Les résultats probants de l’expérience sur les fumeurs et les méthodes employées, ouvrent (a) sur une réflexion sur l’intervention du pharmacien sur les déterminants des comportements de santé des patients et notamment dans une composante motivationnelle, et (b) sur les prémisses d’une nouvelle acception du rôle de « conseil » du pharmacien. De « médiateur » entre une prescription et/ou une demande précise où le conseil consiste à expliquer le comportement adéquat à adopter par le patient, nous passons à un « conseil officinal centré sur la personne », basé sur le recours à des techniques de « communication thérapeutique » offrant une connaissance du patient et du contexte dans lequel il évolue, venant enrichir la connaissance de ses pathologies et thérapeutiques. Reste alors à envisager les modalités financières et organisationnelles de ces évolutions, qui ont des retentissements positifs dans l’accompagnement des patients fumeurs. Molécules diminuant le sevrage : après la clonidine, un grand intérêt se développe actuellement pour les substances qui peuvent interférer avec les effets libérateurs de dopamine et de noradrénaline. C'est ainsi que le buproprion, un antidépresseur est devenu un point de mire intéressant dans les recherches actuelles sur la cessation tabagique. Self-help ou encadrement ? Les chiffres d'efficacité varient selon la démarche choisie. La forte médicalisation de la population belge rend possible le recours par le médecin au programme d'intervention minimale ou A.A.A.A. lequel permet de 'situer' le fumeur dans sa démarche. This review compared the effect of high-dose nicotine replacement therapy (NRT) and combinations of NRT for increasing smoking abstinence rates compared to standard-dose NRT patch, varenicline, and bupropion on smoking abstinence.
METHODS:
Ten electronic databases were searched (up to January 2012) for randomized controlled trials (RCT) of standard-dose (≤ 22 mg) or high-dose nicotine patch therapy (> 22 mg), combination NRT (e.g. nicotine patch + nicotine inhaler), bupropion, and varenicline. Analysis consisted of random-effects pairwise meta-analysis and a Bayesian multiple treatment comparison (MTC).
RESULTS:
We identified 146 RCTs (65 standard-doses of the nicotine patch (≤ 22 mg); 6 high-dose NRT patch (> 22 mg); 5 high versus standard-dose NRT patch; 5 combination NRT versus inert controls; 6 combination versus single NRT patch; 48 bupropion; and 11 varenicline). The MTC found that all therapies offered treatment benefits at most time points over controls. Combination NRT and higher-dose NRT did not demonstrate consistent effects over other interventions. With the exception of varenicline, the benefits of treatments over standard-dose NRT were not retained in the long term.
CONCLUSIONS:
All pharmacologic treatments were significantly more effective than inert controls. Varenicline was the only treatment demonstrating effects over other options. La nicotine en est la principale coupable. L'objectif recherché par les méthodes pharmacologiques d'aide à l'arrêt du tabac sera donc de tenter de minimiser ce qu'on appelle le 'syndrome de privation', c'est-à-dire les symptômes que vous ressentez si vous arrêtez de fumer brutalement. Ces symptômes peuvent être d'ordre psychique (nervosité, irritabilité, envie irrésistible de fumer, boulimie...) ou s'exprimer dans votre corps (crampes digestives, tremblements,...). La plupart des traitements vont donc chercher à stimuler les circuits cérébraux de récompense qui étaient sous l'influence de la nicotine. Cela peut affecter le traitement médicamenteux par des mécanismes pharmacocinétiques et pharmacodynamiques. Les enzymes induites par le tabagisme peuvent également augmenter le risque de cancer en renforçant l'activation métabolique des substances cancérogènes. Les hydrocarbures aromatiques polycycliques contenus dans la fumée de tabac seraient responsables de l'induction du cytochrome P450 (CYP) 1A1, CYP1A2 et éventuellement du CYP2E1, le CYP1A1 étant principalement une enzyme extrahépatique présente dans les poumons et le placenta. Il existe des polymorphismes génétiques dans l'inductibilité du CYP1A1, et certaines indications indiquent qu'une inductibilité élevée est plus fréquente chez les patients atteints d'un cancer du poumon. Le CYP1A2 est une enzyme hépatique responsable du métabolisme de plusieurs médicaments et de l'activation de certains procarcinogènes. Déméthylation de la caféine, en utilisant les données de clairance sanguine ou de métabolites dans l'urine, Utilisé comme marqueur in vivo de l'activité du CYP1A2, démontrant clairement un effet du tabagisme, le CYP2E1 métabolise un