Well-documented challenges have hampered both intervention development and research in teen smoking cessation. Addressing these challenges, the Hutchinson Study of High School Smoking (HS Study), the largest group-randomized trial in adolescent smoking cessation to date, incorporates several design innovations to investigate the effect of a counselor-initiated, individually tailored telephone counseling smoking cessation intervention for older adolescents. This paper presents and discusses these innovative design features, and baseline findings on the resulting study population. Method. The trial used a population-based survey to proactively identify and recruit all high school juniors who had smoked in the past month - potentially expanding intervention reach to all smokers, even those who smoked less than daily and those not motivated to quit. For ethical and intervention reasons, some nonsmokers were enrolled in the intervention, also. Other important design features included the random allocation of schools into experimental conditions (intervention vs. no-intervention control) and a multi-wave design. Results and conclusion. The design innovations address problems and challenges identified in adolescent smoking cessation literature. PURPOSE:
To examine the effect of plain packaging on adolescents' perceptions of cigarette packs, attributes of smokers, and expectations of cigarette taste, and to identify the effect of increasing the size of pictorial health warnings on appraisal of plain packs.
METHODS:
We used a 5 (degree of plain packaging and graphic health warning)x 3 (brand type) between-subjects experimental design, using a Web-based methodology to expose adolescents to one randomly selected cigarette pack, during which respondents completed ratings.
RESULTS:
When brand elements such as color, branded fonts, and imagery were progressively removed from cigarette packs, adolescents perceived packs to be less appealing, rated attributes of a typical smoker of the pack less positively, and had more negative expectations of cigarette taste. Pack appeal was reduced even further when the size of the pictorial health warning on the most plain pack was increased from 30% to 80% of the pack face, with this effect apparent among susceptible nonsmokers, experimenters, and established smokers.
CONCLUSIONS:
Removing as much brand information from cigarette packs as possible is likely to reduce positive cigarette brand image associations among adolescents. By additionally increasing the size of pictorial health warnings, positive pack perceptions of those who are at greater risk of becoming regular addicted adult smokers are most likely to be reduced.Permalink : https://biblio.fares.be/opac_css/index.php?lvl=notice_display&id=7956 Exemplaires (1)

The plethora of product devices, formulations, and flavors, combined with the lack of industry standards and labeling requirements, quality control, and limited product oversight, has given rise to public concern about initiation of use and potential for adverse exposure and negative long-term health outcomes. This volume discusses how analytical methods can address these issues and support the manufacturing, labeling, distribution, testing, regulation, and monitoring for consistency of products with known chemical content and demonstrated performance characteristics.
The book begins with the background on aerosol drug delivery services and e-cigarettes, constituents of nicotine-containing liquid dosing formulations, typical use scenarios and associated aerosol emissions, and chemical exposures and pharmacological and toxicological effect profiles, and then continues with descriptions of the analytical methods used to characterize the chemicals in formulations and emissions from e-cigarettes, including their stability, physical particle-size distribution and thermal degradation under commonly employed conditions of use.
Analytical methods enabling detection of biomarkers of exposure and harm in complex biological matrices are discussed, with an emphasis on constituents or emissions of current medicinal interest or with potential to produce harm. Plusieurs hypothèses sur le positionnement des sages-femmes sont abordées, telles que : l’investissement dans la prévention du tabagisme durant la grossesse l’information des femmes enceintes sur les risques du tabagisme durant la grossesse la pratique de la mesure du monoxyde de carbone en routine clinique la formation au dépistage du tabagisme actif et/ou passif ressentie comme insuffisante. Afin de répondre à cette série d’hypothèses, une enquête est réalisée auprès de 160 sages-femmes belges volontaires exerczant en secteur(s) hospitalier(s) public et/ou privé. Cette enquête est réalisée à partir d’un autoquestionnaire qui comporte des questions fermées, à choix multiples et une question rédactionnelle courte à partir d’un cas clinique. Sur 150 questionnaires envoyés, 70 nous ont été retournés. Il semble apparaître qu’à l’heure actuelle, la sage-femme belge a la volonté d’informer la femme enceinte fumeuse des risques encourus, de proposer la mesure du monoxyde de carbone expiré, de prendre en charge ses patients, mais se sentant insuffisamment formée, elle oriente trop souvent vers un autre professionnel de santé. To test the hypothesis that appropriate interventions delivered by midwives within usual primary maternity care, can assist women to stop or reduce the amount they smoke and facilitate longer duration of breast feeding.
