of participants. Study participants were individuals representative of the U.S. civilian, noninstitutionalized population, ≥ 4 years of age. We analyzed serum cotinine and interview data from NHANES obtained during surveys conducted during four distinct time periods. Our results document a substantial decline of approximately 70% in serum cotinine concentrations in non-smokers during this period. This decrease was reflected in all groups within the population regardless of age, sex, or race/ethnicity. The large decrease that we observed in serum cotinine concentrations suggests a substantial reduction in the exposure of the U.S. population to SHS during the 1990s. The exposure of nonsmokers to SHS represents an important public health concern. Il prévoit ainsi éliminer certaines exceptions quant à l’usage du tabac dans les lieux fréquentés par le public, réduire le nombre de points de vente et mieux encadrer les détaillants autorisés, ainsi que restreindre ou interdire certaines pratiques promotionnelles. Le projet de loi touchera de près ou de loin tous les établissements commerciaux où il est présentement permis de fumer ou ceux où sont vendus des produits du tabac. Les mesures proposées pourraient affecter positivement ou négativement les revenus des établissements touchés en affectant leurs ventes et la clientèle. Cette étude calcule l’impact économique des mesures proposées en déterminant le nombre d’entreprises touchées et en mesurant, pour différentes catégories d’entreprises, l’impact sur les profits en étudiant comment les mesures affecteront les revenus et les coûts des établissements visés. L’impact de ces mesures est ensuite comparé à certains bénéfices attendus de la réduction anticipée du taux de tabagisme afin de voir si les bénéfices attendus se comparent aux coûts calculés. Le calcul d’impact est basé sur les mesures proposées en date du 21 mars 2005 et pourrait donc ne pas refléter en tout point celles du projet de loi éventuellement déposé. Setting Public houses in three areas in the Republic (intervention) and one area in Northern Ireland (control).
Participants 329 bar staff enrolled in baseline survey; 249 (76%) followed up one year later. Of these, 158 were non-smokers both at baseline and follow-up.
Main outcome measures Salivary cotinine concentration, self reported exposure to secondhand smoke, and respiratory and sensory irritation symptoms.
Results In bar staff in the Republic who did not themselves smoke, salivary cotinine concentrations dropped by 80% after the smoke-free law (from median 29.0 nmol/l (95% confidence interval 18.2 to 43.2 nmol/l)) to 5.1 nmol/l (2.8 to 13.1 nmol/l) in contrast with a 20% decline in Northern Ireland over the same period (from median 25.3 nmol/l (10.4 to 59.2 nmol/l) to 20.4 nmol/l (13.2 to 33.8 nmol/l)). Changes in self reported exposure to secondhand smoke were consistent with the changes in cotinine concentrations. Reporting any respiratory symptom declined significantly in the Republic (down 16.7%, −26.1% to −7.3%) but not in Northern Ireland (0% difference, −32.7% to 32.7%). After adjustment for confounding, respiratory symptoms declined significantly more in the Republic than in Northern Ireland and the decline in cotinine concentration was twice as great.

To explore the self-reported smoking-cessation interventions of maternity staff with pregnant smokers, and their attitudes towards smoking in pregnancy.
DESIGN AND SETTING:
Postal survey of all 137 maternity staff (clinical grades A-H) at a large acute hospital in London; 78 questionnaires were completed (response rate: 57%).
FINDINGS:
Over two-thirds of respondents (71%) reported not advising any pregnant women to give up smoking within the previous 7 days. However, nearly two-thirds (64%) felt that women should not be left to make up their own minds about whether or not to smoke during pregnancy, and even more (81%) agreed or strongly agreed that many pregnant women would like to give up smoking but need help and advice on how to succeed. Helping a pregnant woman to give up smoking was seen as being one of the most important things a midwife can do by 73% of the respondents.
CONCLUSIONS:
Although the reported attitudes were supportive of the midwife's role in smoking cessation, they did not translate into practice. The level of smoking cessation interventions was low.
IMPLICATIONS FOR PRACTICE:
The findings from this study indicate some deficits in knowledge among maternity staff that training may address. PURPOSE:
Smoking is the leading controllable risk factor for heart disease. Only about 69% of U.S. indoor workers are currently covered by a smoke-free workplace policy. This analysis projects the cardiovascular health and economic effects of making all U.S. workplaces smoke free after 1 year and at steady state.
