Young people who become addicted to nicotine are at greater risk of becoming lifelong tobacco consumers. The use of nicotine-delivering electronic cigarettes has risen dramatically among youths worldwide. In addition to physical dependence, adolescents are susceptible to social and environmental influences to use electronic cigarettes. The product design, flavours, marketing, and perception of safety and acceptability have increased the appeal of electronic cigarettes to young people, thus leading to new generations addicted to nicotine. Moreover, there is growing evidence that electronic cigarettes in children and adolescents serve as a gateway to cigarette
smoking. There can be no argument for harm reduction in children. To protect this vulnerable population from electronic cigarettes and other nicotine delivery devices, we recommend that electronic cigarettes be regulated as tobacco products and included in smoke-free policies. Sale of electronic cigarettes should be
barred to youths worldwide. Flavouring should be prohibited in electronic cigarettes, and advertising accessible by youths and young adults be banned. In order to support the process of opinion formation on this issue, the German Cancer Research Center (Deutsches Krebsforschungszentrum, DKFZ) has issued the present report, which evaluates currently available studies on product quality of electronic cigarettes, potential health implications and their usefulness as tobacco cessation aids. The German Cancer Research Center welcomes the suggestion made by the European Commission to regulate electronic cigarettes as medicinal products. For one thing, regulation as medicinal products guarantees good product quality and for another, the products would thus be available for smokers who do not manage to quit by other means. Thus, electronic cigarettes are either used as an alternative to smoking to reduce tobacco use for harm reduction or as a new smoking cessation aid. Health advocates have been reluctant to endorse a harm reduction approach out of fear that tobacco companies cannot be trusted to produce and market products that will reduce the risks associated with tobacco use. Recently, companies independent of the tobacco industry introduced electronic cigarettes, devices that deliver vaporized nicotine without combusting tobacco. We review the existing evidence on the safety and efficacy of electronic cigarettes. We then revisit the tobacco harm reduction debate, with a focus on these novel products. We conclude that electronic cigarettes show tremendous promise in the fight against tobacco-related morbidity and mortality. By dramatically expanding the potential for harm reduction strategies to achieve substantial health gains, they may fundamentally alter the tobacco harm reduction debate. Methods: A total of 160 participants were divided into four groups according to the type of product used, including non-smokers, users of conventional combustible tobacco (cigarettes), heated tobacco, and electronic, tobacco-free vapor products (e-cigarettes). Buccal mucosa samples were analyzed using the micronucleus cytome assay
to assess cytotoxic and genotoxic damage.
Results: E-cigarette users showed significantly higher values for all tested parameters in the micronucleus test compared to non-smokers (p < 0.05). Similarly, users of tobacco heating products showed an increase in all parameters (p < 0.05), with the exception of the number of cells with micronuclei. Conventional cigarette smokers showed a notable increase in the number of binucleated cells and cells with karyorrhexis and karyolysis (p ≤ 0.05). When assessing the differences between users of traditional combustible tobacco products and non-combustible alternatives, these did not appear to be significant, except for e-cigarette users, who had significantly more cells with condensed chromatin (p ≤ 0.001), while users of tobacco heating products had more pyknotic cells (p ≤ 0.001).
Conclusion: The results of this study underscore the heightened occurrence of cytotoxic and genotoxic damage in users of both conventional combustible tobacco products and non-combustible alternatives compared to non-smokers, emphasizing the detrimental impact of these products on the oral mucosa.

Le tabagisme, responsable de 8 millions de décès par an dans le monde, est une cause de morbi-mortalité majeure. En Belgique, un belge sur 4 consomme du tabac. Bien que controversé au niveau de l’efficacité et de la sécurité, la cigarette électronique semble de plus en plus utilisée dans le sevrage tabagique. Les objectifs de ce travail sont d’évaluer les avis, les connaissances et la place que lui réserve les médecins généralistes dans leur pratique.
Matériels et méthodes
Une revue de littérature a permis l’élaboration d’un questionnaire en ligne qui a été adressé aux médecins généralistes par envoi électronique via le département de médecine générale de l’Université Libre de Bruxelles. Les résultats ont finalement été analysés par statistiques descriptives simples grâce aux outils de traitement de données « Google Form » et « Excel ».
Résultats
Sur les 47 réponses obtenues, près de la moitié (44,7 %) des participants semblent utiliser la cigarette électronique dans le sevrage tabagique, le plus souvent sur demande du patient (près de ¾) ou en 2ème intention après échec du 1er traitement (47,6 %) et le plus souvent sous forme d’utilisation exclusive avec une réduction progressive de la nicotine du liquide (85,7 %) et un suivi médical unanimement rapporté. Les participants lui associent d’autres méthodes de sevrage tels la prise en charge psychologique (93,3 %), des médicaments (86,7 %) et les patchs nicotiniques (80%). Les raisons invoquées pour la non-utilisation de la cigarette électronique sont le manque d’étude sur la toxicité (65,4 %) et l’efficacité (53,8 %), le manque de connaissances personnelles (46,2 %) et l’absence de positionnement des autorités (38,5 %). La quasi-totalité des participants (91,5 %) s’estiment insuffisamment informés pour pouvoir guider et conseiller les patients.
Conclusion
La cigarette électronique, dont l’innocuité et l’efficacité dans le sevrage tabagique restent à étudier plus amplement, semble de plus en plus populaire. Par manque de connaissance, l’accompagnement par le médecin généraliste dans le choix de l’appareil, du liquide ou la gestion de la nicotine parait insuffisant, ce qui pourrait être préjudiciable pour le patient. This study explored whether HTP retailer proximity and density to secondary schools were associated with youth use of HTPs in four Canadian provinces.
Methods: An online search between November 2020 and March 2021 identified retailers selling IQOS devices and HEETS (tobacco sticks used in IQOS) within 500 m, 1000 m and 1500 m radius circular buffer zones around high schools (N = 120) participating in the COMPASS study in 2020–2021. Retailer proximity/density data were linked to crosssectional student-level data (N = 40 636 students), and multilevel regression models examined the association between HTP retailer proximity and density and current HTP use, controlling for relevant covariates.
Results: While only 10.0% of schools had at least one retailer selling IQOS devices within 1000 m of the school, 65.0% of schools had at least one retailer selling HEETS. The school a student attended accounted for 23.7% of the variability in the likelihood of currently using an HTP. However, HTP retailer proximity to and density around schools were not significantly associated with the likelihood of students currently using HTPs.
Conclusion: While the school a student attended accounted for a significant amount of variability in HTP use, these findings suggest that students may be obtaining HTPs through other, non-retail sources.