certain nombre de médicaments et active certains cancérogènes. Notre laboratoire a découvert dans une étude intra-individuelle que la cigarette augmentait de manière significative l'activité du CYP2E1 mesurée par la clairance de la chlorzoxazone. Lors d'études chez l'animal, la nicotine induit l'activité de plusieurs enzymes dans le cerveau, notamment le CYP2E1, le CYP2A1 / 2A2 et le CYP2B1 / 2B2, mais on ignore si cet effet est cliniquement significatif. De même, bien que des effets inhibiteurs du monoxyde de carbone et du cadmium sur les enzymes CYP aient été observés in vitro et chez l'animal, la pertinence de cette inhibition chez l'homme n'a pas encore été établie. Le mécanisme impliqué dans la plupart des interactions entre le tabagisme et les drogues implique l’induction du métabolisme. Les médicaments pour lesquels le métabolisme induit par la cigarette peuvent avoir des conséquences cliniques comprennent la théophylline, la caféine, la tacrine, l'imipramine, l'halopéridol, la pentazocine, le propranolol, le flécaïnide et l'estradiol. Le tabagisme entraîne une clairance plus rapide de l'héparine, probablement liée à l'activation de la thrombose liée au tabagisme avec une liaison accrue de l'héparine à l'antithrombine III. La vasoconstriction cutanée par la nicotine peut ralentir le taux d'absorption d'insuline après une administration sous-cutanée. Des interactions pharmacodynamiques ont également été décrites. Le tabagisme est associé à une diminution de l’ampleur de la pression artérielle et à une diminution de la fréquence cardiaque au cours du traitement par les bêta-bloquants. moins de sédation des benzodiazépines et moins d'analgésie de certains opioïdes, reflétant probablement les effets des actions stimulantes de la nicotine. L'impact de la cigarette doit être pris en compte lors de la planification et de l'évaluation des réponses à la pharmacothérapie. Smoking remains the leading preventable cause of premature deaths. Several pharmacological interventions now exist to aid smokers in cessation. These include Nicotine Replacement Therapy [NRT], bupropion, and varenicline. We aimed to assess their relative efficacy in smoking cessation by conducting a systematic review and meta-analysis.
METHODS:
We searched 10 electronic medical databases (inception to Sept. 2006) and bibliographies of published reviews. We selected randomized controlled trials [RCTs] evaluating interventions for smoking cessation at 1 year, through chemical confirmation. Our primary endpoint was smoking cessation at 1 year. Secondary endpoints included short-term smoking cessation (approximately 3 months) and adverse events. We conducted random-effects meta-analysis and meta-regression. We compared treatment effects across interventions using head-to-head trials and when these did not exist, we calculated indirect comparisons.
RESULTS:
We identified 70 trials of NRT versus control at 1 year, Odds Ratio [OR] 1.71, 95% Confidence Interval [CI], 1.55-1.88, P =< 0.0001). This was consistent when examining all placebo-controlled trials (49 RCTs, OR 1.78, 95% CI, 1.60-1.99), NRT gum (OR 1.60, 95% CI, 1.37-1.86) or patch (OR 1.63, 95% CI, 1.41-1.89). NRT also reduced smoking at 3 months (OR 1.98, 95% CI, 1.77-2.21). Bupropion trials were superior to controls at 1 year (12 RCTs, OR1.56, 95% CI, 1.10-2.21, P = 0.01) and at 3 months (OR 2.13, 95% CI, 1.72-2.64). Two RCTs evaluated the superiority of bupropion versus NRT at 1 year (OR 1.14, 95% CI, 0.20-6.42). Varenicline was superior to placebo at 1 year (4 RCTs, OR 2.96, 95% CI, 2.12-4.12, P =< 0.0001) and also at approximately 3 months (OR 3.75, 95% CI, 2.65-5.30). Three RCTs evaluated the effectiveness of varenicline versus bupropion at 1 year (OR 1.58, 95% CI, 1.22-2.05) and at approximately 3 months (OR 1.61, 95% CI, 1.16-2.21). Using indirect comparisons, varenicline was superior to NRT when compared to placebo controls (OR 1.66, 95% CI 1.17-2.36, P = 0.004) or to all controls at 1 year (OR 1.73, 95% CI 1.22-2.45, P = 0.001). This was also the case for 3-month data. Adverse events were not systematically different across studies.
CONCLUSION:
NRT, bupropion and varenicline all provide therapeutic effects in assisting with smoking cessation. Nicotine replacement therapy (NRT) and varenicline are both effective in helping smokers quit. There is growing interest in combining the two treatments to improve treatment outcomes, but no experimental data exist on whether this is efficacious. This double-blind randomised controlled trial was designed to evaluate whether adding nicotine patches to varenicline improves withdrawal relief and short-term abstinence rates.