METHODS:
We estimated the number of U.S. indoor workers not covered by smoke-free workplace policies, and the effects of making all workplaces smoke free on smoking behavior and on the relative risks of acute myocardial infarctions and strokes. One-year and steady-state results were calculated using an exponential decline model. A Monte Carlo simulation was performed for a sensitivity analysis.
RESULTS:
The first-year effect of making all workplaces smoke free would produce about 1.3 million new quitters and prevent over 950 million cigarette packs from being smoked annually, worth about 2.3 billion dollars in pretax sales to the tobacco industry. In 1 year, making all workplaces smoke free would prevent about 1500 myocardial infarctions and 350 strokes, and result in nearly $60 [corrected] in savings in direct medical costs. At steady state, 6250 myocardial infarctions and 1270 strokes would be prevented, and $279 million [corrected] would be saved in direct medical costs annually. Reductions in passive smoking would account for 60% of effects among acute myocardial infarctions.
CONCLUSION:
Making all U.S. workplaces smoke free would result in considerable health and economic benefits within 1 year. Reductions in passive smoking would account for a majority of these savings. Cigarette smoking—highly prevalent during pregnancy—is associated with addiction and withdrawal syndrome in adults. We conducted a prospective, two-group parallel study on 17 consecutive newborns of heavy-smoking mothers and 16 newborns of nonsmoking, unexposed mothers (controls). Neurologic examinations were repeated at days 1, 2, and 5. Finnegan withdrawal score was assessed every 3 h during their first 4 d. Newborns of smoking mothers had significant levels of cotinine in the cord blood (85.8 ± 3.4 ng/mL), whereas none of the controls had detectable levels. Similar findings were observed with urinary cotinine concentrations in the newborns (483.1 ± 2.5 μg/g creatinine versus 43.6 ± 1.5 μg/g creatinine; p = 0.0001). Neurologic scores were significantly lower in newborns of smokers than in control infants at days 1 (22.3 ± 2.3 versus 26.5 ± 1.1; p = 0.0001), 2 (22.4 ± 3.3 versus 26.3 ± 1.6; p = 0.0002), and 5 (24.3 ± 2.1 versus 26.5 ± 1.5; p = 0.002). Neurologic scores improved significantly from day 1 to 5 in newborns of smokers (p = 0.05), reaching values closer to control infants. Withdrawal scores were higher in newborns of smokers than in control infants at days 1 (4.5 ± 1.1 versus 3.2 ± 1.4; p = 0.05), 2 (4.7 ± 1.7 versus 3.1 ± 1.1; p = 0.002), and 4 (4.7 ± 2.1 versus 2.9 ± 1.4; p = 0.007). Significant correlations were observed between markers of nicotine exposure and neurologic-and withdrawal scores. Methods: A cross-sectional national survey of a random sample of 6,021 Israeli residents over the age of 21 were interviewed by telephone. The sample consisted of 4248 Jews, 858 Arabs and 915 immigrants, 856 of them were from the former Soviet Union.
Results: Compared to the Jewish population the Jews who immigrated from the former Soviet Union began smoking at an earlier age and the Arabs started smoking at an older age. The young respondents in all ethnic groups reported starting to smoke at a younger age compared to the older respondents, and there was a decrease in smoking initiation at an older age. Father smoking during childhood predicted earlier age of smoking initiation but not mother or sibling smoking, significantly among Jews and immigrants and non-significantly among Arabs.
Conclusions: Age of smoking initiation is dependent on the ethnic background in which the smokers grow up, however, the influence of the father smoking seems to be similar in all population groups. Promotion and implementation of a smoke-free work environment therefore fall under the ILO’s mandate to create healthy and safe workplaces. This working paper, based to a large extent on an informal survey carried out by ILO SafeWork in 2003, sets out to provide an overview of attitudes, legislation and practices related to smoke-free workplaces in different parts of the world. It could serve as a background paper for further ILO tripartite discussions leading up to a new international instrument.
Because of the rapidly changing nature of the debate and legal frameworks related to workplace smoking, the overview will need to be reviewed and periodically updated. Such exercises may require pooling more examples of enterprise and trade union policies and programmes and undertaking a thorough analysis of existing legislation, policies and best practices.
The working paper has been written within the general framework of preventing psychosocial problems at work.