DESIGN, SETTING AND PARTICIPANTS:
In a cluster randomised trial of smoking education and breast-feeding interventions in the lower North Island, New Zealand, midwives were stratified by locality and randomly allocated into a control group and three intervention groups. The control group provided usual care. Midwives in the intervention groups delivered either a programme of education and support for smoking cessation or reduction, a programme of education and support for breast feeding or both programmes. Sixty-one midwives recruited a total of 297 women.
INTERVENTIONS:
Structured programmes provided by midwives.
FINDINGS:
Women receiving only the smoking cessation or reduction programme were significantly more likely to have reduced, stopped smoking or maintained smoking changes than women in the control group, at 28 weeks and 36 weeks gestation. Women receiving both the smoking cessation and breast-feeding education and support programmes were significantly more likely than women in the control group to have changed their smoking behaviour at 36 weeks gestation. There was no difference in rates of cessation or reduction between the groups in the postnatal period. Consenting patients (n = 95) and staff (n = 41) concurrently completed a modified form of the S-KAP survey instrument. Ninety-five percent of patients reported currently smoking, and half endorsed wanting "to quit smoking now." This patient desire to quit smoking was significantly underrated by staff compared to the patients themselves (p = .028). Both patients and staff demonstrated suboptimal knowledge of smoking health risks, but 73% of patients reported trying to quit with past pregnancies to avoid harm to the fetus/baby. Although results show that patients could benefit from smoking cessation strategies centered on smoking's fetal/neonatal health risks, organizational interventions that focus on changing staff attitudes about patient desire to quit smoking may first need to be implemented. Electronic cigarettes (e-cigarettes) are often promoted to assist with cigarette smoking cessation. In 2016–2017, the relationship between e-cigarette use and having stopped smoking among ever (current and former) smokers was assessed in the European Union and Great Britain by itself.
Methods:
Cross-sectional logistic regression of the association between being a former smoker and e-cigarette use was applied to the 2014 Eurobarometer survey of 28 European Union countries controlling for demographics.
Results:
Among all ever smokers, any regular ever use of nicotine e-cigarettes was associated with
lower odds of being a former smoker (unadjusted OR¼0.34, 95% CI¼0.26, 0.43, AOR¼0.43, 95% CI¼0.32, 0.58) compared with smokers who had never used e-cigarettes. In unadjusted models, daily use (OR¼0.42, 95% CI¼0.31, 0.56); occasional use (OR¼0.25, 95% CI¼0.18, 0.35); and experimentation (OR¼0.24, 95% CI¼0.19, 0.30) of nicotine e-cigarettes were associated with lower odds of being a former smoker compared with having never used nicotine-containing e-cigarettes. Comparable results were found in adjusted models. Results were similar in Great Britain alone. Among current smokers, daily cigarette consumption was 15.6 cigarettes/day (95% CI¼14.5, 16.7) among those who also used e-cigarettes versus 14.4 cigarettes/day (95% CI¼13.4, 15.4) for those who did not use them (po0.05).
Conclusions:
These results that e-cigarettes are associated with inhibiting rather than assisting in smoking cessation. On the population level, the net effect of the entry of e-cigarettes into the European Union (and Great Britain) is associated with depressed smoking cessation of conventional cigarettes.

This paper examines the effectiveness of the “Smoke-Free Class Competition” in delaying the onset of smoking in adolescence. Each participating class must decide if they want to be a “smoke-free class” for the 6-month period from fall to spring. Classes monitor their (non-)smoking behavior and report it to the teacher regularly. Classes in which pupils refrain from smoking for this period of time participate in a prize draw, in which they can win a number of attractive prizes.
Methods. To evaluate the effectiveness of the competition, a sample of 131 participating and nonparticipating classes (number of pupils 2,142; mean age 12.9 years, SD = 0.98) was compared with regard to their smoking behavior. Smoking status was determined by self-assessment on three occasions: (a) prior to the beginning of the competition, (b) 1 month after the competition, and (c) 1 year after the start of the competition.
Results. From pretest to posttest smoking increased by 7.5% in the comparison group, while it decreased by 0.2% in the intervention group (OR = 2.19; P < 0.001). In the follow-up measurement, a clear increase in smoking prevalence occurs in all groups; however, the pupils in the intervention condition still have a significant lower increase of smoking (OR = 1.45; P < 0.01). Moreover, with regard to the nonsmokers at baseline, pupils in the comparison group showed significantly higher prevalences in smoking than the intervention group in the postmeasurement, 7.8 versus 13.9% (OR = 1.98; P < 0.001), as well as in the in the follow-up-measurement, 17 versus 21.3% (OR = 1.36; P < 0.05).