Methods
117 participants seeking help to stop smoking were randomly allocated to varenicline plus placebo patch or varenicline plus nicotine patch (15 mg/16 hour). Varenicline use commenced one week prior to the target quit date (TQD), patch use started on the TQD. Ratings of urges to smoke and cigarette withdrawal symptoms were collected weekly over 4 weeks post-TQD. Medication use and smoking status were established at 1, 4 and 12 weeks. Participants lost to follow-up were included as continuing smokers.
Results
92% of participants used both medications during the first week after the TQD. The combination treatment generated no increase in nausea or other adverse effects. It had no overall effect on urges to smoke or on other withdrawal symptoms. The combination treatment did not improve biochemically validated abstinence rates at 1 week and 4 weeks post-TQD (69% vs 59%, p=0.28 and 60% vs 59%, p=0.91, in the nicotine patch and placebo patch arm, respectively), or self reported abstinence rates at 12 weeks (36% vs. 29%, p=0.39, NS).
Conclusions

Depuis quelques années en Belgique, la profession de pharmacien d’officine ouverte au public a considérablement évolué. Dans ce contexte, de nombreuses activités pharmaceutiques ont vu le jour afin d’optimiser le bon usage des médicaments délivrés au patient. En voici quelques exemples : le dossier pharmaceutique partagé, le pharmacien de référence, les entretiens d’accompagnement de bon usage du médicament,… La prestation « Entretien d’accompagnement de Bon Usage des Médicaments » comprend la dispensation structurée et documentée d’informations et l’analyse des attentes et expériences du patient. Lors d’un premier entretien d’information, les points suivants sont abordés : effet du médicament et place dans le traitement, importance de l’usage correct et d’une bonne adhérence au traitement, motivation du patient pour un traitement de longue durée. Ce type d’entretien existe actuellement pour les patients asthmatiques. Le tabagisme étant un problème de santé publique majeur dans notre pays, il serait intéressant de réfléchir si cette structure d’entretien pourrait être adaptée au bon usage des médicaments prescrits dans le cadre de la cessation tabagique et d’en évaluer la pertinence.

Background
Smoking is the leading preventable cause of illness and premature death worldwide. Some medications have been proven to help people to quit, with three licensed for this purpose in Europe and the USA: nicotine replacement therapy (NRT), bupropion, and varenicline. Cytisine (a treatment pharmacologically similar to varenicline) is also licensed for use in Russia and some of the former socialist economy countries. Other therapies, including nortriptyline, have also been tested for effectiveness.
Objectives
How do NRT, bupropion and varenicline compare with placebo and with each other in achieving long-term abstinence (six months or longer)?
How do the remaining treatments compare with placebo in achieving long-term abstinence?
How do the risks of adverse and serious adverse events (SAEs) compare between the treatments, and are there instances where the harms may outweigh the benefits?
Methods
The overview is restricted to Cochrane reviews, all of which include randomised trials. Participants are usually adult smokers, but we exclude reviews of smoking cessation for pregnant women and in particular disease groups or specific settings. We cover nicotine replacement therapy (NRT), antidepressants (bupropion and nortriptyline), nicotine receptor partial agonists (varenicline and cytisine), anxiolytics, selective type 1 cannabinoid receptor antagonists (rimonabant), clonidine, lobeline, dianicline, mecamylamine, Nicobrevin, opioid antagonists, nicotine vaccines, and silver acetate. Our outcome for benefit is continuous or prolonged abstinence at least six months from the start of treatment. Our outcome for harms is the incidence of serious adverse events associated with each of the treatments.
We searched the Cochrane Database of Systematic Reviews (CDSR) in The Cochrane Library, for any reviews with 'smoking' in the title, abstract or keyword fields. The last search was conducted in November 2012. We assessed methodological quality using a revised version of the AMSTAR scale. For NRT, bupropion and varenicline we conducted network meta-analyses, comparing each with the others and with placebo for benefit, and varenicline and bupropion for risks of serious adverse events.
Main results
We identified 12 treatment-specific reviews. The analyses covered 267 studies, involving 101,804 participants. Both NRT and bupropion were superior to placebo (odds ratios (OR) 1.84; 95% credible interval (CredI) 1.71 to 1.99, and 1.82; 95% CredI 1.60 to 2.06 respectively). Varenicline increased the odds of quitting compared with placebo (OR 2.88; 95% CredI 2.40 to 3.47). Head-to-head comparisons between bupropion and NRT showed equal efficacy (OR 0.99; 95% CredI 0.86 to 1.13). Varenicline was superior to single forms of NRT (OR 1.57; 95% CredI 1.29 to 1.91), and to bupropion (OR 1.59; 95% CredI 1.29 to 1.96).