Conclusions. Materials and methods: HaCaT, UMSCC10B, and HN30 were treated with nicotine-containing and
nicotine-free vapor extract from two popular e-cigarette brands for periods ranging from 48 h to 8 weeks.
Cytotoxicity was assessed using Annexin V flow cytometric analysis, trypan blue exclusion, and clono?genic assays. Genotoxicity in the form of DNA strand breaks was quantified using the neutral comet assay and c-H2AX immunostaining.
Results: E-cigarette-exposed cells showed significantly reduced cell viability and clonogenic survival, along with increased rates of apoptosis and necrosis, regardless of e-cigarette vapor nicotine content. They also exhibited significantly increased comet tail length and accumulation of c-H2AX foci, demon?strating increased DNA strand breaks.
Conclusion: E-cigarette vapor, both with and without nicotine, is cytotoxic to epithelial cell lines and is a DNA strand break-inducing agent. In this study, we describe the prevalence and correlates of youth exposure and engagement with tobacco- and e-cigarette-related social media.
Methods: Data are from the baseline survey of the Texas Adolescent Tobacco and Marketing Surveillance system, a cross-sectional sample of sixth, eighth, and 10th graders (n = 3907, N = 461,097). Weighted logistic regression models were used to examine associations between demographic characteristics, sensation seeking, tobacco use, and exposure and engagement with tobacco-related social media.
Results: Overall, 52.5% of students reported exposure to tobacco-related social media in the past month, whereas < 6% reported engagement. Exposure and some forms of engagement were more common among high school students, girls, those with friends who use tobacco, and high sensation seekers (p < .05). The odds of exposure were significantly higher among students susceptible to combustible tobacco (adjusted odds ratio [AOR] = 1.71, p < .05), e-cigarettes (AOR = 2.10, p < .01), and both combustible tobacco and e-cigarettes (AOR = 2.24, p < .001). The odds of engaging with social media was higher among those who were susceptible to, had ever, or currently use both combustible tobacco and e-cigarettes (AOR = 2.10-3.46, p < .05).
Conclusions: About 1 in every 2 adolescents in Texas are exposed to tobacco-related social media. Adolescents who are susceptible to or use e-cigarettes and/or combustible tobacco are exposed to and engage with tobacco-related social media more than their peers. Social media appears to be an important venue when targeting vulnerable youth in prevention campaigns.

Many nonregulatory interventions targeting children and youth have been implemented at three levels: directed at the individual (e.g., interactive video games), delivered to students at school (e.g., campus bans), and launched in the community (e.g., mass media campaigns). This systematic review aims to synthesize the evidence on the effectiveness of interventions aimed at preventing e-cigarette initiation among children and youth.
Methods
MEDLINE, CINAHL, Embase, APA PsycINFO, and Web of Science Core Collection were searched for papers published between January 1, 2004 and September 1, 2022 that reported more than one outcome on vaping prevention among individuals aged less than 21-years-old: vaping prevalence/incidence, initiation intentions, knowledge/attitudes, and other tobacco product use prevalence/initiation intentions. Interventions were at the individual, school, or community level. The risk of bias was assessed using ROBINS-I and RoB 1.
Results
Thirty-nine publications met the eligibility criteria. Fourteen individually-based (4 parental monitoring, 3 video games, 2 text messages, 3 graphic message themes, 2 healthcare), 19 school-based (14 educational and skill interventions, 5 vape-free policies/bans), and 6 community-based (3 social media, 3 mass media campaigns) interventions were reported. E-cigarette initiation prevention was observed with high perceived parental monitoring; however, the cross-sectional study designs precluded causal claims. There was promising but limited evidence that social-emotional skills curricula and peer leader programming prevented vaping initiation.
Discussion

Although the relationship between tax and price and youth cigarette use is well established, little is known about these associations and youth e-cigarette use. This research examines U.S. youth sensitivity to changes in e-cigarette prices and tax using standardized measures of e-cigarette taxes and prices.
Methods:
This analysis uses national data on past 30-day use and the number of days using e-cigarettes (i.e., the intensity of use) from the repeat cross-sectional 2015−2019 Youth Risk Behavior Survey, in combination with inflation-adjusted standardized e-cigarette price and tax data to understand whether changes in e-cigarette price and tax were associated with changes in e-cigarette use. Two-part demand regression models controlling for demographics and e-cigarette restriction policies
were conducted to calculate price and tax elasticities of demand, in addition to $0.50 and $1.00 price and tax increase simulations.