Varenicline was more effective than nicotine patch (OR 1.51; 95% CredI 1.22 to 1.87), than nicotine gum (OR 1.72; 95% CredI 1.38 to 2.13), and than 'other' NRT (inhaler, spray, tablets, lozenges; OR 1.42; 95% CredI 1.12 to 1.79), but was not more effective than combination NRT (OR 1.06; 95% CredI 0.75 to 1.48). Combination NRT also outperformed single formulations. The four categories of NRT performed similarly against each other, apart from 'other' NRT, which was marginally more effective than NRT gum (OR 1.21; 95% CredI 1.01 to 1.46).
Cytisine (a nicotine receptor partial agonist) returned positive findings (risk ratio (RR) 3.98; 95% CI 2.01 to 7.87), without significant adverse events or SAEs.
Across the 82 included and excluded bupropion trials, our estimate of six seizures in the bupropion arms versus none in the placebo arms was lower than the expected rate (1:1000), at about 1:1500. SAE meta-analysis of the bupropion studies demonstrated no excess of neuropsychiatric (RR 0.88; 95% CI 0.31 to 2.50) or cardiovascular events (RR 0.77; 95% CI 0.37 to 1.59). SAE meta-analysis of 14 varenicline trials found no difference between the varenicline and placebo arms (RR 1.06; 95% CI 0.72 to 1.55), and subgroup analyses detected no significant excess of neuropsychiatric events (RR 0.53; 95% CI 0.17 to 1.67), or of cardiac events (RR 1.26; 95% CI 0.62 to 2.56).
Nortriptyline increased the chances of quitting (RR 2.03; 95% CI 1.48 to 2.78). Neither nortriptyline nor bupropion were shown to enhance the effect of NRT compared with NRT alone. Clonidine increased the chances of quitting (RR 1.63; 95% CI 1.22 to 2.18), but this was offset by a dose-dependent rise in adverse events. Mecamylamine in combination with NRT may increase the chances of quitting, but the current evidence is inconclusive. Other treatments failed to demonstrate a benefit compared with placebo. Nicotine vaccines are not yet licensed for use as an aid to smoking cessation or relapse prevention. Nicobrevin's UK license is now revoked, and the manufacturers of rimonabant, taranabant and dianicline are no longer supporting the development or testing of these treatments.
Authors' conclusions
NRT, bupropion, varenicline and cytisine have been shown to improve the chances of quitting. Combination NRT and varenicline are equally effective as quitting aids. Nortriptyline also improves the chances of quitting. On current evidence, none of the treatments appear to have an incidence of adverse events that would mitigate their use.

Long-term SLT use is associated with adverse health consequences. Effective pharmacotherapies are needed to treat SLT users who want to achieve tobacco abstinence. In the current review, we discuss the pharmacological interventions identified in a recent meta-analysis of interventions for SLT users, with inclusion of additional articles identified by searching PubMed up to August 2011. Nicotine replacement therapy (NRT) has been demonstrated to increase short-term tobacco abstinence rates and to alleviate craving and withdrawal symptoms among SLT users trying to quit. Bupropion sustained release has been shown to decrease craving and attenuate post-cessation weight gain among SLT users trying to quit. Varenicline is the only available medication demonstrated to increase long-term (≥6 months) tobacco abstinence rates among SLT users. Overall, findings from studies investigating pharmacotherapies for SLT users have been relatively disappointing. SLT reduction interventions may hold some promise for increasing abstinence rates among SLT users not interested in quitting. Additional investigations of higher dose NRT and combination pharmacotherapy are needed to advance the treatment of SLT users. Varenicline binds with subnanomolar affinity only to alpha4beta2 nAChRs and in vitro functional patch clamp studies in HEK cells expressing nAChRs show that varenicline is a partial agonist with 45% of nicotine's maximal efficacy at alpha4beta2 nAChRs. In neurochemical models varenicline has significantly lower (40-60%) efficacy than nicotine in stimulating [(3)H]-dopamine release from rat brain slices in vitro and in increasing dopamine release from rat nucleus accumbens in vivo, while it is more potent than nicotine. In addition, when combined with nicotine, varenicline effectively attenuates the nicotine-induced dopamine release to the level of the effect of varenicline alone, consistent with partial agonism. Finally, varenicline reduces nicotine self-administration in rats and supports lower self-administration break points than nicotine. These data suggest that varenicline can reproduce to some extent the subjective effects of smoking by partially activating alpha4beta2 nAChRs, while preventing full activation of these receptors by nicotine. Based on these findings, varenicline was advanced into clinical development and recently shown to be an effective and safe aid for smoking cessation treatment. Nicotine dependence is probably mediated through