Results: Increased e-cigarette prices and taxes were associated with significant reductions in past 30-day use. Prices were also significantly associated with decreases in the intensity of use. A $0.50 and $1.00 tax increase leads to a 6.3% and 12.2% decrease in past 30-day use and a 4.7% and 9.3% decrease in intensity, respectively. A $0.50 and $1.00 price increase leads to a 4.1% and 8.2% decrease in past 30-day use and a 4.2% and 8.3% decrease in intensity, respectively.
Conclusions: Higher prices and taxes reduce youth current e-cigarette use and days using e-cigarettes. To evaluate the joint effect of cigarette smoking and alcohol consumption on mortality. Methods. A population-based cohort of 66,743 Chinese men aged 30-89 in Shanghai, China recruited from 1996 to 2000. Lifestyle data were collected using structured questionnaires. As of November 2004, follow-up for the vital status of 64,515 men was completed and death information was further confirmed through record linkage with the Shanghai Vital Statistics Registry. Associations were evaluated by Cox regression analyses. Results. 2514 deaths (982 from cancers, 776 from cardiovascular diseases (CVD)) were identified during 297,396 person-years of follow-up. Compared to never-smokers, both former and current smokers had significantly elevated mortality from any cause, CVD, and cancer risk increased with amount of smoking. Intake of 1-7 drinks/week was associated with reduced risk of death, particularly CVD death (hazard ratio (HR): 0.7, 95% confidence interval (CI): 0.5, 1.0), whereas intake of N42 drinks/week was related to increased mortality, particularly cancerrelated death (HR: 1.7, 95% CI: 1.1, 2.5). The HR for total mortality associated with moderate alcohol consumption increased from 0.8 (95% CI: 0.6, 1.0) for non-smokers to 1.0 (0.9, 1.2) for moderate smokers and 1.4 (95% CI: 1.2, 1.7) for heavy smokers. Heavy drinkers and heavy smokers had the highest mortality (HR: 1.9, 95% CI: 1.6, 2.4). Conclusions. Light and moderate alcohol consumption reduced mortality from CVD. 189 midwives constituting 77% of all midwives working in that State.
FINDINGS:
midwives reported that they assessed smoking behaviour regularly (77%), addressed the consequences of smoking (70%) and advised women to quit. Among the midwives, 81% saw low chances of success and parents' expectations as the biggest barriers to counselling. Midwives reported that about 28% of women quit following their advice.
KEY CONCLUSIONS:
smoking and exposure to environmental tobacco smoke are seen as prominent health threats that midwives reported they addressed routinely, including giving advice to stop smoking.
IMPLICATIONS FOR PRACTICE:

The purpose of this study was to examine tobacco dependence treatment practices among certified nurse-midwives who treat pregnant women who smoke. Midwives were surveyed to determine knowledge, perceptions, and beliefs about tobacco cessation treatment and to identify practice environmental factors that support treatment practices. Half of all midwives had not heard of the US Public Health Service Guidelines (5 A's) to assist smokers in cessation treatment. We found varying levels of adherence to the clinical practice guidelines. Nearly all midwives routinely ask, advise, and assess; while fewer encourage patients to set a quit date or discuss medication options (assist) and perform follow-up activities (arrange). Barriers significantly associated with clinical tobacco treatment practice are lack of training and competing priorities in the visit. One-office support, a system in place to provide smoking cessation information and resources, was associated with increased practice. In summary, midwives believe they should be providing tobacco dependence treatment, yet they identify a need for training. The findings of this study also indicate that sustained practice change, which includes the entire practice environment, should be targeted to enhance tobacco dependence treatment. [TABAC] tabagisme:risque:facteur associé:alcoolIndex. décimale : TA 6.2.3.1 Substitution nicotinique Résumé : This article reviews and comments on studies of treatment and prevention of cigarette smoking in individuals with comorbid psychiatric and non-nicotine substance abuse disorders. Despite a high prevalence of cigarette smoking in these populations and interest in quitting, treatment interventions and studies of these interventions are sparse. Multiple barriers to implementation of interventions exist. Existing data suggest that provision of cigarette-smoking interventions in substance abuse treatment patients is efficacious and does not appear to interfere with abstinence from alcohol or illicit drugs, but more research is needed. There are few studies in populations with psychiatric disorders, with the exception of studies of individuals with a history of major depressive disorder. The available data suggest at least moderate efficacy and little evidence of exacerbation of these disorders. Integration of interventions into existing treatment clinics appears desirable. Despite the identification of subgroups that are especially likely to adopt cigarette smoking, there have been no targeted prevention efforts. Further research is recommended in both the treatment and prevention of cigarette smoking in individuals with psychiatric and substance abuse disorders. It is reasonable to offer existing treatments to these subgroups of smokers, since there is some evidence of efficacy and little evidence of harm.