the activation of multiple subtypes of neuronal nicotinic acetylcholine receptor (nAChR), among which the mesolimbic alpha(4)beta(2) subtype has a pivotal role. Here, we discuss the rationale for and the design of alpha(4)beta(2) nAChR partial agonists as novel treatments for tobacco addiction. Such agents are expected to exhibit a dual action by sufficiently stimulating alpha(4)beta(2)-nAChR-mediated dopamine release to reduce craving when quitting and by inhibiting nicotine reinforcement when smoking. Potent and selective alpha(4)beta(2) nAChR partial agonists that exhibit dual agonist and antagonist activity in preclinical models can be identified. [TABAC] tabagismeIndex. décimale : TA 6.2.3 Approche pharmacologique Résumé : Since the 1996 publication of guidelines on smoking cessation from the Agency for Health Care Policy and Research and the American Psychiatric Association, several new treatments have become available, including nicotine nasal spray, nicotine inhaler, and bupropion hydrochloride. In addition, nicotine gum and patch have become available over-the-counter. This article reviews the published literature and US Food and Drug Administration and pharmaceutical company reports on these therapies. Based on this review, clinical logic, and experience, we conclude that pharmacotherapy should be made available to all smokers. All currently available therapies appear to be equally efficacious, approximately doubling the quit rate compared with placebo. Concomitant behavioral or supportive therapy increases quit rates and should be encouraged but not required. Combining patch with gum or patch with bupropion may increase the quit rate compared with any single treatment. Because patient characteristics predictive of success with a particular therapy are not yet known, the best treatment choice for an individual patient should be guided by the patient's past experience and preference and the product's adverse effect profile. This review aims to examine economic evaluations of varenicline, to compare the reported cost-effectiveness of varenicline with that of treatments for major smoking-related diseases and to evaluate the findings for decision making.
METHODS:
A literature search was performed to identify published articles in English indexed in MEDLINE and the Cochrane Library (Issue 1, 2009), which includes the Economic Evaluation Database. Additional sources also were searched to identify unpublished varenicline studies, including conference abstracts. The search for varenicline studies was limited from 2006 to October 2009; searches for all other types of studies were limited from 1990 to October 2009.
RESULTS:
The search yielded a total of 20 relevant economic evaluations of varenicline. In addition, 37 reviews of economic evaluations in chronic obstructive pulmonary disease, non-small cell lung cancer and cardiovascular disease, as well as studies evaluating the impact of economic rewarding were considered in this review. From these identified economic evaluations, the incremental cost-effectiveness ratios for varenicline ranged from dominance (more effective and cost saving) to €18,582 per quality-adjusted life-year (including indirect costs). These estimates appeared substantially lower when compared with incremental cost-effectiveness ratios reported for secondary prevention of smoking-related diseases, which in some cases were as high as €66,218 per quality-adjusted life-year.
CONCLUSIONS:
Varenicline appears to be cost-effective from the perspective of both health care payers and employers, because of reduced health care consumption and costs. Methods: Fourteen thousand seven hundred and forty-four Norwegian men aged between 20 and 50 years were selected at random from a national representative web panel and sent a questionnaire by E-mail. Of the 7,170 (48.6%) who responded, there were 1,775 former and 1,808 current smokers. They were asked about the method they used and the outcome of their last attempt to quit smoking.
Results: In a regression model in which education, number of previous attempts to quit smoking, perception of risk, and age were controlled for, the odds ratio (OR) for reporting total abstinence at the time of the survey was significantly higher for those who had used varenicline (OR = 4.95, p < .006) and snus (OR = 2.68, p < .001) compared with those who had used nicotine chewing gum (reference OR = 1). For smokers who reported that they had tried to quit with the help of snus, 62.4% reported that they still used snus at the time of the survey either daily (43.8%) or occasionally (18.6%). The proportion who still used medicinal nicotine products at the time of the survey was 9.5%.
Discussion: Compared with medicinal nicotine products, snus and varenicline increased the probability of quitting smoking completely, but snus seemed to maintain nicotine dependence.En ligne : https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2910876/ Format de la ressource électronique : HTML.
